SERVICE PROVIDER CRITERIA

The successful vendor will be responsible for participating in the Federal E-Rate program.

In addition, the Lennox School District, may use this proposal to issue purchase orders and pay for items on a purchase order that are not eligible for E-RATE discounts. The Lennox School District will pay the vendor using Lennox School District funds for these purchases.

Prices must be held firm for the duration of the E-Rate Year 19, fiscal year 2016/2017, ending September 30, 2019 or until all work associated with the project(s) are complete

These projects and services depend on partial funding from the E-rate program.

• Lennox School District expects each Service Provider to make themselves thoroughly familiar with any rules or regulations set forth by the E-rate program.

• All contracts entered into as a result of this RFP will be contingent upon the specific funding of the Funding Request Number (FRN) at the percentage rate submitted for.

• In accordance with FCC rules, bids for E-rate services must be the Lowest Corresponding Price. The maximum percentage the Lennox School District will be liable for is the pre-discount amount minus the funded amount as shown on the Form 471 Block 5. The Service Provider will be responsible for invoicing the Schools and Libraries Division for the funded amount.

• Services must be delivered prior to invoicing USAC. NO billing can take place before July 1, 2016 or the release of the Funding Commitment Decision Letter (FCDL) and the delivery of service, whichever comes first. In the event of questions during the E-Rate audit process, the successful vendor is expected to reply within 3 days to questions associated with their proposal.

• The contractor is responsible for providing a valid SPIN (Service Provider Identification Number) at the time the proposal is submitted.

• The contractor is responsible for providing an FCC Registration Number (FRN) and evidence of Green Light status with their proposal. Any potential vendor found to be in Red-Light Status will be disqualified from participation in the proposal process and will be considered non-responsive.

• All work is subject to the 100% approval of the project or purchase by the FCC under the E-rate discount program of the Telecommunications Act of 1996.

• No change orders will be allowed for the work resulting from this posting. • The Service Provider will be required to send copies of all forms and invoices submitted to

SLD prior to invoicing the SLD to Lennox School District for our records. • Within 5 days of award, Winning Vendor agrees to submit the item 21 template in USAC’s

official format to the district. • Vendor agrees to complete invoicing to USAC within 120 days of last date of service.

• The Service Provider acknowledges that no change in the products and/or services specified in this document will be allowed without prior written approval from the district and a USAC service substitution approval with the exception of a Global Service Substitutions.

• Lennox School District intends to use both E-rate and District funds for this bid submission. The exact quantities may be contingent on budget available from E-rate funding. Should the budget funding be insufficient for the needs of the district, the district may, at its option, purchase additional units from the same contract outside of E-rate.

• During the term of any Agreement resulting from this RFP, the District may elect to procure additional or like goods and/or services offered by the Respondent. Such services shall be negotiated and obtained via an official amendment to this Agreement and approval by the District’s Governing Board. All terms, conditions, warranties, obligations, maintenance and support of said goods or services shall have a coterminous expiration date with the original date of this Agreement. The District shall not enter into a separate Agreement for said goods or services. Respondents must state in their proposal that they acknowledge, accept and are in agreement with coterminous expiration conditions.

• Vendors may bid on any or all sections of the RFP. In your submission, please clearly state the section(s) that you are bidding on.

• Lennox School District prefers that the equipment requested be purchased off of a state master contract or award schedule (e.g. CMAS) or other similar award vehicle. The cabling component of the RFP will be governed by Public Works contract rules. Effective March 1, 2015 all bidders who submit bids under public works contracts must be registered with the DIR, or are automatically disqualified. If bidding on the Cabling section of the RFP (Section 3), Vendors must supply DIR status with their submission. All Service Providers will be responsible for procuring the discounted amount from the SLD through the Service Provider Invoice (SPI) form 474. Lennox School District will not provide the Form 472 (BEAR form reimbursement process).Lennox School District reserves the right to deny any or all proposals associated with this RFP, even with SLD funding approval. The Lennox School District reserves the right to accept the pricing proposal solely dependent upon SLD approval.

Prospective Service Providers will be required to submit three references, with the proposal describing experience with comparable projects in the K-12 customer market, their SPIN

number and their FCC Registration Number (FRN). Failure to supply these documents may be considered grounds for disqualification. In addition, Lennox School District reserves the right to fund, (proceed with project or purchase) or not to fund regardless of E-rate approval.

The Lennox School District requires certification by the awarded contractor, prior to project commencement, concerning criminal records check in accordance with California law. Participating contractors must have an active Service Provider Identification Number (SPIN) with the SLD.

The RFP will be posted on the district’s website at: http://www.lennox.k12.ca.us/District/Department/1-Technology-Department on Wednesday January 13, 2016 and advertised in the Daily Breeze.

All proposals are to contain costs for taxes and shipping as separate items. All proposals will clearly separate the cost for goods and services not eligible for E-Rate discount.

Venders are expressly notified to not contact any other Lennox School District personnel for any reason

prior to award. The only exception to this is in the case of ongoing operations with a current provider of

services to Lennox School District. Failure to comply will result in rejection of the proposal.

The contractor who is awarded the contract will be expected to deliver a fully functioning solution for all

systems, i.e. a turnkey solution. Each of the technologies is addressed in the following sections.

Lennox School District is required to publically post the RFP and all related documents both on the

District’s website http://www.lennox.k12.ca.us/District/Department/1-Technology-Department, and the E-rate Program administrator USAC http://www.usac.org/sl/tools/e-rate-

productivity-center/default.aspx. In the event of a discrepancy between the two documents, the

version on the District website is the primary document and all others versions are copies.

EVALUATION CRITERIA

Proposal Evaluation Criteria include but are not limited to:

• 30% Price is the most heavily weighed factor in selecting the awarded service provider • 20% Ability to deliver services within desired timeframe • 20% Cost of ineligible components • 10% Past successful E-Rate experience • 5% Completeness of response • 15% Compatibility with existing network

PROPRIETARY DOCUMENTS

The information contained in this RFP and the RFP itself are proprietary in nature and shall not under any circumstances be released, in part or in whole, to third parties without the prior written approval of Lennox School District.

All proposals received by Lennox School District will be considered a Public Record as defined in section 6252 of the California Government Code and shall be open to public inspection, except to the extent the Proposer designates trade secrets or other proprietary material to be confidential. Any documentation, which the Proposer believes to be a trade secret, must be provided to the Lennox School District in a separate envelope or binder and must be clearly marked as a trade secret. Prices and terms of payment shall be publicly available regardless of any designation to the contrary

SUBMITTALS

Venders are required to submit the following with their proposal in electronic format on either a thumb drive or via email, in addition to any bid delivered to the district in a hard copy.

1. Brochures, technical manuals, installation guides and user's guides giving technical and operational information about the proposal system(s) and all instruments

2. A copy of the Vender’s purchase contract.

3. A copy of the Vender’s standard maintenance contract, in addition to any maintenance contract options, including but not limited to, response time options, prepayment maintenance plans, and various length of term (i.e. 2nd year, 3rd through 5th year, etc.) options.

4. A copy of all warranties related to any equipment or services provided 5. A parts list of all components 6. Any Software License agreements

Bid must break down costs by E-Rate eligible and E-Rate ineligible cost accounts

Current Infrastructure (networking components)

Lennox School District network consists of Cisco switches already installed. Proposals should contain Cisco 3650 switches that are 10GBASE-T capable. While the district will entertain bids from vendors using other solutions, the solution must be compatible with the existing Cisco Network, and it is incumbent on the Vendor to demonstrate compatibility.

SCOPE OF WORK Section 1: Equipment – Switch Installation All switch proposals must include the following- (This is new equipment only)

1. 125 - Cisco 3650 Switch 2. 1 - 10 GB GBIC including SC to LC fiber patch cord per switch 3. 2 – 10 GB GBIC including LC to LC Fiber Patch cord per IDF requiring more than 1 switch (53

instances) 4. 1 Cat 6 patch cable for each available port 6’ Length 5. Any Modules needed for Catalyst 4510s at each site MDF to be 10GBASE-T Capable (Determined

at walk through)

Installation, configuration, and documentation of each switch is required at all sites.

See attachment A for site breakdown and numbers

Section 2: Wireless Network Electronics

Lennox School District is expanding the current Cisco Wireless

Network. Cisco products will be used as examples of wireless devices

type and capabilities.

Requirement: Product should integrate into the existing Lennox School District Cisco wireless network

Section 3: Building Wiring (Fiber and Ethernet)

! Ethernet School Sites (All) 12 data drops per classroom, 4 drops are to be ceiling mounted and the other 4 pairs are to be distributed one pair per wall. Existing drop locations can be used but new CAT6a cables must be used along with new RJ45 Female connectors and wall plates.

Distance of cabling is approximately 1000 feet per drop, for 12 drops per classroom. Please use the maps enclosed with the RFP to identify the number of classrooms per school to build your estimates.

• Install 2 new CAT6a cables terminated with an RJ45 female wall mount plate in the center of the ceiling, in a ceiling tile. This will include a red CAT6A RJ45 modular jack on both the wall mount plate and patch panel side.

• Label Drops/Patch panel with Room number, Drop number, IDF Name and patch panel number. o Example – Room 34 Drop 4 – IDF 5 Patch 32

• All wall drops will be blue RJ45 Female connectors • Provide 2 red CAT6a 3’ Cables per room • Provide labor to uninstall/pull all existing copper cabling (cat5 and cat5e) and dispose of it.

District Office (Not E-rate Eligible) Existing drop locations can be used but new CAT6a cables must be used along with new RJ45 Female connectors and wall plates.

• 4 Ethernet female RJ45 drops per conference room split into pairs in opposing walls • 4 Ethernet female RJ45 drops per office split into pairs in separate walls • 2 Ethernet female RJ45 drops per cubicle, underneath the desk area • 4 Ethernet female RJ45 drops installed in the Warehouse • 2 Ethernet female RJ45 drops installed in tech storage room in Maintenance building • 8 Ethernet female RJ 45 drops installed in the Board room, split into pairs one on each wall. • 2 drops are to be ceiling mounted in the Board room • Label Drops/Patch panel with Room number, Drop number, IDF Name and patch panel number.

o Example – Conf Rm A Drop 4 – IDF 5 Patch 32 • All wall drops will be blue RJ45 Female connectors • Provide labor to uninstall/pull all existing copper cabling (cat5 and cat5e) and dispose of it.

Computer Labs

• Install 4 new CAT6a cables terminated with 2 RJ45 female wall mount plate in the ceiling (each plate as per indicated in the maps), in a ceiling tile. This will include all red CAT6A RJ45 modular jacks on both the wall mount plate and patch panel side

! Fiber Additional strands of fiber to be ran between all MDFs to existing IDFs. The Fiber must be 10GBASE-T

capable, Single Mode and will be installed as shown in the maps.

• Install 1 new strands of 10GBASE-T Capable Single Mode Fiber to each IDF as per shown in the maps with a Termination added onto the existing Fiber Shelf.

• Provide labor to uninstall/pull all existing Fiber cabling that does not meet the new criteria of providing 10GBASE-T speeds via Single Mode.

Overall Cabling requirements

• New cabling will be installed according to manufacturer specifications and TIA/EIA Standards. It will be properly routed, supported and protected.

• Label and test the new cable and provide the testing report in PDF format • Provide BLUE CAT6a UTP Plenum Rated Cable • Provide 10’ slack in each cable run • Where required WHITE Panduit surface mount raceway will be used • All cabling in each IDF location and MDF will be “dressed” and will meet Lennox School District

expectations.

Existing conduit (Internal and External) and wall drops exist and may be used but may be added to as needed for the required amount of drops needed. The preference for wall drops is in wall and in ceiling cables instead of raceways or surface mounts. The usage of surface mounted conduit should be minimized where possible.

Network Cabinet Replacement

• Replace the IDF Cabinets in the Lennox School District that are of beige color as indicated by the mandatory walk through.
