The Rehab Center at Thornapple Manor o�ers access to an unmatched continuum of care, including inpatient, transi-tional, outpatient and long-term care services.

Located in beautiful Barry County, along the Thor-napple River in Hastings, The Rehab Center at Thornapple Manor o�ers a central destination for not only local patients, but individuals traveling from Grand Rapids, Kalamazoo, Battle Creek and all of Western Michigan.

The Rehab Center at Thornapple Manor o�ers a facility designed to maximize a patient’s return to independence. Rehabilitation services take place in a comfortable environ-ment where patients can safely learn to manage day-to-day activities. We have a spacious gym with specialized rehabili-tation equipment used to execute your personalized treat-ment plan.

For patients that require an inpatient stay, we o�er private rooms with a full suite of amenities. To relax, we encourage patients to take a stroll down ‘Mainstreet’ — o�ering a Chapel, Salon, Café, Gift Shop and much more!

Since 1958, The Rehab Center at Thornapple Manor’s inte-grated patient care team has been providing patients with a breadth of expertise unrivaled in our area. We know rehabil-itation works best with a diverse team of experts, all working together.

Working in concert to develop a treatment plan, we are proud to o�er professionals that include physical therapists, physical therapist assistants, nurses, certified nursing assis-tants, occupational therapists, occupational therapist assis-tants and speech language pathologists.

In addition to our in-house team of rehabilitation experts, we work with the finest internal medicine, orthopedic sur-geons and family practice doctors in West Michigan — max-imizing each patient’s overall functional independence and recovery.

BEGINNING THE JOURNEY,ON THE ROAD TO RECOVERY.

A D M I S S I O N S

Being admitted to The Rehab Center at Thornapple Manor means you’ll become a patient that receives honest com-munication, respect and compassion.

Licensed professional therapists employed directly by Thor-napple Manor work with an interdisciplinary team to pro-vide complete one-on-one care. The entire team meets with you regularly to discuss your personalized treatment plan, goals and barriers to discharge. In addition, the Thor-napple Manor Rehabilitation Director leads formal team meetings that include physicians, therapy and nursing teams to discuss treatment objectives.

To begin the journey, a referral for rehabilitation care can be made by a patient, a family member, physicians, a case manager or a social worker. To initiate a referral, please contact our Admissions Department.

Families are encouraged to take an active role in their loved one’s rehabilitation process. Our goal is for each patient’s successful recovery, even after discharge.

RETURNING HOME, WITH CONFIDENCE.

After admission referral paperwork, a Thornapple Manor Admissions team member will get a patient settled in quick-ly and comfortably. There will be a number of forms to be signed and information to be shared, including the neces-sary consent for physicians, nurses and therapists to begin treatment. Having a family member and/or legal guardian accompany the patient will help to expedite the process. We also require patients to provide copies of insurance information so that we may confirm coverage and billing addresses.

S T A R T I N G T H E R O A D T O R E C O V E R Y

The Rehab Center at Thornapple Manor participates with Medicare and Medicaid and most major insurances, work-er’s compensation, auto and managed care plans. Current-ly, we contract with BCN of Michigan, Aetna, CIGNA, Merid-ian, United Healthcare, Priority Health and more.

I N S U R A N C E A C C E P T E D

Our interdisciplinary team conducts home evaluations to help ensure patients will be able to safely return home. The Social Services department also provides connections to community support to help ensure a smooth transition.

ENSURING PATIENTS NEVERSTOP PUSHING FORWARD.The Rehab Center at Thornapple Manor o�ers access to an unmatched continuum of care, including inpatient, transi-tional and outpatient services. From the time a patient is admitted to the hospital or immediately following an injury, until an individual is ready to receive outpatient care, the Rehab Center at Thornapple Manor is there for each patient throughout their rehabilitation journey.

We pride ourselves on cultivating personal relationships that evoke empathy and clearly communicate the goals of therapeutic tasks to ensure patient success. We encourage patients through a specialized color-coded recovery system of red, yellow and green. Red means a patient needs full assistance, yellow, transitional assistance and green, the patient is doing great and ready for discharge!

Rehabilitation with a variety of neuro-facilitation techniques for those recovering from neurological injuries such as Parkinson’s disease, traumatic brain injuries, or other neurological problems.

S T R O K E R E C O V E R Y / C V A

Customized treatment plan that emphasizes improving functional performance at home and in community set-tings. Patients train with adaptive Equipment to gain high levels of function.

L S V T ® B I G / ( P A R K I N S O N ’ S T H E R A P Y )

Our certified therapists provide LSVT® BIG training for individuals with Parkinson’s disease and other neurogenic disorders a�ecting movement and gait. V E S T I B U L A R R E H A B I L I T A T I O N

Rehabilitation with specialized exercises designed to reduce vertigo, gaze instability, and/or imbalance and falls. Most vestibular rehab exercises involve head movement to stimulate and retrain the vestibular system.

L O W V I S I O N R E H A B S U R G E R Y

An occupational therapist delivers vision teaching on work-ing with magnifiers and adaptive devices to aid in reading for those that may su�er from Macular Degeneration, Glau-coma and Diabetic Retinopathy.

Rehabilitation for patients recovering from angina, heart attacks, open heart surgery, Angioplasty, percutaneous cor-onary intervention (PCI) procedures and Ventricular assist device placement.

S P I N A L C A R E

Our back and neck care programs are custom-tailored, with special expertise in helping patients recover from spinal sur-gery.

T H E R A P E U T I C M O D A L I T I E S

Rehabilitation with the use of therapeutic modalities to help address pain, limitations in range, strength, and swelling. Patients are provided with treatment approaches to pro-mote optimum health and recovery.

E R G O N O M I C S

Patients with repetitive-use or stress injuries such as carpal tunnel syndrome, bursitis and tendonitis learn how to per-form daily work and life activities without aggravating their condition.

P O S T - O R T H O P E D I C

Rehabilitation for joint replacements, amputations, frac-tures, diabetic foot and wounds, osteoarthritis, orthopedic trauma, Rheumatoid arthritis, shoulder and back injuries.
