Eligibility Requirements

• MUST:• Be 62 years of age or older• Live independently, not in need of immediate care• Complete application process, which includes medical and

financial eligibility• Live in Lee or Collier County• Maintain health insurance coverage (Medicare and

Supplemental coverage)*A diagnosis of an existing degenerative neurological disease may limit plan options
