BARRIERS - SIZE

• Transportation not readily available to get the services they have been referred to or for which they are trying to receive referrals.

• Resources for housing and supportive services are fewer in the rural areas.

BARRIERS - INVOLVEMENT

• Number of service providers do not participate in their Continuum

• Everyone has their own intake forms• Involvement also depends on how

accurate the information is that service providers submit to the 2-1-1 centers at any given time

SOLUTIONS - INVOLVEMENT

• The State is requiring involvement in the Continuum of Care Centralized Intake for all subrecipients of the Emergency Solution Grant funds.

BARRIERS – MULTIPLE HMIS• Each Continuum manages their own HMIS• Tulsa County, North Central, Northeast and

Southeast Continua combined their systems into one HMIS system that now covers 41 out of the 77 counties in Oklahoma

• 20 counties (3 CoC) work on one Software package, but 3 separate systems

• 16 counties (1 CoC) use the same system as the Continua that combined; but kept their system separate from the merge.

Cleveland County CoC:

Cleveland’s Central Intake will be managed by one organization and data will be entered directly into their HMIS.

Solutions – State 2-1-1 System

• Only Statewide System• The Continua can contract with 2-

1-1 systems to amend questions asked at intake

• Both 2-1-1 main centers already have experience with screening from their involvement with HPRP funds

• System is available 24/7
