reflects, anticipates and innovates to fulfil the future service needs of children, families and schools and delivers on health priorities.

Children and young people in the three boroughs
The child population in the Tri-borough area is smaller than London and England, particularly in Westminster and K&C.
People are more likely to move out of the more affluent areas, leaving a larger proportion of families in areas of social housing and deprivation

• The official definition of poverty is having a household income less than 60% of the national median income that year. (£12,600 in 2012/13)
• More children (<20) are in poverty living in the deprived areas, particularly Queens Park, White City and North Kensington
• At the end of 2011/12, there were 345 children who were subject to a child protection plan
• The number with child protection plans per population in Hammersmith and Fulham was much higher than the London and England averages and accounted for half of all those in the ‘tri-borough’ area – 151 children
• In Kensington and Chelsea and Westminster, the rates were much lower than London and England averages

• Low immunisation coverage in all three boroughs.
• Significantly high rates of one or more decayed, missing or filled teeth during 2011/12 in Westminster and Hammersmith & Fulham.
• Significantly high diagnoses with acute sexually transmitted infections, including Chlamydia among 15- 24 year olds, in Kensington & Chelsea and Hammersmith & Fulham.

SERVICE MODEL
The Proposed Contract
• WCC procure on behalf of all three boroughs to award a framework agreement for three + two years.
• Hammersmith & Fulham and Kensington & Chelsea will then call off from that framework
Proposed Service model
Aims:The School Health Service will improve the health and wellbeing of children and young people and reduce health inequalities by:
1. Leading and providing a range of services2. Contributing, coordinating and facilitating access
to services3. Enabling, connecting and improving access to
services
Service Design Principles
1. School Health teams in each borough will be aligned with Early Help Localities to improve multidisciplinary working.
2. School health teams will participate in the “Team Around the Child” and “Team Around the School”. School Health Teams will be multidisciplinary and include a broad skill mix.
3. A cluster team will be allocated to a grouping of schools with each having a named lead contact.
4. The named lead contact will undertake termly planning and review meetings with the school to ensure continuity and quality of service and scheduling of interventions

5. The school health teams will be responsive to the needs of schools, pupils and families through:• improved access to advice and guidance
through a website, social media etc.• booking online appointments with the school
health service.
6. The school health service will provide confidential one to one sessions to secondary pupils.
7. The team will be expected to deliver the core requirements as set out in the Healthy Child Programme (5-19yrs)
The ServiceSchools will be provided with:• A 5-19 health web-site which will give them access to a
comprehensive range of guidance and key documents as well as the facility to book appointments.
• A telephone helpline to their cluster team which will be manned from 8.30am to 5.00pm term time.
• Dedicated central email for their cluster team so that all queries/issues can be prioritized and dealt within the agreed timescale.
• A named School Health Team member for each school who will carry out the termly planning and review meetings and ensure that there is continuity of the relationship between the School Health Team and the school.
The SHS Primary will:• Undertake handover & continuation of each child’s health record
from health visiting service• Health Assessment at school entry (via health questionnaires to
parents and carers)• Introduce the school health team and provide local information on
services for families via open evenings, assemblies etc.• Review immunization status (including BCG) and refer as required.• Where physical, emotional or developmental problems have not
been identified or addressed previously enable both the referral and access to the appropriate services.
• Provide families and school staff with information on specific health issues e.g. bed wetting sleep, common infectious diseases, constipation etc… via “Keeping your Child Healthy and Well” group sessions and website.
SHS Primary (contin.)• Provide one to one advice by appointment. • Introduce and prepare schools, families and pupils for the
National Child Measurement Programme (NCMP). • Deliver the NCMP for reception aged & Year 6 children and
feedback outcomes to parents. • Working with the Public Health Commissioned Child Obesity
Prevention and Family Healthy Weight Services Provider provide follow-up of children who are identified as not being a healthy weight.
• Work with families to make lifestyle changes, refer on to other relevant services and participate in delivering the Child Healthy Weight Care Pathway.
• Screen all children between 4 and 5 years of age for hearing and visual impairment in line with the National screening guidelines.
SHS Primary (contin)• Be alert to risk factors and follow local safeguarding procedures
when there is cause for concern.• Contribute to Child Protection processes of the local authority
where the child is resident. • Support health promotion activities within schools, including the
Healthy Schools Partnership Programme, and deliver health promotion interventions as required.
• Support the school if requested in the delivery of health related areas of PHSE, and in particular support teachers in the delivery of Relationships and Sex Education including puberty using lesson plans in line with the Healthy Schools Partnership Guidance for PSHE and on hygiene.
• Provide training and support to school staff on the management of children’s health and medical needs & healthcare plans
SHS Primary (contin.)• For pupils with identified health needs provide liaison and
co-ordination with other health services especially GPs, CAMHS
• Provide termly liaison and referral meetings with the school.
• Actively follow-up children identified with oral health issues according to the oral health care pathway.
• Work with the “team around the school” and relevant other Children and Families services.
SHS Secondary• As per Primary
In addition• Provide confidential wellbeing clinic sessions for pupils by
appointment
Relationships and Sex Education• Via a team of sexual health promotion specialists and the
school health service cluster teams a programme of Relationships and Sexual Health Education will be offered to school in accordance with the Healthy School Guidance on Personal, Social, Health and Economic Education.
• Primary Delivery offer to include:• Puberty• Reproduction• Female Genital Mutilation
Relationships and Sex Education• Secondary Delivery offer to include:
• Sexually Transmitted Infections (STI) information - including importance of the early detection of chlamydia and HIV and promotion of take up of the HPV vaccine
• Contraception information• Condom demonstrations• Signpost young people accurately and effectively to
appropriate services• Pupils with Learning Disabilities:
A programme of young people with learning disabilities appropriate to their level of learning
