Notification and Reporting Criteria based on:
• List of Dangerous Substances in WSH(MHI) Regulations
• American Petroleum Institute’s Recommended Practice
754 Process Safety Performance Indicators for the Refining
and Petrochemical Industries
Notify MOM as soon as reasonably practicable
Process-related incident reporting
Submit report on WSH Incident
Reporting eServices on MOM’s
website within 10 calendar days
