• Humans acquire the disease usually through the biteof infected fleasof infected fleas– when their saliva or faeces is rubbed in or throughaerosols of dried faecesaerosols of dried faeces.

Clinical presentation• Endemic typhus resembles many other illnesses and very few

patients are provisionally diagnosed correctly.

• Headache and fever (in 12% of cases)• Rash develops in 54% of patients.Rash develops in 54% of patients.• Nausea, vomiting, diarrhoea and abdominal pain suggest

gastrointestinal diseases while cough and abnormal chestgastrointestinal diseases while cough and abnormal chestradiograph suggests pneumonia or bronchitis.

• The rickettsiae are transmitted transovarially in ticks,which therefore act as both vectors and reservoirswhich therefore act as both vectors and reservoirs.

• The infection may be transmitted to vertebrate hostsby any of the larval stages or by adult ticksby any of the larval stages or by adult ticks.

• Tick typhus was first observed in India in the foothillsof the Himalayas.y

• Subsequently, the disease was reported from manyparts of the countryparts of the country– Allahabad, Narsapatnam, Ratlam, Secunderabad,Trichinapally, Bangalore, Jhansi, Darjeeling, Pune andLucknow.

• The tick sp. Rhipicephalus sanguineus is the mostimportant vector and is generally found infestingp g y gdogs all over.
