Making Changes During the YearGenerally, you can only change your benefit elections during the annual benefits Open Enrollment Period. A Qualifying Life Event (QLE) such as marriage, divorce, birth, adoption, dependent age, and the open enrollment of your spouse’s employer may provide the ability to make a change. You must notify Human Resources with 31 days of any QLE to make changes. Otherwise, you’ll have to wait until the next open enrollment period. Any changes you make to your benefit plans must be directly related to the life event. Proof of the change is required (example: a marriage license or birth certificate).

When Coverage EndsMost benefits end at termination or the last day of the month following last day worked, QLE, or dependent age limit reached. However, under certain circumstances, you may continue your health care benefits through COBRA.

Coverage

Life, AD&D, and disability insurance aren’t

something you think about everyday, but

they should be an important part of

everybody’s financial planning – no matter

what your age. Your benefit program

offers the following coverages to help

protect you and your family when it’s

needed most:

Basic Life and AD&D (company-

paid)

The basic life benefit amount is equal

to two times the employee’s base

annual salary to a maximum of

$340,000. AD&D insurance provides a

benefit to you or your beneficiary if,

due to an accident, you suffer loss of

life or limb.

Supplemental Life and AD&D

(employee-paid)

Employees may purchase one or two

times their base annual salary up to a

maximum of $340,000.

Long Term Disability (company-

paid)

Long- term disability (LTD) coverage

provides up to 60% of base salary per

In fact, based on your situation, you may be able to save money by adding a supplemental plan to a lower cost medical plan. Be sure to consider your anticipated medical needs for the year along with the cost of the medical plans available to you.

AccidentProvides cash benefits in cases of eligible accidental injuries to help pay for uncovered medical expenses like your deductible or coinsurance, or for ongoing living expenses.

Critical IllnessProtects against the financial impact of certain illnesses, such as a heart attack or cancer. You receive a lump-sum benefit to pay for your treatment costs or for everyday living expenses like special transportation services or daycare.

Hospital IndemnityProvides cash payments for eligible hospital stays that you can use to cover expenses your medical plan does not cover such as deductibles, coinsurance, and other out-of-pocket costs.

Pet InsuranceYou work hard to provide your family with everything they need. So whether your family includes kids with two feet or kids with four paws, you know what responsibility looks like.

How to EnrollColumbus State’s Benefit Platform allows easy enrollment with built in decision support. This

simplified shopping experience will help you make informed decisions with confidence when

electing coverage.

Step 1 – Before Enrolling

• Gather all your dependent information. Legal name, social security number,

and date of birth.

• Children – the birth certificate or adoption papers

• Spouse – marriage certificate plus your most recent 1040, the top

half that shows marital status

• Domestic partner – notarized domestic affidavit

Step 2 – Enroll Online• Log on information:

• https://csccbenefits.hrintouch.com

• Create your user name and password. (This will be used for the website and

the APP. Once the website is complete, you may download the APP for

access on any smart device.)

Step 3 – Finalize Enrollment• Once your enrollment is completed, click on the Benefit Detail Report to

verify that you completed all sections of the enrollment process. This report

will show you all the benefits elected, the level of benefit, and each

dependent enrolled by benefit level.
