Continuous Clinical Practice: Ongoing Assessment Assessing Core Learning Outcomes Unit 1: 1, 2, 5, 6 Unit 2: 1, 2, 3, 4, 5, 6, 7, 8 Throughout the year you will be seeing clients, observing your peers and building on your clinical skills. This is your chance to practice what you have learnt in theory workshops, develop your skills as a clinician and reflect upon your experience of being a student healthcare practitioner. Keep all clinic forms for your final portfolio. Your supervisor will assess your case taking and clinic manner as per criteria on the Supervisor Feedback form, including demonstration of National Occupational Standards (NOS) and appropriate professional conduct. You are required to demonstrate the development and maintenance of safe and appropriate fitness to practice. The forms are a guide for your supervisor and yourself as to where you need to develop your abilities. You may also be evaluated and aided by an assistant supervisor. You will need a minimum of three clients of your own (including initial and follow up) to include a minimum of two completed cases with Fitness to Practice competence, in both the initial and follow-up appointments to pass your practical ongoing assessment by the end of the year. Please note family or friends including classmates cannot be used as part of your three client cases, but family or friends to be passed to a classmate. A maximum of five clients may be seen in clinic, subject to your competence and availability of appointments. Continue your reflective journaling from Nutrition 1 and attend to your action points regularly. Fulfilling action points will assist you in your clinical development. You must include your own specific action point aims in line with your developmental needs. Before the beginning of your consultation present your supervisor with:

• Your CNM Supervisor Feedback Booklet to enable your supervisor to monitor your progress. Find it in the Nutrition Clinic Handbook Appendix.

• A copy of your student clinic insurance certificate During the break in the consultation you will be required to present your supervisor with a synopsis including any Red Flags, naturopathic summary, therapeutic aims and your nutrition/supplement plan. Your supervisor must sign off on any therapeutic advice you give and will be assessing your competency in this regard. At this time please also present your signed Terms of Engagement & Consent Form to ensure you may use the case in your case study assignments. After the consultation your supervisor will give feedback and negotiate action points if necessary. This will be documented for your future clinical development. Be sure to reflect after each case in your reflective journal.

Observations:

In addition to your training whilst practicing you will also be learning whilst observing.

During observation you are required to fill in the:

• Nutrition 2 Clinical Skills Checklist Observing students will need to use this form for 9 separate consultations you watch and will only fill in 1 section for each consultation seen. Completed forms are kept in the portfolio. Use the first section for patient one, the second section for patient two and so on. This will allow you to become knowledgeable about the National Occupational Standards (NOS) and the core curriculum which you will be assessed against.

• Student Observation Reflection Form – 10 completed forms required for your Portfolio. These will help inform your Reflective Essay and PDP assessment as well as being a required element of your Portfolio assessment.

Unit 1 Clinical Experience

Assessment I: Case Study 1 and Case Study 2

Weight: 50% per case study

Assessing Core Learning Outcomes: 1, 2, 3, 4, 5, 6

You will be required to provide two written case studies: Case Study 1 will be one of the cases you have taken including the initial consultation and follow-up (2000 words) Case Study 2 will be a case study based on an observed case of a colleague or pre-recorded clinician online, which will be based on the initial consultation only (1000 words).

Your case studies must represent a variety of clinical examples (different genders, health complaints, age). Any case you use must have had the Terms of Engagement and Consent Form signed. Use initials only to refer to the client. Normal referencing rules apply throughout.

You are required to justify your approach to the cases with references for the Naturopathic Summary, Therapeutic Aims and Nutrition Plan sections. Your list of recommended reading, prescribed texts and any peer reviewed journals or published studies will be acceptable. Please refer to the CNM Referencing Guidelines for a more comprehensive referencing guide; your overall marks for each section will reflect the quality of your work and appropriate referencing. See your Journal Resource on the student website for ideas.

Case Study 1: Include the following criteria in the order specified below

• A synopsis of the case including; MYMOP, date of consult, presenting conditions, dietary strengths and weaknesses with assessment of nutritional needs, lifestyle factors, referrals required, current medication, supplements, drug nutrient interactions, anthropometric measurements, red flags.

• Naturopathic summary, demonstration of your naturopathic thinking showing analysis and evaluation of all factors of the case. Include the suspected drivers, triggers and contributory influences, systems under stress and their interrelated function. Discuss any conventional pathology and functional tests that might support or negate your hypothesis and appraise the level of evidence. The functional medicine model may be included if preferred.

• Therapeutic aims, which are specific and tie in with the naturopathic summary.

• Naturopathic Nutrition plan - demonstrating appropriate and safe practice. Include all advice given to the client; diet, lifestyle and supplement advice including brands suggested, dosage, and time of day it was to be taken. Include why you have made your suggestions using appropriate mechanistic rationale and support with a careful appraisal of primary research where available. Limitations to nutrition plan including negotiations with client, considerations such as client compliance and willingness can be mentioned here.

• Please note - A copy of the original CNM Case History form DOES NOT NEED

TO BE SUBMITTED but may be requested by the marker.

• Summary of progress - Discuss client’s progression since their last visit including their MYMOP, symptoms, health condition, how well they managed to comply with the nutrition plan, anything they didn’t manage to do and why, any new symptoms, revised nutritional needs if applicable, test results, medication, red flags, drug nutrient interactions, anthropometric measurements and any new priority.

• Revised naturopathic summary - Incorporate new information and include your independent research findings. You should be researching the case between consultations which will be reflected here.

• Formulation of updated and revised therapeutic aims.

• Formulation of an appropriate revised nutrition plan demonstrating appropriate and safe practice showing consideration of any new information to the student has analysed and evaluated the client case.

• Reflection on your experience as a practitioner with this client.

Pointers for producing a Case Study when reporting on your own client

• Imagine you are handing your case study over to a colleague who will be taking on your client, and they do not have access to your case history notes. All of the relevant information should be found within the case study, as well as your naturopathic thinking/hypothesis behind the scenes and the client’s reactions.

• We want it to be an accurate representation of what you did, but you may also modestly build upon it and show it in its best light. ‘In retrospect I would’ve looked into…’ or ‘my therapeutic aim was to balance blood sugar, but in retrospect I would’ve termed it improve insulin receptor sensitivity as this was my true aim’ would be an example.

• The main function is to serve to show what you have done, with supportive rationale and reference support as to why.

• Do not build a fantasy case, but if you feel you missed something important and would like to demonstrate that you have done further research, then do so and add it to the relevant section – with clear indication that this was not your original thinking.

• Your follow-up should not contain a lot of ‘in retrospect’ additions as you should be researching the case in between appointments and have had the chance to dig deeper into your naturopathic summary work.

• Research is relevant both in what you have already learnt about the support of various body systems, the references you utilised when preparing your first nutrition/supplement plan as well as the additional research you did between appointments.

• Look at your assessment descriptor AND the marking matrix. Remember that your marker will be guided by the marking matrix – make sure you fulfil the criteria for each section and pay attention to the sections where the marks are awarded.

Please use initials only to identify the client and the student practitioner/practitioner

Include the following criteria

• A synopsis of the case including - MYMOP, date of consult, presenting

conditions, dietary strengths and weaknesses with assessment of nutritional needs, lifestyle factors, referrals required, current medication, supplements, drug nutrient interactions, anthropometric measurements, red flags.

• Naturopathic summary - Demonstration of your naturopathic thinking showing analysis and evaluation of all factors of the case. Include the suspected drivers, triggers and contributory influences, systems under stress and their interrelated function. Consider any pathology testing or functional testing to help support or negate your working hypothesis and appraise the level of evidence. The functional medicine model may be included if preferred. This is your interpretation, not the practitioners.

• Therapeutic aims, which are specific and tie in with the naturopathic summary, again your own ideas please.

• Naturopathic Nutrition plan - demonstrating appropriate and safe practice. Include all advice that would be given to the client by you; diet, lifestyle and supplement advice including brands suggested, dosage, and time of day it was to be taken. Include why you have made your suggestions. Limitations to nutrition plan including negotiations with client, considerations such as client compliance and willingness can be mentioned here. * You may also add as an appendix your specific recommendations, recipes and handouts etc. as an appendix which would not form part of your word count.

• Reflection and discussion – Discuss what you would have done differently to the observed practitioner. Consider all aspects of the consultation and the alternative plan that you have created. Describe how your client would have benefitted your interpretation and understanding of the case, how you may have improved upon the therapeutic relationship, increased the likely compliance or buy-in of the client and anything else you feel important.

You are required to justify your approach to the cases using a careful and critical appraisal of the best available evidence, from research where available. This includes your Naturopathic Summary, Therapeutic Aims and Nutrition Plan sections. It also includes any laboratory tests your client brought, or which you selected. Please refer to the CNM Referencing Guidelines for a more comprehensive referencing guide; your overall marks for each section will reflect the quality of your work and appropriate referencing.

Advanced Case Study 3 and Advanced Case Study 4: 3000 words each

These will be from the cases you have taken including the initial consultation and follow-up. Your case studies must represent a variety of clinical examples (different genders, health complaints, age).

Any case you use must have had the Terms of Engagement & Consent Form signed. This should have been checked off by your clinic supervisor upon initial consultation. Use initials only to refer to clients.

Your case studies must include the following criteria in the order specified below:

• A synopsis of the case including; MYMOP, date of consult, presenting conditions, dietary strengths and weaknesses with assessment of nutritional needs, lifestyle factors, referrals required, current medication, supplements, drug nutrient interactions, anthropometric measurements, red flags. This is not a place for analysis of the case; it is for stating the pertinent facts.

• Naturopathic summary - Demonstration of your naturopathic thinking showing analysis and evaluation of all factors of the case. To include the suspected drivers, triggers and contributory influences. Systems under stress and their interrelated function. Consider any basic pathology testing or functional tests that may help to confirm, support or negate your thinking.

• Therapeutic aims - Which are specific and tie in with the naturopathic summary. Prioritise them in order of importance and discuss future considerations.

• Naturopathic Nutrition plan - Demonstrating appropriate and safe practice.

Include why you have made your suggestions using clear mechanistic rationale and support with primary research where available. Consider all aspects of the consultation and the alternative plan that you have created. Describe how your client would have benefitted your interpretation and understanding of the case, how you may have improved upon the therapeutic relationship, increased the likely compliance or buy-in of the client and anything else you feel important. Consider the use of reflective academic models e.g. John’s or Schon’s reflective cycles to help focus your reflective thinking (see Clinic Year 1 Case taking slides)

Assessment: Reflective Essay & Practitioner Development Plan (PDP) Weight: Pass/Fail Word count: 2000 Assessing Core Learning Outcomes: 1, 6, 7, 8 Submission: See your Lecture Timetable THIS MUST BE SUBMITTED IN AS PART OF THE CLINIC PORTFOLIO IT WILL NOT BE ACCEPTED INDIVIDUALLY Outline This assessment is designed to encourage you to reflect upon your practice and to develop your ability to analyse and critique the process of a naturopathic nutrition consultation by observing your fellow students and reflecting upon your own development. It is important that you explore and record your experiences in clinic and that you continue to keep up your reflective journaling (from Nutrition 1). This will enable you to consistently reflect on your progression and limitations as a practitioner and will expose your strengths and weaknesses. Reflection should be ongoing process, part of your life-long learning and continual professional development following graduation as a naturopathic nutritional therapist. Reflection is not re-telling all that happened in a consultation. It is about the clinical skills used during a consultation. It is a chance for you to focus on what is working and why; for exploring what doesn’t work very well and what you might expect to gain from doing things differently. Academic skills can come into it, but keep your focus from a clinical perspective – for example; Explanations? Application of knowledge? Client friendly language? Product knowledge? Offering alternatives? Styles of questioning? Please ensure you expand from your Year 1 Reflective Essay to include at least 3-4 relevant references. Instructions Part A You will be required to fill out a particular section of your Nutrition 2 Clinical Skills Checklist, per observation which is against the NOS and core curriculum requirements. Doing so for each consultation will guide you how to best approach your consultations from a professional perspective. Continued journaling, observation and reflection will build upon the generation of action points from first year. Reflection upon what your classmates are doing well, or could improve will also provide valuable learning. Part B Utilising the information you have gathered by fulfilling Part A you will be able to produce a relevant self-critique across the three key areas, personal, academic and clinical, this reflective essay will feed into a practitioner development plan (PDP) with Action Points. Your PDP will demonstrate your commitment to ongoing learning; promoting independent, safe and effective practice. A timeline or chart of action points can be added as an appendix for better utilisation of word count.

In order to create your own PDP, you will need to follow the four stages below: The Four Stages of a PDP:

1. Assess learning needs Work out which areas you wish to develop in the coming year. These can be divided into three key areas; academic, personal and professional. Reflect on where you are now, assess your learning needs in these areas and prioritise what is most important. Example; I have a good understanding about the symptoms and different tests available and associated with osteoporosis but on reflection at my past consultation I lacked knowledge about the different naturopathic nutrition options for osteoporosis- which foods, nutrients and nutrient levels are appropriate to use. It is important that I research all of these areas above.

2. Write a learning PDP plan for the coming year following graduation (i.e. 12 months).

Your PDP plan should be tailored to meet your learning needs. The plan should be SMART: Specific, Measurable, Achievable, Realistic, and Time specific (that is, it should be done within a given time). This is basically working out what you need to do to be able to achieve your learning needs once you have completed your training. Example; Read the chapter on osteoporosis in the Textbook of Naturopathic Medicine – complete before September the 8th. Attend seminar by **** on osteoporosis and bone health on the 8th September Create a handout on foods that are beneficial for osteoporosis by October 15th Research different supplements designed for osteoporosis to broaden my knowledge of what is available. Look at six different supplement companies by 10th November. Create a summary of my findings for quick reference by November 10th

3. Implement the plan

This part consists of carrying out your plan (attending courses, reading, attending meetings, discussions with other practitioners, and other academic development such as creating handouts or practicing computer IT skills).

4. Review the plan At the end of the year or whenever you decide is most relevant, assess and reflect on whether you met your learning needs and identify if further action is necessary.

Assessment: Clinic Portfolio

Your Clinic Portfolio provides evidence to show you have met the criteria of the course and contains all of your CNM clinical work.

After graduation you are expected to continue adding to your portfolio; items such as menu plans, diet diary sheets, substitution information, certification, supplement information and the like, as part of your continuing professional development as a practitioner. Seminars to enrich your clinical knowledge Attending seminars for CPD is an essential requirement as a practitioner and this process begins in year 2. These must be related to nutrition and of a correct academic pitch. For example, public talks are not acceptable, neither are yoga seminars or business management courses or such like. CPD are described as the learning activities professionals engage in to develop and enhance their abilities. It enables learning to become conscious and proactive, rather than passive and reactive. Please plan your year accordingly as leaving this component of your portfolio to the end of the academic year, could prevent you from graduating Do not leave this task till the end of the academic year.
