O n behalf of the Community Hospice Foundation’s Board of Directors, I am honored to share our 2010 Report to the Community with you. The Community Hospice Foundation is grateful to all of you who made home health and hospice care possible for countless individuals and families throughout the area that are facing serious illnesses and end of life. The ThedaCare At Home Hospice program continues to thrive thanks to your financial support. This, along with the many programs we provide, such as massage, pet therapy and spiritual counseling, serves hospice patients well in an inspirational and loving living environment.

The Community Hospice Foundation helps support our community and individuals during one of the most difficult times in their lives. In 2010, the Foundation was pleased to support the construction of the new ThedaCare At Home studio apartment residence housed within The Heritage and Peabody Manor communities. The development of the studio residence has drawn continued positive reactions from patients, their families, and their friends.

It is through your continued giving towards this very much-needed type of care, that we are able to make projects likethis become a reality.

T he Community Hospice Foundation provided a $382,000 grant to ThedaCare At HomeTM for renovations to create a more patient-friendly hospice residence within The Heritage and Peabody Manor communities. The nine private, home-like and fully-furnished studio apartments include high-end amenities such as:

• Livingspacewithakitchenette,familygatheringspace, and sleeping area all beautifully decorated and designed to meet the resident’s needs;

• Handicapped-accessiblebathroomwithroll-in

shower;

• On-sitemeals,housekeeping,andlaundryservices;

• Spa/massageandothertherapyprogramsincluding pet and music therapy; and

• 24/7servicesprovidedbyamulti-disciplinaryteamof RNs, CNAs, chaplains and social workers.

“Traditionally, hospice has been about supporting those who wish to stay in their own homes,” explains Brett Marston, hospice supervisor for ThedaCareAt Home. “However, we have found that hospice care is really less about the location and more about the service. For someone who wants or needs long-term hospice care outside the home, ThedaCare’s studio apartment complex offers the best of both worlds.”

The goal for the studio apartment complex is to create an environment that feels like home rather than a hospital, while still providing the critical pain management and holistic-style of patient care and family support that are the hallmarks of hospice care.

“As Martson explains, “It’s amazing how much hospice care can improve those last few weeks or months of life for both the patient and their family.”

T he fourth annual Hospice Rocks! took place on Friday, September 10 at Riverview Country Club. Guests enjoyed a fun-filled evening which included a dinner buffet. Those attending also had several opportunities to participate in raffles during the evening. The cocktail hour featured the jazz sounds of The John Pfeiffer and Tom Powell Duo. Following brief remarks about the opening of the new ThedaCare Hospice Residence, friends of the Community Hospice Foundation enjoyed musical selections from the Easy Pieces Band.

O n October 12, the Community Hospice Foundation held it’s fifth annual fundraising event for hospice families in the Waupaca and surrounding area. Net proceeds from “Light Up the Night for Hospice – Hope, Care, and Comfort” totaled$11,640.

The evening’s program included a moving testimonial from Susan Schultz, who shared her family’s experience with the hospice program and the comforting care they received for her late husband, Gordon. Tom Johnson emceed the event, held at the Waupaca Ale House. Guests participated in a key raffle for a chance to open a box that held a beautiful necklace donated by Fletcher’s Jewelry. The proceeds from the fundraiser are used to support the advancement for hospice care in the Waupaca area and at surrounding communities.

Holiday Party – Dorothy Zanskas and Kay Lamberg

O n December 13, 2010, more than 160 ThedaCare At Home hospice volunteers and their spouses attended the annual volunteer holiday party celebrating over 10,000 hours of compassionate care given to hospice patients and their families during the year. “The volunteers look forward to the holiday party each year. We are very fortunate to have so many amazing volunteers who share their time and talents each and every day, and this is our opportunity to give something back to them,” commented Mary Betters, hospice volunteer coordinator. “Following the party, a hospice volunteer expressed to me that he felt the night truly had a ‘magical’ feeling due to the warm and caring people who gathered together to share their passion for providing care and support to hospice patients and their families,” she added. A grant from the Community Hospice Foundation provided funding for the event.




Gifts of Cash – Cash gifts make an immediate impact on patient programs, community education, and research initiatives. Gifts may be made in memory or honor of a friend or loved one.

Endowed Named Funds – Donors may establish endowed named funds to be administered to support a particular program or initiative with a gift of $10,000 or more, payable over three-to-five years. Endowed funds help ensure that important programs will be sustained into perpetuity.

Gifts of Appreciated Assets – Gifts of non-cash assets such as common stock can offer valuable tax advantages when compared with cash gifts. In addition to avoiding capital gains tax on the appreciated value of the stock, donors receive a tax deduction for the full cash value of their gift of stock.

Gifts that Return Income – Significant tax advantages make life income agreements such as charitable trusts and gift annuities an attractive way to offer the opportunity to receive income during a donor’s lifetime while ultimately supporting the caring mission of Community Hospice Foundation.

Gifts by Will – Simply inserting the desired numbers into the sentence,“I bequeath $____ or ____% of my estate to Community Hospice Foundation” will cost nothing today, but will help in the future, and reduce the donor’s estate taxes.
