Hygiene Today

Today, hospital disinfection is a topic of serious concern. A great amount of knowledge and effective disinfection methods are available. However, there is no reason for an all-clear message.

According to official statistics, more than 500.000 patients yearly are infected with bacteria, fungi or viruses during a hospital stay, alone in Germany. The ever increasing number of antibiotic resistant microorganisms and the rising average age of patients, as well as their susceptibility for infectious diseases, are two reasons for the rise in nosocomial infections (infectious disease picked up during treatment in a hospital). Nosocomial infections cannot be completely eliminated. European studies show figures between 3.5 and 9.0 per 100 in-patients. Even so, a study by the German Ministry of Health comes to the conclusion that 25% of all nosocomial infections could be avoided. (The study records the decrease of the infection risk when active quality management as well as a hygiene taskforce are introduced in a hospital or clinic. The results of the study: a proud 26% decrease).

Economic Aspects of well-directed Hygiene Measures

Nosocomial infections are not just unnecessary for the patient. Nosocomial infections also generate unnecessary costs for health insurance companies, hospitals and clinics. A high infection risk will lead to higher staff absences and can ruin the reputation of an institution.

The costs incurred by well-directed hygiene measures can easily be compensated by the benefits resulting from fewer infections. A lower infection risk for the hospital staff will result in lower sick leave figures. Additionally, a lower infection risk can have a positive effect on the image of the institution. Private patients are very much interested in the reputation of a hospital or clinic.

The right Disinfectant: a difficult Choice

Most chemical disinfectants used today contain one or more of the following basic ingredients: Aldehydes, Alcohols, Phenolics, Halogens and Quats. The following points should be considered at the time of making a choice:

Spectrum of Activity: Many disinfectants on the market will kill a small range of disease-causing organisms. This selectivity may lead the user into a dangerous false sense of security as far as the reliability of the disinfectant is concerned.

Application: Products that are negatively affected by temperature level, sur-face structure and/or inactivation by the presence of proteins are not stable in their reliability. Lightly flammable products additionally involve a fire/explosion hazard.

Toxicity: Some of the above mentioned disinfectant components may have negative side effects. Skin irritations may arise as a consequence of the degreasing and irritating properties of disinfectants. Disinfectant vapours and aerosol may irritate the respiratory tract. Some of the disinfectants being used today are even suspected of having carcinogenic properties.

Corrosion: Some of the above mentioned disinfectant components have a corrosive effect on metals and/or synthetic materials.

Ecology: The chemical composition (and consequently the biological degradability) of most disinfectant ingredients on the market is such that they have to be considered as harmful for the environment.

Important Requirements to be fulfilled by a Disinfectant

Requirement: Reliable Biocide Effectiveness

1. The disinfectant should kill the pathogen organisms present in a fast and reliable manner.

2. The product should have no gaps in the spectrum of activity.

3. The disinfectant should prevent a renewed contamination.

4. There should be no build up of resistance against the disinfectant by the microorganisms.

5. The disinfectant should be listed by the German DGHM/VAH.

1. The disinfectant should have a good cost/return ratio.2. The products should have a long shelf life with practically no loss of effectiveness during storage.3. A wide range of appropriate application equipment should be available.4. Service in form of detailed know-how in particular applications as well as a long experience in the field

of disinfection should be readily available.5. Manufacturing of the disinfectants must comply with ISO standards and should guarantee the continu-

ous highquality of the products.

A closer Look at Sanosil Disinfectants

Ingredients

The main active substance used is hydrogen peroxide, an environment friendly substance. In a complex manufacturing procedure, the active substance is stabilized and boosted in its effectiveness against microorganisms by adding silver, thus achieving a better antimicrobial effect compared to hydrogen peroxide alone. The minimal traces of silver remaining on the surfaces treated are invisible, non toxic and they effectively inhibit a renewed contamination. The elemental oxygen (O2) 1. that separates from hydrogen peroxide oxidises the cell membranes of microorganisms thereby denaturing and destroying them. This effect is reinforced by the silver ions 2. binding themselves to the disulfide bond of particular proteins which causes the proteins to be inactivated or precipitated.

Convincing Effectiveness The convincing biocide effectiveness covers all types of microorganisms. Sanosil Disinfectants are effective against gram positive and gram negative bacteria (incl. spore forming organisms), viruses, yeast, fungi and protozoa. The effectiveness of Sanosil Disinfectants has been tested and confirmed in more than 270 assays (carried out by international and well-known institutions, laboratories and universities). The product Sanosil S003 is listed by the VAH (German Association for Applied Hygiene). A few examples of successfully tested germs: Bacteria: Bacillus anthracis, Clostridium perfringens, Eschericia Coli, Legionella pneumophilia, Mycobacterium tuberculosis, Pseudomonas aeruginosa, Staphylococcus aureus/MRSA, Streptococcus faecalis, Salmonella typhimurium, Vibrio cholerae, etc. Viruses: Hepatitis B, Herpes simplex, HIV-1, Influenza A (incl. H5N1), Orthopoxvirus, Norovirus, Papovavirus, etc. Yeast and Fungi: Aspergillus niger, Candida albicans, Trichophyton mentagrophytes, Penicillium digitatum, etc. Protozoa: examples are Amoebae, Cilliates, Cryptosporidiae, etc.

Sanosil S003 is tested according the requirements of the German DGHM and listed by the VAH (German Association for Applied Hygiene). Surface disinfection for prophylaxis in hospital and primary healthcare (bactericidal and yeasticidal). With mechanical action / dirty conditions, high interfering substances / contact time: 15 Min. / undiluted (100%)

Effective against Biofilm

Contrary to most other disinfectants on the market, Sanosil Disinfectants are very effective against biofilm. Biofilms are a real challenge for disinfectants. They form at the borders of aqueous systems (where dry and wet areas meet - example: drain in washbasin) but also within aqueous systems. Biofilms consist of a thin slimy layer of excreted extra- cellular polymeric compounds (EPS), forming a sort of hydrogel or matrix. This matrix protects the microorganisms from external influences and from disinfection measures. Thanks to the oxidising properties of Sanosil Disinfectants, the protective biopolymeric matrix can be penetrated, exposing the microorganisms within which, under normal circumstances, would resist any attack by chemical biocides.

Inhibition of a renewed Contamination

The effect of silver makes Sanosil Disinfectants particularly long-lasting in their effectiveness. A renewed contamination of a treated surface is effectively inhibited (bacteriostatic effect). An assay in collaboration with a global pharmaceutical company confirmed that 12 hours after applying a 3% Sanosil solution a microbiocide effect (measurable germ reduction) was still detectable. After this period of time the effect changed from bactericide to bacteriostatic.
