Call to Order –Chair James Schlender called the meeting to order at 9:00 am. Roll Call taken. Quorum was met. Certification of Compliance with the open meeting law was met. Meeting Agenda Public Comment – Louise Ladenthin Minutes from Previous Meeting – A motion was made by Mr. Buckholtz to approve the minutes of the July 1, 2021, meeting; second by Mr. Schleeter noting a correction be made to read “Mr” Schleeter instead of “Ed” Schleeter in the motion to approve the June minutes. Motion carried without negative vote. Circuit Court Report – Mr. Hoff presented an update regarding ongoing meetings with Venture Architects and Miron Construction on the courthouse remodeling project. A design schematic was presented to the Courthouse Construction Ad Hoc Committee on the morning of August 5th. The Ad Hoc committee approved the schematic to move forward to the Public Works Committee and County Board meeting. This will allow the drawings to go to the next phase which is developing the budget for the plan. Clerk of Court Report – A written report was provided. Ms. Kelsey advised that the yearly restitution report showed that the County was able to send out over $49,000 in victim restitution this year, significantly more than the $22,000 sent out the previous year. The increase is accredited to dedicated collections within the office. District Attorney’s Office Report – Sheriff’s Department Report – Sheriff Mrotek stated that the County has not experienced a single fatal car crash to date, despite the busy summer. The office continues to have lots of communication with hound and bear hunting groups to try to reduce nuisance calls. Communication Center Report – Chief Sajdera reported that dispatch is short one full-time employee, and they have had an influx of 911 hang-up calls, representing approximately 6% of total calls. Patrol Report – Lieutenant Ripczinski advised that there were 1356 calls this month, up by 300 calls over last month. A large number of the calls were the 911 hang-ups. They are seeking applicants for a part-time patrol position. Jail Report – Lieutenant Johnson advised that jail numbers have been increasing and pre-sentence incarcerations are the highest they’ve been in a while. The Dodge Correctional facility is now taking inmates and we transported inmates to that facility last week. The jail staff is short one full-time male position and are looking for part-time help, as well.

Animal Control Report – A written report was provided. Ms. Shelton reported having responded to a rising number of wild life calls, as the DNR hasn’t been available or appear to no longer be responding to these types of calls. Jail Tour – A county board member tour of the jail will be available at 6:00 pm on August 19th prior to County Board meeting. Coroner’s report – A written report was provided. Emergency Management Department Reports – A written report was provided. Criminal Justice Coordinating Council Update – The CJCC committee did not meet in July and the next meeting is August 11, 2021, at 12:00 pm. Ambulance Service Department report – A written report was provided. Mr. Hoff will refer the ambulance stationing concern brought up in public comment to corporate counsel to begin reviewing policy on coverage options. Sawyer County Communications System Specialist – July 2021 PS Committee Report

1.) Sawyer County Microwave – The aspects of moving to a microwave completely owned and

operated by Sawyer County has been researched. A planned microwave would encompass a link

from the Courthouse to the Tower site near the casino. It would then spur or branch off to two

sites locally and a third site down to Pipestone. From Pipestone it would branch off to Moose

Lake, Meteor, and Winter. Finally, from Moose Lake it would go towards Loretta/Draper and

allow connection with the main radio sites. This system would also allow internet to be passed

through to help provide high speed internet to locations that need it for county usage (Hwy, EM,

etc.)

Cost for these sites and equipment would be in the $60,000-$70,000 range approx. and could be

leased over a three year period or purchased outright. This would then eliminate the need for IP

backhaul or connectivity from ISP/telephone companies. The assurance that ISP/telephone

companies made for the connections has deteriorated over the last few years. When the

ISP/telephone companies have been asked about the quality they have since returned

responses of this is the best we can provide.

2.) Connors Lake/DNR partnership possibility – The DNR is looking for possibilities or options with

the Connors Lake Site. This is an old DNR fire tower, on WI State Land, that is no longer

structurally safe to be used for any purposes. Currently the DNR has a repeater for the DNR park

down in that area. Sawyer County many years ago placed two antennas on the structure and has

been providing a shelter (outdoor style radio cabinet) for the DNR repeater as part of the

agreement. Sawyer County had planned to use that site but with the changes and on/off again

aspect of the tower status it has never been populated.

The DNR has suggested a partnership with Sawyer County in looking at the current fire tower

being taken down and Sawyer County funding the cost of an approx. 180ft tower at the site.

Sawyer County would be guaranteed antenna space on that tower for the life of the tower.

Right now depending on construction, type, and setup of a tower life expectancy could range

from 30 to 50 years depending. IN turn the DNR would provide the needed radio shelter,

Generator, Power, and take on FULL responsibility for the tower maintenance as part of this

deal. The DNR has done this with another county in Wisconsin and is interested in seeing if

Sawyer County would like to have a meeting on the topic.

3.) WISCOM Busy within the last week – It was reported that within the last week, while deputies

were responding to a car accident, that a couple of them received ‘BUSY’ while attempting to

key-up. Upon receiving this notification I checked with the State of Wisconsin (WISCOM) who

checked the Hayward tower and radio ID’s. No reported issues came back with the towers or

radio ID’s. They did find that 120 key-ups did occur on the SWLAW1 talkgroup in one hour. This

is approx. one keyup every 30 seconds. Factoring in the average length of a transmission is

approx. 15 to 20 seconds (with some being shorter or longer) it is possible deputies were trying

to keyup over one another.

No other reports from other WISCOM users or agencies were received and this appears to be an

isolated incident. If this sort of situation was to occur again it is advisable to have dispatch notify

me right away in order to do real time checking.

4.) WISCOM RFI – WISCOM will be sending out RFP notices within the next couple of weeks for

submission by radio manufacturers for moving forward with WISCOM 2.0 (as it is being

referred). This will be a migration from VHF to 800 MHz for the entire state and has caused

some concerns for many users in the state who are in VHF user areas. While the RFP will cover

infrastructure and consoles it will NOT cover subscriber units (mobiles, portables, and base

stations). Subscriber equipment Sawyer County would need to purchase dual band radios. This

would be for both mobile and portable radios in order to allow for operation on the WISCOM

System as well as interoperability with Price, Rusk, Barron, Washburn, Burnett, and Parts of

Douglas and Bayfield Counties. This is due to the fact that the surrounding counties are VHF and

not 800 MHz system users.

As of todays pricing for budget figures the cost of a mobile radio, dual band, with installation

would be approx. $5,000. While the pricing for budget figures the cost of a portable radio, dual

band would be approx. $3,500. Based on a rough estimate of 100 ID’s (50 mobile/50 portable)

per department $250,000 for mobiles and $175,000 for a total of $425,000 in this case. This

does NOT factor in the additional tower locations that would be needed for coverage. 800 MHz

has less coverage than VHF and with three VHF sites in the county not providing sufficient

coverage the cost could well be over $2,000,000.00 for sites and upgrade radio equipment for

Sawyer County alone.

This is really based upon law enforcement users and not factoring EMS, Hwy, Transit, and Fire

Department subscriber costs. This is also impacted by the fact that no projection of grant

funding is known at this time to help offset costs.

5.) CIP – With the information about WISCOM looking to move to 800 MHz in the next three to five

years the CIP (Captial Improvement Project) has been worked on to reflect a migration off of

WISCOM for agencies, increase towers used, and increase channels used.

As of this point Sawyer County will utilize six towers for channels for Law Enforcement, Paging,

Fire/EMS, and Other Sawyer County radios users. With the five year plan with an approx. cost of

$1,200,000.00 (project actual cost might be less) it would increase the county to the following

for radio coverage: 8 towers sites in total; Law Enforcement = 2 channels, Paging, Fire/EMS = 2

channels, OPS = 3 county wide channels for all users, Hwy = 1 channel (four sites only), and

Transit = 1 channel (four sites only).

A slight delay in the exact numbers and plan has occurred due to the DNR item as listed above

which greatly impacts the one site and microwave linking.

6.) Additional work – Update radios with new firmware, and set-up programming of new radio.

Assisted with power issue with newly assigned squad. Assisted in radio ID for new LCO

Conservation squads.

• Patrol calls volume did drop from last month's report, June '21 was 1067, July '21 with 1356, while Aug'21 is 965. Which is also a decrease from Aug '20, which was 1159.

• Our deputies, in addition to our task force partners, have continued the HYE, under the WI BOTS seatbelt grant. Specifically, Sawyer County Deputies in this period have issued ll.. Citations for traffic offenses, Q_ written warnings and 1 Non-Traffic Citation. For Aug 2021 there have been 12 Total Crashes,! with Personal Injury, and Q Fatalities in our Counfy.

• We are still seeking new applicants for part-time patrol positions. Of those prospective applicants last month, one has advanced past the intial stages of hiring and has accepted a conditional offer of hire from us, and tentatively planned to start Field Training in mid­September. A second applicant in still in the background phase of selection.

• We recently assisted the Seely Lions Club with Pre Fat Tire, along wi:h the Radisson Harvestfest and will be assisting the annual Exeland Troutfest, Labor Day weekend.

Sawyer County Communications Center

Trainees Forren and Barthel are still in the training process and both doing well. Dispatch has received four applications and will be inviting selected applicants in for shadows in the near future.

We currently have one full time dispatch opening and will look at posting the position once our two trainees have completed training.

We have received concerns from emergency responders regarding damaged/faded/missing fire number signs and the difficulties locating some addresses when responding. We will be meeting with Land Records to discuss the issue.

Inter-facility Transports

Purpose: To assure that we are providing inter-facility transports in a safe manner from one facility to another.

Policy: Sawyer County EMS has a primary responsibility to staff the 911 service area with a Paramedic ambulance, legally inter-facility transports are secondary in priority to 911 coverage. The purpose of this policy is to define the appropriate use of EMS personnel utilized curing an inter-facility tlansfer,

This policy addresses the appropriate transfer of a patient by amJulance with an emergency medical condition, who has been stabilized by the transferring facility, accepted by the receiving facility and transported with appropriately qualified personnel. Prior to initi3ting any transfer, the transferring physician has the responsibility to perform a medical screening exam to determine if the patient has an emergency medical condition, stabilize (if possible) and prepare the patient for the transfer.

Transfers occur with the expectation that the reasonably anticipated risks and complications en-route shall be non-existent to minimal, but may be adequately handled by trrnsport team personnel

Prehospital providers that participate in inter-facility transfers raust follow written these written guidelines and have appropriate training for the particular type of patient tlaey will encounter. At all times operate within your scope of practice and training as approved by Medica_ Direction.

!fa hospital or clinic determines the patient is in need of inter-facility transport, the hospital shall call the Sawyer County paramedic who will arrange for the proper level :rew and equipment.

Transferring Physician/Facility: 1. Shall stabilize the patient within the capacity of the facility. 2. Perform a risk/benefit analysis of transfer. 3. Determine the mode of transfer based on patient acuity, dis:ance and weather conditions in

consultation with the receiving facility physician and EMS. 4. Determining the medical needs of the patient during transfE:r and the medical qualifications/level of the ~

personnel required to meet that need, 5. Recognizing and respecting the limitations of the provider'~ scope of practice, which means: ...,_

a. Not requiring a higher level of care from these pErsonnel than is defined by scope. b. Recognizing the need for specialty transport services: i.e. Neonate transport team. c. Assuring medical control physician may be reached by radio or phone during the transport.

Modes and Level of response: 1. The physician can request a response mode:

a. Emergency Response= If a patient at a health fac:lity has an emergency medical condition which needs to be evaluated or stabilized by a ph r1sician atthe receiving hospital in a rapid manor. Does not always mean lights and siren, orly an immediate response.

b. Non-emergency Response= A request for transport of a patient who does not have, or is not perceived to have, an emergency condition but needs ambulance transport to a higher level facility or for a procedure not currently available at the local hospital or clinic,

2. The level of response will be determined by the attending p 1ysician at the facility. The physician can request one of three levels of care: EMT, Advanced EMT, and Paramedic/Critical Care Paramedic.

3. Sawyer County EMS employees Registered Nurses, They mc.y operate to the Paramedic level of service, or if CEN or Critical Care certified to that level of service as determined by the Medical Director(s).

INTERFACIL!TY CONTINUED ... EMT /Medic Instructions:

1. Once at the hospital, obtain a verbal patient report from a11appropriate hospital staff, complete an inter­facility transport form (see forms section).

2. Collect the patient's records for the receiving hospital, incl.1ding any x-ray /CAT scan CD/MRI being transferred with the patient. Patient belongings being tran;ported (encourage family members to take belongings. Document what you accept and where it is left.at receiving hospital).

3. Collect a face sheet, PCS form, and insurance card and if n~essary have the patient sign the ABN. 4, Obtain and document a set of vitals prior to leaving the facility. 5. Transferring Physician remains medical control until recei-1ing facility signs PCR. 6. Document the patient's condition during transport. If patiET1t1s condition deteriorates or there is a

significant change, consider ALS intercept or diverting to t:le closest hospital. 7. Call in report to the appropriate receiving hospital.

ALS Monitorin(;! durio(ittransport: All patients being transported shall have an appropriate level of monitoring:

1. Continuous EKG monitoring if Paramedic or CCP transfer is ordered. 2. Intermittent measurement: Rate, Pulse rate, Continuous mtmitoring by pulse oximetry, Blood pressures,

etc. 3. Intubated patients receiving mechanical support ofventila:ion should have airway pressure monitored.

If a transport ventilator is used, it should have alarms to injicate disconnects or excessively high airway pressures. End tidal and wave form monitoring required.

4. A medical record documenting the patient's status and ma,agement during the transport is required.

Minimum e<wioment durini: transport: 1. All Sawyer County Ambulances are equipped per state reg.1lations with sufficient levels of stock for

transfers. All Staff shall be trained on all equipment within their scope of practice. Paramedics, CCP and RN's shall be knowledgeable on the use and trouble-shoothg of IV pumps, ventilators, etc.

2. Guidelines recognizes that even with anticipation ofpatien: needs during transport, situations will arise that cannot be foreseen. In this event, transport personnel :1re to initiate care based upon the written order while seeking consultation with on-line medical control and/or requesting an appropriate level intercept or diverting to an appropriate facility.

3. EMS Providers must assure the patient conditions fall with n their scope of practice or arrange for a higher-level provider to take the transfer. If patient conditi:ms warrant additional staff on the transfer for sufficient care of patient, the duty officer shall arrange fDr more appropriate staff.

4. EMS must document all vital signs and interventions/cond tion changes of patient performed by staff, 5. Training for the Sawyer County EMS is conducted by WisCJnsin Indian head Technical College, and

medical direction physicians. 6. On-line Medical Control is provided by the Hospital Emergency Room Physician.

Mutual Aid

Sawyer County EMS Requesting Mutual Aid from Other Agencie.: There are times when ambulances from outside the Sawyer Couaty EMS coverage area are needed: 1. All Sawyer County ambulances are used and additional ambulances are needed at that

same scene 2. All Sawyer County ambulances are already on separate call;; or out of service 3. Sawyer County EMS staffing is limited

Dispatch must be notified when mutual aid is desired. The crew 111ust specify the name ofth0 ambulance service desired. Typically, the nearest ambulance service is preferred. However, others may be requested based on resources available and required levels of training.

Department coverage area to stand-by for other calls or respons~ to the scene to free up one of our crews. The crew must specify to Dispatch where the ambulance should stage or respond. Generally, if the out of service time for Sawyer County is anticipated to be more than thirty min-1tes, the mutual aid ambulance should stage at the Sawyer County EMS stafun

In mass-casualty situations or more-complex situations beyond those described above, it is the responsibility of the Duty Officer/Lead EMT on a run to determine which addit:onal ambulance services are needed based on closest department and their staffing and runs at the time needed.

The Duty Officer shall thank and release the assisting units as socn as possible.

Sawyer County EMS Responding Outside the Normal ServiceArea:

Any time Sawyer County EMS is requested by another service to -espond out of our service area to assist them at a scene or stage at their station to cover their service area we :ntend to respond based on the following guidelines:

Mutual aid requests for Sawyer County EMS will be filled based oo available resources and personnel. Our primary goal is to ensure adequate emergency medical service ccverage to our own public service area (PSA).

The Duty Officer shall establish a Paramedic level on call crew for our PSA first. Mutual aid calls get the next highest level crew available,

Our crews shall return as soon as released by requesting agency.

Mandatory Medical Director Notification

Certified providers operating within Sawyer County EMS syste,1 practice under the authority of the Medical Director. As such, any incident which potentially has a• adverse or negative impact on the patient or system must be immediately reported to the Medical Direct(Jr, or in their absence his/her designee as soon as practical after the completion of the call.

The Medical Director is to be notified when any of the followin 5 occurs:

1. Pediatric Arrest or death. 2. Mass Casualty Incident (MCI). 3. Critical Incident Stress Debriefing (CISD) 4. Performed advanced procedures 5, Pericardiocentesis. 6. Surgical Airway, (Any attempt) 7. Rapid Sequence Induction. (RSI, Unable to secureairw~) 8. Injuries of personnel (that are transported to hospital) 9. System certified provider who has operated outside of !:.is/her level of certification and/or training.

Multi-Casualty Incident/MCI Plan

PURPOSE: The purpose of this plan is to provide guidance to a:nbulance and first responder services to ensure the capability of providing triage and transportation for at least 500 adult and pediatric patients per million population. (Pediatric patients usually account for 20%,ofthe total.)

Definition of a Mass Casualty Incident: An incident with multiple casualties that overwhelms the resources of Sawyer County EMS. An MCI cannot be defined by a set numbe:- of casualties. Due to variations in actual and potential resources (e.g., ambulances, EMT staffing, supplies, et.) and the nature ofan incident (e.g., natural disaster, haz-mat, MVC, etc.), the number of casualties will vary

MCI training will take place bi-annually. Sawyer County EMS maintains a supply of disaster supplies at the station and on ambulances able to respond to a SO person casu3lty situation. The service participates in county wide training and drills to maintain readiness.

Sawyer County EMS has adopted the full plan from the State of Wisconsin EMS Advisory Board EMS Special Operations Committee dated 10/2010, The entire plan can be found in the forms section at the end of this manual.

In addition to the responsibilities under s. DHS 110.34, an ambulance service provider licensed to provide interfacility transports shall satisfyall of the following requirements:

(1) The ambulance service provider shall ensure that interfacility transports do not interfere with its responsibility to provide 9-1-1 emergencyresponse in its primary service area, if it is also licensed as a 9-1-1 provider.

(2) The ambulance service provider shall assure proper staffing for interfacility transports based on the acuity of the patient, the orders of thesending physician and the staffing requirements in s. DHS 110.50.

(3) The ambulance service provider shall not use mutual aid agreements to cover its primary service area while providing interfacilitytransports.

(4) If the ambulance service provider is licensed as both a 9-1-1 provider and interfacility provider, the provider shall have a minimum of oneambulance for 9-1-1 emergency response and one ambulance for interfacility transports, unless the ambulance service provider has acoverage agreement with a neighboring ambulance service provider that will be able to provide one 9-1-1 ambulance for each primaryservice area.
