WHY CGM*?

of type 1 patients say they have altered insulin doses for fear of hypoglycemia.2

74% 78%of highs and lowsare missed byfingersticks alone.1

With Guardian Connect continuous glucose monitoring (CGM), you and your patients get easy access to better glucose data – for improved efficiency and better therapy adjustments.

Actionable data for therapy management

Gives doctors up to 288 glucose readings a day, revealing patterns not possible with only five daily blood glucose readings

Helps practices work more efficiently by reducing time spent on meter downloads and logbook reviews

With the Guardian Connect system, your patients know they have clear, complete diabetes information.

Advanced glucose management for patients

Offers patients convenient access to real-time glucose readings, right on their compatible iOS device

Alerts patients if they’re going high or low so they can take action. Your patients must calibrate twice a day and take a blood glucose test to confirm treatment decisions

Allows patients to share information with care partners remotely

WHY CGM*?

CareLink software gives doctors easy access to patient data. The app gives patients readings and trends on the go.

The Guardian Connect system is for patients using insulin injections who want to take the next step.

The new smartphone-based CGM system from Medtronic

the Guardian Connect system consists of a small, wearable CGM device your patients can use to check glucose levels on their compatible iOS device – so they can see highs and lows, and improve daily self-management.

Better ways to share information

Your patients can also use the Guardian Connect system with Carelink® software to share their glucose information online with caregivers and with your office. This can lead to more productive conversations for improved patient engagement.

* CGM (Continuous Glucose Monitoring) uses a special sensor to measure sugar levels just below the skin known as interstitial fluid. These sensor glucose (SG) values are different from blood glucose (BG) measurements using a BG meter. Sensor glucose values should not be used to make treatment decisions. Patients should always do a BG fingerstick before making treatment decisions.

Safety Information: CGMContinuous glucose monitoring systems are intended for monitoring interstitial glucose levels in persons with diabetes and should only be used under the direction of a healthcare professional familiar with the risks associated with these systems. These systems provide information regarding potential high and low glucose levels and can provide an indication of when patients should obtain a glucose reading using a home glucose reader. The information provided by continuous glucose monitoring systems is intended to supplement, not replace, readings from a home glucose meter. Insertion of a glucose sensor may cause bleeding or irritation at the insertion site. Consult a physician immediately if you experience significant pain or if you suspect that the site is infected. For details, visit MedtronicDiabetes.com/Important-Safety-Information.

Safety Information: CareLink® SoftwareCareLink software is intended for use as a tool to help manage diabetes. The purpose of the software is to take information transmitted from insulin pumps, glucose meters and continuous glucose monitoring systems, and turn it into CareLink reports. The reports provide information that can be used to identify trends and track daily activities such as carbohydrates consumed, meal times, insulin delivery, and glucose readings. CareLink report data is intended for use as an adjunct in the management of diabetes and NOT intended to be relied upon by itself. Nor should it be used for the treatment of medical conditions other than diabetes. Patient blood glucose levels should be monitored at least 4-6 times a day and patients and their healthcare providers familiar with the management of diabetes should consult one another prior to engaging in treatment or changes in treatment. See CareLink User Guide at medtronicdiabetes.com/support/download-library/user-guides for details.

The system includes a small transmitter and sensor the patient wears on their side. It transmits glucose readings every five minutes to an app on the patient’s iOS compatible device.

The Guardian Connect system can benefit patients who are:

n Looking to manage diabetes more easily and confidently

n Seeking to avoid hypoglycaemia

n Injecting insulin for the first time

n Pregnant or looking to become pregnant
