Responsibilities
Do what you can to look after your own health – good diet, regular exercise, don’t smoke or abuse substances (drugs and/or alcohol).
Care for yourself when appropriate, e.g. treat yourself at home for common ailments such as coughs, colds and sore throats. Get advice from a pharmacist.
Give blood if you are able and carry an organ donor card or special needs card or bracelet.
Listen carefully to advice on your treatment and medication. Tell the doctor about any other treatments you are taking.
Treat NHS staff, fellow patients, carers and visitors with respect.
Keep your appointment of let the GP, dentist, clinic or hospital know if you cannot make it. Book routine appointments in advance.
Return any equipment that is no longer required, e.g. crutches.
Pay prescription charges and any other charges promptly when they are due.
Rights
To a service based on clinical need not the ability to pay.
To have information kept confidential.
Right to have access to information about services, options and treatment -including risks associated with treatment.
