Take Care New York 2024 Community Engagement – Broad Outreach Partners

Overview This document describes the requirements and application process for organizations interested in becoming Take Care New York (TCNY) Outreach Partners to implement the What Creates Health Toolkit in community events. Each Outreach Partner can receive $2,000 per outreach event for a maximum of $8,000 for four events from the Fund for Public Health in New York (FPHNY) to lead community outreach for TCNY at community events taking place from October 2019 to December 2019. Ten to twenty organizations will be selected as Outreach Partners dependent on the total funding amounting to $99,000 or less. Organizations serving different geographic areas within NYC will be selected as TCNY Outreach Partners. During the 4-month grant period, each awardee will engage community members through pre-designed activities to spark conversations on “What Creates Health?” in their neighborhoods to inform and create momentum for Take Care New York 2024. All awardees will have an opportunity to give feedback on the activities to ensure that they are accessible, user-friendly, and effective. Once the activities are finalized, awardees will be trained and receive materials to support facilitation. Applicants must specify the NYC Community Districts and neighborhoods that they propose to serve as a TCNY Outreach Partner. FPHNY and its partner, the New York City Department of Health and Mental Hygiene (DOHMH), intends to fund this work in as many NYC communities as possible to facilitate health conversations that can inform the planning and development of TCNY 2024. Preference will be given to organizations that can reach multiple NYC Community Districts and neighborhoods.

Background and Objectives In December 2014, the New York State Department of Health selected FPHNY to lead the New York City Population Health Improvement Program (PHIP) in partnership with DOHMH, the United Hospital Fund, and the New York Academy of Medicine. The PHIP will promote health equity as well as the “Triple Aim” of better care, lower health care costs, and better health outcomes for New Yorkers. One of the goals of the New York City PHIP is to generate broad community and stakeholder participation in advancing TCNY. TCNY is the City’s blueprint for advancing health equity and giving everyone the chance to lead a healthier life. TCNY was first launched in 2004 and since then has been updated periodically with new goals that promote the health of all New Yorkers. The last iteration, TCNY 2020, was released in Fall 2015, outlining objectives for the City to achieve by the year 2020. Unlike previous iterations of Take Care New York, TCNY 2020 focused on achieving health equity and creating healthy communities by using a racial equity and a social justice lens, as well as focusing on upstream factors and the social determinants of health. In early 2020, DOHMH will launch TCNY 2024. Building on the success of TCNY 2020, TCNY 2024 will have three core functions:

1. Instigate discussions among cross-sector stakeholders about what creates health and amplify the urgency of addressing health inequities; 2. Build new and strengthen existing networks that can support the development of multi-sector collaborations with shared priorities around eliminating

health inequities; and 3. Catalyze action and ensure that stakeholders can effectively lead projects that will drive change in health inequities.

DOHMH aims to generate momentum and interest in TCNY 2024 through broad outreach in neighborhoods across New York City. DOHMH seeks to partner with organizations that have a physical presence in and/or demonstrated ties to the community members in any of the neighborhoods listed in Appendix B.

Scope of Services Awardees will be funded to support the activities outlined below in collaboration with FPHNY and DOHMH. DOHMH reserves the right to award funding based on the expected attendance/coverage of events and proposed neighborhoods. DOHMH will determine which events will be selected per application, awardees may be funded to do fewer than the total number of proposed events based on neighborhood selection, expected attendance, and event type.

1. Outreach Activities: Awardees will be funded to engage community members at events through the TCNY 2024 broad outreach toolkit activities. The TCNY 2024 broad outreach toolkit (still in development) will consist of 3-4 engaging activities that selected awardees can use to engage community members in conversations about health in their neighborhoods (examples of the types of activities can be found in Appendix A). All awardees will participate in developing the toolkit by giving feedback on the activities to be included in order to ensure that they are accessible, user-friendly, and effective. Once the activities are finalized, awardees will be trained and receive materials to support facilitation.

Applicants that are planning to lead or participate in upcoming summer/fall/winter community events are eligible to apply for $2,000 per event in order to engage community members through the TCNY 2024 broad outreach toolkit activities.

Any applicant can apply for a maximum of four events for a total of $8,000. Each event must take place in a separate neighborhood.

Events can include, but are not limited to: Outdoor concerts, plays, street fairs, block parties, Weekend Walks, NYCHA Family Days, fundraisers, holiday parties, drives, townhall meetings, faith-based meetings, etc.

Events must have expected attendance of at least 100 community members.

TCNY 2024 broad outreach activities can be facilitated concurrently with other planned activities of the event.

Applicants can apply to participate in events contingent on being selected as an Outreach Partner to engage community members through the TCNY 2024 broad outreach toolkit activities.

Examples of events: Organization ABC annually tables in the summer community fair in Coney Island to talk about their services and upcoming programming with community residents.

This year with the funding from TCNY 2024, they will also engage community members in the activities from the TCNY 2024 Broad Outreach Toolkit.

Organization R2D2 wants to strengthen their relationship with the community in neighborhoods where they are currently building their presence. They are aware of an upcoming fall festival that will be taking place sometime in September in the Mott Haven, Bronx that is expected to have large numbers of attendees. They will apply to participate in the fall festival if selected as a TCNY Outreach Partner to engage participants in activities from the TCNY Broad Outreach toolkit as icebreakers to other conversations.

A faith-based institution in Queens Village has been working very hard to raise awareness about healthy eating to combat advanced diabetes in their congregation. If selected as a TCNY Outreach Partner, they will feature the TCNY Outreach Toolkit activities alongside other planned activities at their annual walk-a-thon.

A graffiti artist from Tottenville, SI will be conceptualizing a mural with the community and other stakeholders at a kick-off meeting in October. They decide to use the activities from TCNY 2024 Broad Outreach Toolkit as icebreakers to get people thinking about what creates health in their neighborhood.

2. Ongoing: Reporting/Accountability:

Participate in mandatory meetings, including biweekly calls with DOHMH and/or the TA provider

Use templates provided by DOHMH to submit: i. Brief summary report on lessons learned for each event and a compilation of information collected (participant email sign up lists, photos, etc) during

the events within timeframe specified by DOHMH ii. Documentation showing completion of the activities outlined above, including (but not limited to) email sign up lists, and photos of the Broad

Outreach Toolkit being used iii. Invoices and necessary documentation for contract payment

Eligibility & Selection Criteria

Applicants must be in good standing with the federal government, the State of New York, and the City of New York. Applicants must demonstrate a “developed base” (defined by a physical or programmatic presence and strong network of support) in the target neighborhood(s) and among the target population(s) in which they propose to conduct outreach.

Applicants must fall into one of the below categories: - Health or human services organization - Faith-based organization - Neighborhood association or other non-governmental neighborhood-based organization (e.g., tenant association, PTA, etc) - Educational organization - Arts and cultural organization - Local business – M/WBE preferred - Community groups with fiscal sponsors - Individuals with a Social Security number (e.g., artists, community organizers, etc) - Coalitions are welcome to apply, as long as a lead organization is identified. The lead organization will be the official awardee of grant funds from

FPHNY. It will be responsible for ensuring complete execution of the workplan and fulfillment of reporting requirements.

Contract Development Awardees will be expected to finalize their contract workplans (clearly describing their planned activities) in partnership with DOHMH and FPHNY. The finalized workplan will become part of the contract. The awardees will be expected to submit documentation to DOHMH to demonstrate completion of contract deliverables. Contract continuance throughout the specified contract period will be based upon performance and progress in meeting contract goals. The awardee will be subject to all specified state reporting requirements.

Contract Term and Schedule of Activities The expected contract term will be from October 1, 2019 to December 31, 2019. The 3-month workplan will be finalized in consultation with DOHMH and FPHNY as part of contract execution. Use of Funds Funds may be used for, but are not limited to, the following program-related purposes as deemed necessary by the contractor:

Hiring/assigning staff, including a project coordinator/community organizer to assist with organizing community member and stakeholder convenings, logistics, and documenting all efforts

Event expenses such as room rental fees, food, interpretation services, or rental equipment

Outreach materials such as fliers or paid social media posts
