    ` Any qualified person as assessed and duly accredited

    by the bureau (DOLE-BWC) to practice and render occupational safety and health services and are tasked

    to implement occupational safety and health programs in

    the workplace in accordance with the provision of the

    Standard.

    ` Must have completed the 40-hour Basic Occupational Safety andHealth Training Course

    ` Must have at least 3 years of relevant experience in occupationalsafety and health

    ` Must possess the minimum qualifications stated under Rule 1964.01nos. 1 to 5, whichever may be applicable. (For first-aiders, nursesand physicians)

    ` Must be duly-licensed to practice his or her profession in thePhilippines

    ` If the applicant is a graduate of any 4 or 5-year college course

    without a license, he/she must have at least four years of relevantexperience in occupational safety and health.

    ` If the applicant is a college undergraduate, he/she must have atleast 10 years of relevant experience in occupational safety andhealth.

    ` To develop, implement and promote occupational safety and health

    programs in the work place

    ` To advise and assist the employer in complying with the provisions

    of the Standards, especially in the submission of reporting

    requirements

    ` To perform at least a quarterly appraisal/assessment of the

    implementation of the occupational safety and health programs in

    the workplace

    ` To prepare Annual Report of the safety and health programs in the

    workplace

    ` To be present during the scheduled safety inspections by the

    authorized government agents and as requested during regular

    health and safety meetings

    ` Part-time Safety Officer shall be required to perform the duties of aSafety Officer at least six hours per week.

    ` Hazardous Workplace refers to a workplace where operations orpractices performed by a worker exposes him/her to hazards likely tocause disabling injury, illness, death or physical or psychological harm.

    ` Highly Hazardous Workplace - a workplace where potential hazard withinthe company may affect the safety and/or health of workers not only withinbut also persons outside the premises of the workplace.

    ` Refers to any practitioner in occupational safety and health duly-accredited by the Bureau to practice, perform, and/or render consultative or advisory services on occupational safety and health.

    Requirements for Accreditation as Safety Consultant:

    1. Must be an accredited occupational safety practitioner for at least 5

    years with experience in at least 2 fields of specialization.2. Must have experience in policy formulation and development,

    monitoring and evaluation and implementation of occupational safetyand health management systems.

    3. Must have completed 80-hour advanced occupational safety andtraining course as prescribed by the Bureau; and

    4. Must have earned an additional 480 hours of training/seminar orrelated learning processes in various field of occupational safety andhealth from accredited organizations

    Is a planning and policy making group in allmatters pertaining to safety and health. In every

    place of employment, a health and safety

    committee shall be organized within one month

    from the date the business starts operating.

    1. Plans and develops accident prevention programs for theestablishment.

    2. Directs the accident prevention efforts of the establishment inaccordance with the safety programs and governmentregulations in order to prevent accidents from occurring in theworkplace.

    3. Conducts safety meeting at least once a month4. Review reports of inspection, accident investigation and

    implementation of program5. Submit reports to the manager on its meetings and activities.6. Provides necessary assistance to government inspecting

    authorities in the proper conduct of their activities such asenforcement of the provisions of the Standards.7. Initiates and supervises safety trainings for employees.8. Develops and maintains a disaster contingency plan and

    organizes such emergency service units as may be necessaryto handle disaster situations.

    Type A:- In every workplace having a total of over four hundred workers, the

    following shall compose the Health and Safety Committee:

    Chairman - The Manager or his authorized representative who

    must be a top operating official.

    Members Two Department Heads

    - Four workers (Union members, if organized)

    - The company Physician

    Secretary - The Safety Man

    Type B:- In every workplace having a total of over two hundred to four

    hundred workers, the following shall compose the Health and Safety

    Committee:

    Chairman - The Manager or his authorized representative who

    must be a top operating official.

    Members One Supervisor

    - Three workers (Union members, if organized)

    - The company Physician or the company nurse

    Secretary - The Safety Man

    Type C:- In every workplace with one hundred to 200 hundred workers, the

    following shall compose the Health and Safety Committee:

    Chairman - The Manager or his authorized representative.

    Type D:- In every workplace with less than one hundred workers, thefollowing shall compose the Health and Safety Committee:

    Chairman - The Manager.

    Members One Foreman

    - Three workers (Union members, if organized)

    - The company nurse/first-aider

    Secretary - The Safety Man

    Note: In this workplace, a line type safety organization may beorganized.

    Line type organization a form of organization where the generalmanager or head of the establishment directs the health and safetyprograms and assumes overall responsibility for the safety in theestablishment.

    Type E: Joint CommitteeWhen two or more establishments are housed under one building, the

    health and safety committee organized in each workplace shall formthemselves into a joint coordinating committee to plan andimplement programs and activities concerning all theestablishments.

    Chairman - The Chairman of the establishment committee.

    Members One supervisor from each establishments

    - One worker from each establishments (Union

    members, if organized)Secretary -Appointed by the Chairman (in high rise, the

    secretary shall be the building administrator)

    requirements and nothing shall prohibit increasesin the number of members as may be found

    necessary.

    ` Where workers are not organized, they shall be

    selected by a simple majority of votes of theworkers.
