Program DescriptionProgram DescriptionProgram DescriptionProgram DescriptionProgram DescriptionSince the launch of the i-LIMB Hand in July 2007,more than 150 patients worldwide have been fitted, and patients and prosthetists alike marvel at thetechnological leap represented by the i-LIMB Hand, the first advanced prosthetic hand to have multipleadvanced grip patterns, more like that of a humanhand.

The i-LIMB is the world’s first microprocessorcontrolled, myo-electric hand with proportionalcontrol, four articulating digits and a thumb whichrotates 110 degrees. This gives the user a “palmergrip” which enables the user to have grasp andcontrol which heretofore was not available. The i-LIMB can lift 45 pounds and yet the articulatingfingers produce a realistic movement, so much sothat the small size is acceptable to an adultfemale.

The hand’s function and more natural move-ment give significant benefits to the user along witha unique aesthetic movement and appeal.Today’s session will provide an in depth look at the i-LIMB hand. This will include aspects of functionality, design, and patient considerations.

Program DescriptionProgram DescriptionProgram DescriptionProgram DescriptionProgram DescriptionStance Control Knee Ankle Foot Orthoses (SC-KAFO)became a viable clinical option approximately fouryears ago through the commercial availability ofdedicated component systems. In that time period, the application of the technology has proven to besuccessful in the management of patients withlower limb gait deficits in the large and obviousetiologies such as polio, spinal cord injury and CVA.In the same time period, another group of patientshas emerged with similar lower limb deficits, butthese patients have presented with etiologies thatwere not evident at the clinical inception of thetechnology. This paper will present a retrospectiveutilization review of delivered systems with a per-centage break down into diagnostic groups. Thereview will also discuss stratification of specific SCcomponent features with respect to the patientphysical deficits as they pertain to each of thediagnostic groups.

Learning Outcomes:• Learn a rule-out method of selecting SC

componentry• Realize new categories of SCO clinical

applications• Review recently introduced SCO

componentryIt has been approximately four years since the

wide spread clinical application of stance controlknee ankle foot orthosis. In that time, the applicationof the technology has progressed from obviousetiologies such as polio and spinal cord injury toinclude smaller diagnostic groups. This paper willreview some of the recent trends to serve patientpopulations with smaller rates of incidence withstance control technology.

Patients who where survivors of the polioepidemic in the United States in the 1940s and1950s were targeted as a beta group for theapplication of stance control technology. A deliverydata base review indicates that this group wascorrectly targeted. A second beta group, spinalcord injury (SCI) has also proven to be a correctprediction. There has been a flurry of smallergroups that have benefited from the technology.Patients who have suffered an iatrogenic injury tolumbar spinal roots have emerged as a significantgroup. Patients who have a myopathy such asInclusion Body Myositis and Muscular Sclerosis haveeach benefited from the technology.

Patients who have under gone knee and hiparthroplasty with less than perfect results have alsobenefited from this technology.

The paper will review the aforementionedclinical applications as well as a review of compo-nent matching to specific physical deficits

Program DescriptionProgram DescriptionProgram DescriptionProgram DescriptionProgram DescriptionUsing a constant source of elevated vacuum inlower limb prosthetic suspension provides benefitssuch as increased proprioception, unrestrictedvascular flow, reduced tissue breakdown, andimproved gait. While the benefits are certainlynoteworthy in transtibial applications, they are evenmore significant in transfemoral applications. Thesocket can be designed with a lower trim line thanis possible in standard transfemoral socket design, resulting in substantially increased patient comfort, security, and range of motion. This presentation willexplain how to create a transfemoral socket withmicroprocessor-controlled elevated vacuumsuspension, and will review case studies fromgeriatrics as well as higher-activity amputees.

Program DescriptionProgram DescriptionProgram DescriptionProgram DescriptionProgram DescriptionHuman ambulation, or gait, is one of the basiccomponents of independent function. One objec-tive within the O&P profession is to help patientsimprove or restore ambulatory status, and thusachieve functional independence within the limitsof their particular impairment.

Gait analysis allows the O&P practitioner to:• Compare a patient’s performance to normal

gait parameters, while documentingbaseline gait patterns prior to intervention

• Refine proper alignment and fit of a pros-thetic or orthotic

• Evaluate the effectiveness, function andefficiency of one or more devices

• Satisfying a patient’s desire to understandhis/her options

• Justify treatment decisions and documentoutcomes with objective measures perevidence based directives

It is the purpose of this session to review andcompare practical gait analysis techniques andprovide examples of how these options can beintegrated into an O&P practice.

Biographical StatementBiographical StatementBiographical StatementBiographical StatementBiographical StatementDan Shelby is a sales and service associate for theGAITRite Portable Gait Analysis System, and Presi-dent of Kinesio-Logic; his sales and consultingcompany based in Greenville, SC. Dan brings aunique perspective on analyzing ambulationefficiency and function.

Previously, Dan spent 10 years selling andsupporting metabolic energy expenditure mea-surement instruments. In a gait lab this equipmentis used to determine biological energy costs ofambulation, a key measure of gait efficiency.

Dan has a Master ’s of Science in ExercisePhysiology, and is an avid runner and competitivecyclist. He has benefited greatly from FunctionalFoot Orthotics since the age of 14.

Program DescriptionProgram DescriptionProgram DescriptionProgram DescriptionProgram DescriptionThe development of CAD/CAM in the O&P industrycontinues at an amazing rate. The use of highlyaccurate, optical scanning devices has enhancedthe CAD prosthetic solution to now create successfulresults for upper extremity amputees and also allowclinicians to create truly custom socket interfaces. On the orthotic front, CAD solutions for AFOs, cranialhelmets, insoles, knee braces, and spinal bracescontinue to grow and improve. With all clinicalfunctionality, CAD provides important documenta-tion for payers while allowing clinicians to spendmore time with patients and less time on fabricationtasks.

A pediatric physiatrist’s review of range ofmotion and manual muscle testing will be demon-strated and a synopsis of current treatment modali-ties associated with spasticity in cerebral palsy willbe reviewed. As an orthotist, being an effectivemember of the rehab team requires an intricateknowledge in the realms of normal and pathologi-cal gait mechanics, normal anatomy, as well asthe etiology, classifications, and common deformi-ties/gait abnormalities seen in the CP patientpopulation. This will be reviewed. We will also discussappropriate orthotic recommendations for varioussubgroups in the CP patient population.
