Nutricia Australia has received overseas reports of the Flocare Infinity enteral feeding pump 'air in line' (AIL) alarm not always working when used with mixed or reconstituted powdered tube feeds. It is believed that undissolved particles can accumulate in the feeding line or form a film on the inner side of the tube around the pump sensors, which can interfere with the pump's air detection function.

HAZARDS

If this occurs, it may result in the delivery of excess air into the patient's stomach with resulting discomfort and/or vomiting. If the AIL alarm was being used as an indicator that the reservoir was empty (i.e. feeding was completed), follow-up therapy might be delayed. This delay could place critical care and volume-sensitive patients at risk.

If you or somebody you provide care for uses a Flocare Infinity pump please be alert to the issue.

If you use mixed or reconstituted powdered nutrition, Nutricia Australia recommends the following:

•The 'dose' setting should be used and programmed, rather than relying on the AIL signal alone. When the required volume or dose has been delivered, an 'end of dose' message will appear on the screen and the pump will beep, after which the carer can take action. Do not mute the audio alarm indicating 'end of dose' in these instances. When programming the 'dose' setting, the AIL alarm will remain functional.

Ensure that the volume of nutrition in the container is greater than the volume that is actually needed (i.e. more than the set dose) to prevent excessive air bubbles in the feeding line.

•Ensure the mixed or reconstituted powdered nutrition is prepared and dissolved properly to avoid particles accumulating in the feeding set, potentially hampering pump performance and a normal feeding regimen.

The detachable wall plug could split, exposing a live metallic contact.

HAZARDS

A live metallic contact in the wall plug creates a risk of electric shock.

Consumers should stop using the affected adapters immediately. If the wall plug splits while inserted into a wall outlet, do not attempt to remove it until power to that outlet has been switched off. Consumers can check whether they have an affected wall plug by separating the detachable plug from the power supply and looking for a white dot or etched markings on the plug (see photographs). This recall involves “Step-iT” activity wristbands, which come in two styles—“Activity Counter” and a motion-activated “Light-up Band.” The Activity Counter comes in translucent plastic orange, blue or green and features a digital screen that tracks a child’s steps or other movement. The Light-up Band comes in translucent plastic red, purple, or orange and blinks light with the child’s movement. Both styles of activity wristbands have a square face with the words “STEP-iT” printed on them and a button to depress and activate the wristband. The back of the square face contains the etched words “Made for McDonald’s.”

It was sold in a variety of colors along with the OnBoard 35 infant car seat. Safety 1st is imprinted on the front of the stroller tray. Model number TR314 is printed on a white label on the back of the stroller seat.

HAZARD:

The stroller tray folding mechanism can partially disengage on one side when used with an infant car seat attached to the stroller, posing a fall hazard.

INCIDENTS/INJURIES

The firm has received 30 reports of the front stroller tray that supports the infant car seat disengaging on one side. No injuries reported.
