HERE'S HOW IT WORKS • To qualify for the rebate, customers may purchase any or

all of the items listed below and receive cash back with a limit of two per model on eligible products.

• Items must be purchased from authorized US-based Tele Vue Optics, Inc. dealers between June 01, 2018 and July 31, 2018.

• The original box label is your proof-of-purchase. You must send in the original printed label off each item. For labels on Nagler Zoom eyepieces, cut out the metallic letters “Nagler Zoom” only.

• Only one form is required to redeem all rebates. • This complete form, original US-based dealer sales

receipts, and original box labels must postmarked by August 31, 2018.

• Upon verification of the transaction, a check in US dollars will be mailed to you within eight weeks of receipt by Tele Vue.

ADDITIONAL RULES AND EXCLUSIONS • Rebate requests will not be honored

without a correctly filled out rebate form, original sales receipts, and original box labels.

• Offer not valid on refurbished, used or demo Tele Vue products.

Print this form, gather the receipts for all items indicated above (purchase date must be from June 01, 2018 to July 31, 2018), remove the original box label from each item (for Nagler Zoom, cut out the metallic letters "Nagler Zoom" only) and tape to the back of the printed form. Put all items in an envelope and mail to the address at right.
