GERIATRIC CARE
MANAGEMENT… Demystifying “Aging
in America”
The Geriatric Care Manage-
ment program sponsored by
the School of Social Welfare
at Stony Brook University is
designed to help health care
professionals and senior
advocates better understand
the difficult day-to-day
challenges associated with
“aging in America.” Older
Americans, the fastest grow-
ing segment of the popula-
tion, must make a wide range
of social, health, financial,
legal and end-of-life deci-
sions. Most seniors are able
to make decisions for them-
selves, for others, this is no
longer possible. Either way,
the needs of older adults are
best served when an inter-
disciplinary team works
collaboratively for the benefit
of the older person.
