Park Lawn services reach out to the heart of southwest metropolitan Chicago and suburban areas. Programs are diverse and address the needs of individuals within a wide range of intellectual and developmental disabilities from mild to profound, many having secondary disabilities and/or health issues. Park Lawn provides service without regard to sex, race, religion or national origin. Program participants are admitted solely on the basis that Park Lawn has personnel and facilities to meet their needs.Currently over 590 individuals with intellectual and developmental disabilities receive the servicesthey need to lead active and productive lives.

Fiscal Year 2011/2012 Highlights

• More than 577 individuals with intellectual and developmental disabilities benefit from Park Lawn’s programs and services.

• More than 480,000 Christmas tree bags were packaged through Park Lawn Services.

• 136 additional families were served in the DayProgram under the Building Bridges to the FutureProgram collaboration to better understand optionsfor their student with special needs after highschool.

• 12 families received day program services through a low cost private pay structure.

• Two new companies employed Park Lawn participantsthrough our Supported Employment Program totaling15 companies.

• Over 250 individuals volunteered their time in ParkLawn’s programs and with community events.

• Park Lawn’s Human Resource department reducedthe employee turnover rate from 31% in 2008 to24% in 2011, which is well below the industrialaverage.

Who we are,What We Do.

We believe that people with disabilities earnestly wish tolive a normal life. They want to go to school, hold a jobthat truly needs to be done, a place to live in comfort,and they want a full and active social life. Within this

context Park Lawn provides adult developmental training,vocational programs and residential services. All arebacked with professional staff support.

Park Lawn was founded in 1955 by a small group of parents from Evergreen Park and Oak Lawn who decided that their children diagnosed with intellectual anddevelopmental disabilities deserve better opportunities tohighlight their abilities. As their children grew and theirabilities increased, Park Lawn expanded its programs tomeet their needs. What began with meeting in localchurch basements has grown to 12 facilities throughoutthe southwest suburbs of Chicago. Today, more than 570 adults with intellectual and developmental disabilitiesover the age of 18 benefit from Park Lawn programs andresidential services.

IndependenceThrough Park Lawn’s VocationalServices, individuals enter a program and gain skills, dexterityand experience to continuouslyhave a goal to move to a job more challenging and suited to their interests and ability.

ChoiceIf a worker encounters difficulty performing their duties, adjusting to the environment, or has ahealth issue, they temporarilytransfer to a program that addresses their immediate needsand return to their job when ready.This flexibility maximizes the timespent in the work place.

Access to Community LivingAs individuals aged or becameorphaned, the need for residentialplacement became apparent. Park Lawn’s 10 residential facilitiesare attuned to the capabilities and interests of the residents.Whenever possible, under theguidance of caring staff, residentsparticipate in household activities,complete chores, and schedulesocial activities.

When one is diagnosed with a form of an intellectual anddevelopmental disability it means chronic physical or mentalimpairment as a result of natural or accidental causes. At Park Lawn, it does not matter what type of disability one mayhave, what matters is the commitment to the network ofresources available to promote independence for individualswith intellectual and developmental disabilities. Park Lawn’strained staff guides participants through each aspect of theirdaily lives doing tasks with them, not for or to them.

Park Lawn’s Developmental Training Program serves adultswith developmental disabilities on all levels of need and varying levels of independence. Park Lawn participants spend their days in both vocational training and functionalprogramming. Through these they receive goals toward moreindependence through Park Lawn‘s Developmental TrainingServices. The goals are basic for Park Lawn’s AdultDevelopmental Training (ADT) program, which serves adultswith significant intellectual and developmental disabilities considered to be pervasive and require ongoing supervisionand support. ADT participants engage in self care activitiessuch as functional living, communication skills, sensory-motor,leisure and recreational activities, community access and consulting supports such as pet therapy and music therapy.

Individuals that have accomplished the basic goals areencouraged to move forward to Park Lawn’s VocationalServices. A team of highly qualified staff help program participants with their individual goals to develop useful job skills, good work habits and the ability to complete jobassignments within required timeframes. Through this trainingthey derive a great deal of satisfaction from fulfilling sub-contract work for companies such as Nordstrom, MurphyBeds, TEC Tool, Dermaide and others. They complete a variety of jobs including document shredding, packaging, heat sealing, display assembly, collating, sorting and more. In return, they receive a salary with benefits of paid vacations,sick days and holidays. Classes are incorporated into thework day and assist workshop participants in coping with daily situations and relationships. Time management, grooming, and getting along with co-workers are a few of the topics covered to help achieve their individual goals. The primary goal for Park Lawn’s Vocational Services is toimprove functional and social skills that lead to greater self-sufficiency and independence.

Park Lawn Promotes ChoiceIn order for Park Lawn participants to better understand their abilities in life, they need choices.Park Lawn’s Supported Employment Program (SEP)and Community Employment Services (CES) are programs that give individuals a choice to work intheir local community. Some positions held includeclerical, data entry, food service, janitorial and cleaning,assembly, mail processing and production work. SEPand CES are mutually beneficial because businessesgain qualified and dedicated employees; while workersgain self-esteem and the satisfaction that they are acontributing force within their communities.

When Park Lawn participants are ready for SEP, andwaiting for employment, they have the choice to volunteer in the community to keep their skills sharp.Local non profits and businesses that benefit fromtheir commitment include Share Your Soles, Toy BoxConnection, Children’s Museum in Oak Lawn, PalosLibrary, St. Xavier University, Alsip Park District andmore. Program participants gain satisfaction knowingthat their hard work helps organizations in the community.

Eligible students with special needs from a local highschool district receive an idea of the choices available

in the future through participation in Park Lawn’sHigh School Transition Program. During the district’ssummer session, Park Lawn’s vocational trainingdepartment staff provides a thirty day vocationalenrichment program working on a variety of skillsincluding academic, functional living and more. Most students participating in the summer sessioncontinue with Park Lawn throughout the school yearby attending vocational training services for half oftheir academic day. Upon graduation from high school,students may qualify for full time services throughthe many vocational options available at Park Lawn.

The need for active treatment continues throughoutthe many phases of each individual’s life. With this in mind, Park Lawn provides a choice through theGRACE Program that is for individuals currentlyenrolled in workshop programming and whose medical condition or advanced age make attendancein the vocational setting inappropriate. Park Lawn’semphasis on this program is to develop a new wayto assist older adults with intellectual and develop-mental disabilities. Activities include simple exercises,listening to music, reading the local news and more.The values of this program emphasize one’s respect,flexibility, stimulation, safety, dignity and validation.

Like most people, Park Lawn’s participants look forwardto going home after a day of work or program activities.Many individuals return to the nurturing environment oftheir family home. Others return to the comforts andcamaraderie at one of Park Lawn’s ten residential homes.

Park Lawn’s residential facilities are attuned to the capabilities and interests of the residents. Whenever possible, under the guidance of caring staff, residentsparticipate in household activities, complete chores, andschedule social activities.

Park Lawn Center (PLC) is an intermediate care facilitythat provides twenty-four hour supervision and medicalservices. It is home to forty-one individuals with profoundcognitive challenges and complex medical needs. Withstaff assistance, residents make choices within their livingenvironment and participate in leisure activities.

Fifteen people reside at Park Lawn Homes where

residents aided by staff, cooperatively complete householdduties such as meal planning and preparation, shopping,laundry, upkeep of their bedrooms and planning leisureactivities. Residents flourish in this environment gainingself-confidence and esteem.

Eight Community Integrated Living Arrangements(CILA) are owned and operated by Park Lawn. An averageof five to six residents lives in each of the single familyhomes which are located throughout the southwest suburbsof Chicago. CILAs provide the greatest opportunity forindependent living. With staff guidance, residents have a voice in decisions regarding the running of their household. They plan and prepare meals, clean andupkeep the property, and plan leisure and social activities.

Park Lawn also provides for Intermittent CILA. Suchindividuals receive part time support services while residing in their natural family home.

These combined financial statements are the responsibility of the Organization’s management. Our responsibility is to express an opinion on these financial statements based on our audit.

We conducted our audit in accordance with auditing standards generally accepted in the United States of America. Thosestandards require that we plan and perform the audit to obtain reasonable assurance about whether the combined financialstatements are free of material misstatement. An audit includes examining, on a test basis, evidence supporting the amountsand disclosures in the combined financial statements.
