Attachment “A” to RFP: Selective Demolition Cornerstone Architects Project #:12.114 Package #: 0201 CWD-50 LOUIS RENOVATION SELECTIVE DEMOLITION SCOPE OF WORK Furnish and/or install all materials, labor, equipment and supervision necessary to complete the following scope of work for the above referenced project. All work must be completed in strict accordance with the plans and specifications prepared by Cornerstone Architects project #12.114 This work will be completed on a lump sum basis and includes but is not limited to the following items: 1. Roof Area: Remove the three existing penthouse structures completely below the roof deck level. Remove the stairs , coping , railing ,etc per plans. The removal is to be phased , see the project schedules 2. Entry Lobby and Lower Lobby: Remove ceilings, walls, floor coverings, fixtures, laminate tops, etc. as shown on the project drawings. The brick at the lobby east stairway that is scheduled to be removed must be salvaged for re-use and turned over to ORIONs.Superintendent.to be stored on site. 3. CWD-50 LOUIS RENOVATION SELECTIVE DEMOLITION

SCOPE OF WORK

Furnish and/or install all materials, labor, equipment and supervision necessary to complete the following scope of work for the above referenced project. All work must be completed in strict accordance with the plans and specifications prepared by Cornerstone Architects project #12.114 This work will be completed on a lump sum basis and includes but is not limited to the following items:

1. Roof Area: Remove the three existing penthouse structures completely below the roof deck level. Remove the stairs , coping , railing ,etc per plans. The removal is to be phased , see the project schedules

2. Entry Lobby and Lower Lobby: Remove ceilings, walls, floor coverings, fixtures, laminate tops, etc. as shown on the project drawings. The brick at the lobby east stairway that is scheduled to be removed must be salvaged for re-use and turned over to ORIONs.Superintendent.to be stored on site.

3. The existing Entry Lobby glass window and entry door systems will be removed by the Glass and Glazing contractor.

4. Parking Ramp Connector: Cut and remove the existing floorcovering, deck and framing and stairway to allow installation of the new ramp structure as detailed on the drawings.

5. Parking Ramp Connector:Cut and remove the required opening through the exterior masonry wall for the exit door to the ramp.

6. Service Area.Remove the masonry shaftway and other items on the 1st,2nd , 3rd, 4th and 5th floors per plans. The freight elevator and equipment will be removed by others.

7. Core Demolition-Remove all walls, ceilings, fixtures, etc. on the 2nd , 3rd, 4th and 5th floors.

8. 5th Floor Adjacent to Connector: Remove existing toilet room and all adjacent construction per plan.

9. Furnish company safety manual, certificate of workers compensation

insurance and liability insurance to the job superintendent prior to starting work.

10. Orion will require as a condition to making any payment to Subcontractor, that Subcontractor furnish a Sworn Statement listing all its subcontractors, suppliers, and material, waivers of lien, and such other evidence satisfactory to Orion that persons who may be entitled to assert such liens or claims have been paid.

11. Furnish all necessary taxes, insurance and signed contract (payment will not be made if these items are not supplied to Orion Construction).

13. All work is to comply with Orion Constructions safety manual. 14. Comply with all federal, state and local codes and ordinances including

OSHA and MIOSHA. 15. Clean up debris and place in the trash receptacle to be provided by Orion. 16. If clean up is not performed on a daily basis, Orion Construction reserves

the right to hire the clean up and back charge the subcontractor at a rate of $75/per hour.

17. Subcontractor is to provide enough equipment and manpower to maintain the agreed upon schedule. If behind the agreed upon schedule the subcontractor will work over time on weekends at their expense to maintain schedule.

18. Subcontractor is responsible for scheduling all inspections required by the City, County, and State. Be on site during inspections of the work you performed.

19. Subcontractor is required to complete the work that is involved in their trade and is required to complete the job as directed by any inspector or City Official which is common practice in the profession and area where the work is taking place but may not be part of the plans.

20. All potential change orders to be approved by Orion Project Manager prior to the work being performed and/or materials ordered. Orion site Superintendent does not have the authority to authorize change orders.

21. Dumpsters will be furnished by ORION 22. Subcontractor shall remain in contact with the superintendent to review

scheduling on a continual basis. Any problems are to be relayed to Orion Construction’s superintendent immediately.

CWD – 50 LOUIS RENOVATION CONCRETE

SCOPE OF WORK

Furnish and/or install all materials, labor, equipment and supervision necessary to complete the following scope of work for the above referenced project. All work must be completed in strict accordance with the plans and specifications prepared by Cornerstone Architects project # 12.114 This work will be completed on a lump sum basis and includes but is not limited to the following items:

1. Entry Lobby:Remove and replace the sidewalk in the exterior door and window openings at the Entry Lobby as per plans. Install new concrete window sills at the west wall per plans.

2. Fifth Floor Ramp Connector:Furnish labor and materials to install the concrete infill at the Fifth floor ramp connector as shown on the plans.

3. Fire Stairway to Roof Lobb y Expansion:Furnish labor and materials to install the concrete infill in the new steel pan stairway expansion and landings from the 7th floor to the new elevator equipment room.

4. Furnish company safety manual, certificate of workers compensation insurance and liability insurance to the ORION prior to starting work

5. Orion will require as a condition to making any payment to Subcontractor, that Subcontractor furnish a Sworn Statement listing all its subcontractors, suppliers, and material, waivers of lien, and such other evidence satisfactory to Orion that persons who may be entitled to assert such liens or claims have been paid.

6. Furnish all necessary taxes, insurance and signed contract (payment will not be made if these items are not supplied to Orion Construction).

7. Furnish all permits, fees, assessments, licenses, etc. which may be required to complete this scope of work.

8. All work is to comply with Orion Constructions safety manual.

9. Comply with all federal, state and local codes and ordinances including OSHA and MIOSHA.

13. Provide project superintendent with close out documents within ten (10) days of completion of project. One (3) copies of each.

14. Subcontractor is to provide signed one (1) year warranty letter, as builts, Owner Manuals, etc. before retainage is paid.

15. Subcontractor is to provide enough equipment and manpower to maintain the agreed upon schedule. If behind the agreed upon schedule the subcontractor will work over time on weekends at their expense to re-gain lost time and maintain the schedule.

16. Subcontractor is responsible for scheduling all inspections required by the City, County, and State. Be on site during inspections of the work you performed.

17. Subcontractor is required to complete the work that is involved in their trade and is required to complete the job as directed by any inspector or City Official which is common practice in the profession and area where the work is taking place but may not be part of the plans.

23. Furnish all hoisting and pumping as required for your work.

24. Strip and store form work per direction of Project Superintendent. Clean all concrete and debris to dumpster at direction of Project Superintendent.

25. Furnish and install an approved concrete curing compound to all exposed concrete.

26. Contractor is Responsible for Excavation/Backfill

27. Contractor is to provide mix designs and rebar drawings within 5 days of award of contract.

28. Contractor is responsible for taking proper care/precaution of work including tenting for rain, heating for cold, frost blankets, etc

29. Pour Sequencing is to be approved or provided by the superintendent.

SCOPE OF WORK

1. New Membrane Roof system: Remove existing drains from removed roofing system. Install new roof drains and tie into existing plumbing drain system as required by roof plans.

2. Roof Lobby: Furnish and install rough and finish plumbing at the toilet rooms. Furnish and install roof drains per plans.

3. Core-Toilet Rooms 2nd, 3rd, 4th & 5th Floors: Remove existing sinks, faucets, supply and drain lines. Install new sinks, supply and drain lines. Caulking of the new plumbing fixtures is also included.

4. Front entry canopy: Install the drain and piping for the new flower box. 5. Lower Lobby &Toilet Room Renovation: Furnish and install new sinks.

9. Furnish all necessary taxes, insurance and signed contract (payment will not be made if these items are not supplied to Orion Construction).

10. Furnish all permits, fees, assessments, licenses, etc. which may be required to complete this scope of work.

11. All work is to comply with Orion Constructions safety manual. 12. Comply with all federal, state and local codes and ordinances including

OSHA and MIOSHA. 13. Clean up debris and place in the trash receptacle to be provided by

others. 14. If clean up is not performed on a daily basis, Orion Construction reserves

CWD – 50 LOUIS RENOVATION ELECTRICAL

SCOPE OF WORK

Furnish and/or install all materials, labor, equipment and supervision necessary to complete the following scope of work for the above referenced project. All work must be completed in strict accordance with the plans and specifications prepared by Cornerstone Architects Project #12.114 This work will be completed on a lump sum basis and includes but is not limited to the following items:

1. Remove and relocate the existing electrical equipment for the chiller relocation.

2. Remove and terminate service to the abandoned electrical equipment per the project drawings. All abandoned electrical equipment shall be removed from the project site.

3. Disconnect and reconnect all mechanical equipment scheduled to remain on the roof to facilitate the new roof system installation.See the project schedules for required coordination.

4. Provide new electrical switch gear and MDP for the new elevator systems as required.

5. Provide new lighting fixtures, wiring, piping and circuiting as required for the New Roof Top penthouse & deck, The Core Areas,Toilet Rooms and Lobby renovations, the Entry and lower Lobby renovations and the proposed Entry Canopy.

6. This project is design- build and must meet all applicable codes and requirements . Furnishing sealed Electrical drawings for the electrical permit is required.
