1. Gas Cylinder Safety
Be certain cylinders are properly
colored & labeled
Strap cylinders to
cart before
transporting is
attempted
Do not roll or drop cylinders
Keep cylinders
vertical
Cylinders to be secured
through proper fencing &
chaining
Store flammable
gases apart from
oxygen
The storage room shall
be of fire resistant
construction
Open flames, lights, mobile phones, lighting
of fires or any hot work shall be prohibited
near cylinders
Cylinders shall be stored
in cool, dry & ventilated
area and away from
potential sources of heat
Flashback
arrestors should
be used on both
sides i.e., from
the gauge as well

Regularly check hoses and

2. Road Safety Theme – Jan’15
Road safety theme was observed across divisions in Tata Power. Below are some of
the glances of the same.

Defensive Driving awareness Program was organised for drivers, employees and

Safety skit on “Sadak Suraksha, Jeevan Raksha” was played by workers about
importance of wearing Helmet. Prizes were distributed to winners of the Safety Quiz
competition and Winners of Safety Poster of school children.

3. Lessons Learned
3.1 Fall from height while chimney painting
Division : Trombay
Date of Incident : 14th January 2015
Type of incident : Fatal
Incident Time :14:10 Hours
Description of Incident:
Unit#6 chimney painting was in progress
between 2nd and 3rd platform. Painter was (Mr.
Shaktipada Jana) doing painting, while sitting
inside an electrically driven specialized cage.
At around 14.15 hrs, the cage started falling
down rapidly and landed on 2nd platform,
leading to injuries. He was immediately taken
to Trombay Occupational Health Centre (OHC).
At this time he was conscious, oriented and
following the command. He was given First Aid
treatment at OHC -Trombay and immediately
referred to Joy Hospital for further investigation
& treatment. He was succumbed to his injuries
at 17.15 hrs.

Recommendations:
1. Identify department wise all high risk activities in division and maintain a register for the
same.
2. Detailed SOP / Method Statement/ Lifting Plan including HIRA / JSA for each High risk
activity has to be developed by team of experts (division) jointly with working party and
third party consultants,(if required)
3. Purchase request should include the complete approved SOP, Method Statement and
Lifting Plan for all high risk jobs.
4. For all high risk activities, subsequent documents in the chain shall be flagged as “high
risk”.
5. If any high risk SOP / Method statement / Lifting Plan needs to be revised by the working
party, the same shall be vetted by TPCL experts / third party prior to the execution.
6. Religious implementation of CSM (6 step) process shall be ensured for all high risk work
orders and it should be flagged as High risk job.
7. Training on rigging practices and Lift Plan must be arranged for Tata Power Engineers /
Supervisors / Riggers
8. No Modification shall be permitted on the rigging setup or lifting tools & tackles without
approval of the TPCL expert or Job executing Engineer in charge. All modification to be
approved in writing.
9. Technical competency assessment for safety critical jobs must be carried out jointly by
job executing engineer, contractor Engineer and relevant competent person.
10. Only approved competent people can be deputed for the job.
11. The rigging setup shall be inspected / tested by in-house executing engineer or
inspection department or third party consultant to confirm mechanical integrity prior to
commencement of work. The rigging setup shall also be inspected at regular frequency
(as specified in SOP) to confirm consistency / quality.
Equipment design / engineering shall be such as to ensure adequate redundancy (at
least two parallel systems) of critical components to avoid catastrophic consequences in
case of single point failure.
12. All high risk activities must be monitored & adequate competent supervision must be
ensured
3.2 Derrick contact with live HT busbar of feeding transformer
Division : Transmission
Date of Incident : 11th December 2014
Type of incident : Occurrence
Incident Time :14:09 Hours
Description of Incident:
Derrick erection work for replacing CVT for 110 kV Parel – Carnac line was in progress.
While lifting the derrick, the tile on concrete floor on which it was placed broke and the
derrick tilted and came on HT bus bar of transformer group # 4 and resulted in the
tripping of transformer group # 4, HT breaker feeding transformer # 7. No shut down to
consumers, as transformer 6 & 7 were parallel.

1.3 Violation by supervisor: Inadequate supervision and ineffective coordination at site during
the job
Physical Factors
2.3 Improperly prepared equipment: Derrick was placed on tile and strength of tile was
inadequate to sustain derrick load
4.5 Inadequate work place layout: Derrick was not anchored from all directions to restrict its
unwanted movement during lifting.
SYSTEM FACTORS
4.4 Inadequate identification of worksite / job hazards:
Outage of equipment in affected area not taken
Improper selection of ground
1.2 inadequate vertical communication between supervisor and person: No evidence of
toolbox talk
2.2 Inadequate contractor pre-qualifications:
Workers engaged in work were not skilled and trained for the work
Training records for technical / safety not available for workers engaged in the job
Recommendations
JSA to be signed and properly implemented during work execution.
Contractor’s workers shall undergo formal safety training before engaging them in the
work.
Adequate manpower to be made available for the job as planned.
Record of tool box talk to be maintained and steps of JSA to be explained in toolbox talk.
Competent supervision to remain present at site during work. Supervisor involved for
derrick erection shall have rigging knowledge prior to undertake the job.
Stability of floor/ground to be ensured during beginning the job as well as during the
work
Derrick shall be secured from all four direction to avoid unwanted movements.
Outage for affected area to be taken as mentioned in JSA. In such cases where outage
is not possible for affected area, other best practices like use of mechanized Jig etc. to
manage the hazards related to live system to be followed.
Fixed type of earthing arrangement with nut bolt to be made to avoid possibility of
detachment due to movement of Derrick.

Training on LOTO, Chlorination awareness, Working at height, PTW, Mobile Crane Safety
trainings were conducted for own employees. Apart from Safety Induction (picture), first aid,
electrical awareness, Safety training & IMS awareness training, Working At Height trainings
conducted during the month for Contractor employees.

Conducted Office Safety Inspection QAIT department-Trombay Station ‘A’, AOC job – Vashi Colony renovation, Electra, Hardik Heritage, Ramtekdi, Tata Colony Hostel Blds, Vashi colony & Guest Houses.
Conducted Rescue training to Trombay fire staff. Corporate OHS members participated in Safety Bipartite Committee Meeting of Maithon,
Transmission, DCS, and Wind operations.
7. High Visibility Safety Tour
High Visibility Safety tours are conducted by cross Divisional heads with an objective to:
i. To demonstrate leadership and commitment towards Occupational Health and Safety
ii. To encourage informal discussion between Divisional management, employees and contractors
iii. To improve the safety culture and behavior of employees iv. To identify / sharing best practices & also identify improvement areas for

10. Security Highlights
I. Trombay Celebration of 26th Jan 2015 at Trombay Stn B. National Flag unfurled by Op Manager.
Road Safety week was conducted from 11 Jan to 17 Jan in Trombay.
BCP Audit of Tata Power conducted as on 23 Jan 15 at Trombay by the Audit team of BSI
to certify as ISO 22301: 2012 company successfully.
Mock Drill under Eastern Region Police QRT conducted at Trombay on 15 Jan 15.

REPUBLIC DAY CELEBRATION Unfurling of national flag by the CEO-Mr Chandrashekhsr.
SCRAP RECOVERED BY SECURITY

III. Jojobera
Republic Day celebrated with enthusiasm & vigour at Jojobera
Republic Day at neighbouring village of Jojobera attended by the Head Security.
IV. CGPL Mundra:
Celebration of Republic Day on 26th Jan 15 at CGPL including family & Children.
VISIT OF CISF COMMANDANT on 15 Jan 15. PRIOR TO CISF DEPLOYMENT AT CGPL.
Visit of AIO –CID, in context of declaration of CGPL plant as prohibited Area.
Road Safety Day observed in CGPL from 11-17 Jan 15.
