Rehab and Hospital to Home PackagesA clinical and thoughtful approach to homecoming,
keeping you safe and avoiding readmission.
• A nurse will visit a potential client in the hospital or rehab for a FREE assessment.
Scheduling assessments before discharge is key, allowing our nurses to obtain the full spectrum of the client’s status, helping to facilitate post-discharge follow-up, and advocating for the client’s needs.
How do the programs work?• Home Helpers will transport the client home, pick-up
medications, restock groceries, run errands, provide pet care, assist with laundry and light housekeeping.
• Our nurses will develop the care plan, select and train the caregiver, check in on the client and are available 24/7 for emergencies.
• In most cases care can begin within 24 hours.
24 Hour Plan - 6 total visits-3 visits/week with a caregiver-each visit is 4 hours
The Discharge Nurse Plan - One visit for 4 hours with a nurse that includes the same features as the Discharge Caregiver Plan, plus a home safety assessment, a nursing assessment, medication set-up, and any needed one-time skilled nursing services.
The Discharge Caregiver Plan - One visit for 4 hours with a caregiver to help you return home. Restock groceries, pick-up medications, run errands, clean out the refrigerator, get organized, etc.
40 Hour Plan - 10 total visits-5 visits/week with a caregiver-each visit is 4 hours
56 Hour Plan - 14 total visits-7 visits/week with a caregiver-each visit is 4 hours
80 Hour Plan - 20 total visits-5 visits/week with a caregiver-each visit is 4 hours

Our hourly care experience allows our clients to "age in place" at home offering 1 on 1 care. We can start at our minimum 4 hours/shift and increase the schedule as the client’s needs change. All care is under the direction of our nursing team.
