Student: JohnnyGrade Level: 5Problem: Johnny can complete numerical
calculations and he has the ability to read adequately HOWEVER he cannot apply operations to solve mathematical story problems.

Goal of Intervention
Complete math word problems with 90% accuracy after 10 weeks of intervention
