Introduction [from Vol 2. p.153]It is very important to properly diagnose the etiology of neck and shoulder pain,since so many different conditions can cause these symptoms. The generaltreatment strategy for neck and shoulder pain consists of first treatingconstitutional problems discovered through palpation, then treating specificdiseases presented in the patient’s medical history, and lastly, addressing thesymptoms with distal points (if still needed). Often treating the underlying medicalconditions will reduce pain at the neck and shoulder. One of the most importanttreatments related to shoulder pain is the Dai­Mai treatment; this treatment is

especially potent for treating shoulder pain felt while raising the arms sideways. The symptomatic treatments are usually added as touch ups at the end of thetreatment and only take a few minutes. Touch­up treatments usually consist ofMaster Kawai’s Yin/Yang treatments, interdermal or press­tacks at distal pointsthat reduced the presenting symptoms, and/or homework given to the patient(self massage, Tiger Warmer massage, etc.) for maintenance.

Master Kawai’s Shoulder Treatments [Source: Vol. 2, p. 163] In these treatments, pairs of points at the wrist are connected to a regular ion­pumping cord and sparked with a Pachi­Pachi stimulator in order to increaserange of motion and reduce pain at the shoulder joint. These treatments can alsobe used for symptoms at the arm such as lateral or medial epiconylitis, etc.

The point couples are as follows:Lu9 and LI 5 Pc7 and TW4 Ht7 and SI5Procedure: The practitioner should hold each pair of points between the indexfinger and the thumb while the patient moves his or her shoulder. The pair ofpoints that reduced the pain and/or increased the range of motion mostsignificantly should be needled (usually with the flow of the meridian orperpendicularly), and connected to the regular ion pumping cords. The black clipof the ion pumping cord should be connected to the needle at the Yang point,and the red clip to the needle at the Yin point. After the ion pumping cord isplaced, the black clip should be back pin of the Pachi­Pachi stimulator and theneedle to which the black clip had been connected should be stimulated verybriefly. After the spark has touched the needle, the black clip should bedisconnected from the Pachi­Pachi and reattached to the needle. This can bedone every five minutes for a total of three times. After the stimulation,interdermal needles can be inserted at the exact angle and location of points thatreduced the shoulder pain and increased the range of motion.

­­ For shoulder pain along the Lung and Large Intestine Meridians related toswollen lymph glands at the neck, the combination of Lung 9 and LI 5 is indicatedfor this treatment.

­­ For injury and spasm of teres minor, infraspinatus, supraspinatus, or teresmajor muscle (which connects the Heart and Small Intestine meridians), thecombination of Heart 7 and SI 5 is indicated for this treatment.

[Source: Vol.1 p.443­444] In the KM style, pain at the fire point of a channel indicates inflammation alongthat channel, which may be concurrent with pain further along the channel.Treatment of the metal and water points of a channel is always indicated toreduce pain at the fire point and often elsewhere in the channel.

­­ LI 1 and LI 2 can be used to treat shoulder pain along the Large Intestinechannel, particularly if there is pain at LI5.

­­ TW1 and TW2 can be used to treat shoulder pain along the Triple Warmerchannel, particularly if there is pain at TW6.

­­ SI1 and SI2 can be used to treat shoulder pain along the Small Intestinechannel, whether or not there is pain at SI 5.

­­ Heart 3 can also be used to treat any shoulder pain along the Small IntestineChannel.

­­ In cases of chronic axiety, the Small Intestine channel can become tight aroundthe shoulder and neck. Heart 3 and Small Intestine 7 are the preferred points totreat this condition. (Source: Vol.1 p. 322)

­­ Lung 10 may become sore in cases of shoulder and neck pain, especially if thepain is related to swollen lymph glands at the neck or to the Lung and LargeIntestine meridians. In this case, the Master Kawai shoulder point combination ofLI5 and Lung 9 is indicated.

Diode Ring Treatment To address shoulder pain, the diode rings should be worn on the Pinky Finger.As always, the diode on the ring should be left facing the thumb.

Conditions Related to Shoulder Pain The following conditions, recognized by KM style, feature pain in or around theshoulder as part of their symptomatic presentation: [Note: Kikko almost always groups neck and shoulder pain together assymptoms, and usually she is referring to the area along the entire uppertrapezius muscle, with emphasis on the neck. It is very rare that she refers tostructures of the shoulder exclusively. For that reason, most of the material listedhere may also be described in the section on neck or thoracic back pain.]

Dai Mai Imbalance [Source: Vol. 1, pp. 195­199]Symptoms ­­ Low back pain at the quadratus lumborum area, especially associated withtorsion

Neck and Shoulder Pain, at LI 15, with occasional radiationdown the Large Intestine channel. Sometimes the Dai MaiTreatment may also be effective for shoulder pain in theSmall Intestine meridian. In general, it is worth trying to treatthe Dai Mai for any kind of shoulder pain that does notrespond to other treatments.*

­­ Knee pain associated with a rotation of the body.­­ Eye problems, especially narrow vision.­­ Hip pain, especially after hip replacement surgery­­ Frequent urination­­ Shortness of breath, asthma and pectoral muscle pain. (In this case,treatments for the lungs are combined with the Dai Mai treatment.

DiagnosisThe patients sit up straight, clasp hands behind his or her head, and rotateat the waist left and right. A Dai Mai imbalance will be evident as arestriction of movement toward the affected side.With hands still behind the head, the patient should bend laterally, elbow tohip. A Dai Mai imbalance will be evident as a restriction of movementtoward the affected side.Pressure pain or tightness should be palpable at the GB 26 area.Pressing GB 26 bilaterally should increase range of motion for the first twomovements. In milder cases, with less restriction of motion, the pressurepain itself will be the primary indication of Dai Mai imbalance.With the patient sitting upright, one side of the pelvis may be higher thanthe other, as if the patient is bending at the waist.Pressure pain may exist between Lung 1 and Lung 2. (If palpating GB 26reduces this pain, it is due to Dai Mai. If Lung 5 reduces this pain, it is alung problem.)

Treatment(Results are better when the patient can sit up straight on a chair.)­­ Check GB 26 against the symptomatic painful areas.­­ If GB 26 is too painful or causes spasm, reduce this pain with a needle atKid 7, 45 degrees with the flow.

­­ GB 26 can be needled perpendicularly, or at a 75 degree angle toward theback. Up to three needles can be placed in the area, in a “chicken foot”arrangement. Eight thread sized moxas should be applied to GB 26. One orboth sides may be treated.­­ Palpate the area above SI 9 (at the posterior edge of the deltoid) for atender spot.­­ Needle this spot upwards and laterally, and add seven to ten moxathreads. SI 9 contralateral to the primary GB 26 is the primary treatmentspot, but both sides should be treated.­­ Needles are retained for fifteen minutes.Consider the other structural treatments as well.

Four Corner Imbalance [Source: Vol.1, pp. 192­194] SymptomsThe Four Corner treatment should be the primary consideration for anypatient with an obvious shift of a shoulder or hip.The following signs and symptoms may be treated with the Four Cornertreatment primarily, or they may require other corresponding treatments aswell:­­ Shoulder and Neck Pain, carpal tunnel syndrome, thoracic outletsyndrome, upper back ache or muscle spasm between the shoulder blades,lower back ache, sciatica, hip pain, knee pain, sacroiliac dysfunction,headache (especially from the neck up), and unilateral pain of the trunk andlower limbs, chest congestion, asthma, any digestive or uro­genitalcomplaints where there is an appearance of structural imbalance.

Diagnosis ­­ With the patient standing up straight, observe any imbalance in theimaginary rectangle that connects the spines of the shoulder blades withthe iliac crests of the pelvis.­­ With the patient lying prone, palpate for a painful area between theshoulder blades. Then palpate for a painful area under the iliac crests, onthe superior portion of the gluteus muscle.

Treatment ­­ Palpate for the tightest or most painful spot under the spine of thescapula, ipsilateral to the upper back tension. Needle this spot in thedirection that reduces the pain, typically 45 degrees medially or laterally.Two or three needles may be required in this area.­­ Insert a needle perpendicularly in the tightest or most gummy area at theupper portion of the gluteus muscle. If this area is cold, burn Kyutoshin

moxa on the needle.­­ It is important to treat both sides.­­ The needles are retained for fifteen minutes. If pain remains, repeat thistreatment and consider adding the sacroiliac or Dai Mai treatments.­­ If good results are achieved with this treatment, leave interdermalneedles in the needling locations.

Visceroptosis [Source: Vol.1 pp.215­221] Symptoms: The internal downward pulling of connective tissue due to thisdropping of the internal organs may create a series of signs and symptomsfrom the neck and shoulders to the feet:Lower back ache, groin discomfort, tired legs, thigh and knee pain, sciatica,Stiff Shoulders, frequent urination, gastritis, headache, GYN problems,fatigue, exhaustion, Oketsu, Trapezius Muscle Tension**, tightness in the chest,headaches, indigestion, belching, nausea, constipation, diarrhea.

Primary Diagnostic Areas:­­ Pressure pain above the inguinal ligament from the ASIS to the pubic bone­­ Pain at the “Inner Yin” region (along the Kidney meridian) one­third of thedistance from the inner knee to the groin.­­ Pressure pain at Stomach 13 Secondary Diagnostic Areas: ­­ Pressure pain three fingers above Spleen 10.­­ Weakened or atrophied muscles of the thigh/Swollen knees­­ Pulsation at CV 9­­Unilateral pressure pain at Huato Jiaji of L5.

Treatment:Primary treatment point: St 13 (needled towards LI 15, to reduce pain at theinguinal ligament.Supporting points: ­­Stomach Qi lines ­­ Inner Yin (a.k.a. “trapezius releasingpoints) –GB31Back Treatment: ­­ GB25 (needled toward spine) –BL23 (needled downwards) –BL 25 (needled toward umbilicus)

Blood Pressure Imbalance [Source: Vol.1 pp. 120­126]In cases of high or low blood pressure, the neck and shoulder areas may be stiff,tender, or appear thicker than normal. The patient may complain of dizziness,headaches and fatigue as well. Sometimes, the neck and shoulder pain is

induced by an accident or wrong movement, but lingers and does not react totreatments because of the underlying blood pressure problem. In these cases,blood pressure and Oketsu treatments are essential, in addition to any othertreatments chosen to address the neck and shoulder pain.

Standard Blood Pressure Treatment: 1. Needle Sp6 and Sp9 to reduce pain at Oketsu, adrenal and Kid 13 reflexes.2. Needle Pc3 and Pc5 to reduce pain at CV 173. Eight thread­sized moxa stimulations at Master Nagano’s blood pressure pointbehind the third toe.4. Needle the Kori at the lower border of the spine of the scapula, and addKyutoshin moxa.

With a rapid pulse and stubborn neck and shoulder pain, the following pointsshould be added to the blood pressure treatment:­­ CV 14 (toward CV 15) ­­CV 12 (toward CV 15) ­­ Kid 13­­ CV 6 (with Kyutoshin) ­­Bl10/GB20 area (needled into a gummy/tight area,stimulated but not retained.

With low blood pressure and stubborn neck and shoulder pain, the followingpoints should be added:­­ GV 20 ­­Kid 1

Liver/Gallbladder­Type Neck and Shoulder Pain [Source: Vol.2pp. 155­156]Symptoms:­­ Right­sided neck and shoulder pain, with the right Huato of C3 and the RightBl17/BL18 areas particularly tender.­­ Patients with gallstones may feel pressure or spontaneous pain at the rightscapula, particularly SI 11.­­ Pressure pain at the Oketsu reflex­­ Pressure pain at right Liver 14, and fullness and/or pain under the right ribcage.

Treatment:To reduce neck and shoulder pain, BL 35 and BL 25 should be needled at theexact angle and location that reduces pressure pain at the BL17/BL18 area aswell as the neck.Liver Deficiency (Dull pain at right Liver 14): Needle and moxa Liver 1 to reducepain at right Liver 14 and right Huato C3.

Liver Excess (Sharp pain at right Liver 14, and/or a congested, swollen liver):Needle, all on the right side, Kid7, Sp7, Ht3, Japanese Pc4. Liver 3 can beneedled on the right side, against the flow of the meridian, to further reducepressure pain at Liver 14 as well as the neck pain on the right side.Excess of the Liver meridian: If Liver 2 is painful on palpation, Liver 4 and Liver 8should be needled.Gallbladder disorders: Pressure pain at the gallbladder as well as pain at the rightscapula can be treated with the right Metal and Water points of the Gallbladderand Triple Warmer meridians (GB43, GB44, TW1 and TW2)

Eyesight Problem­Type Neck and Shoulder Pain [Source: Vol.2pp.160]Patients complaining of eyesight problems (cataract, glaucoma, eye glasses ofimproper prescription), may complain of severe neck and shoulder problems aswell, especially after using their eyes for a prolonged period of time (reading,watching TV, computer work). Symptoms: Pressure pain and stiffness at the upper part of the trapezius musclefrom the occiput to the superior border of the scapula.

Treatment Points:Ventral: ­­Hashimoto’s eye points at the inner thigh (two points along the linebetween liver 8 and liver 11, one located one­third of the way, the other locatedtwo­thirds of the way.­­ BL2 ­­GB1 ­­GB3 ­­TW23Dorsal: ­­Huato T4 ­­GV14

Nasopharyngitis and Swollen Lymph Nodes­Type Neck andShoulder Pain [Source: Vol.2. p 159]Patients presenting with neck and shoulder pain along the levator scapulaemuscle very often also complain of chronic sinusitis, rhinitis, upper respiratorysystem infections, allergies, and a generally weaker immune system. To see ifpain at the levator scapulae muscle is related to the immune system, test if it isreduced by massage at the immune points.Treatment consists of the immune and nasopharyngitis treatments, described onpages 59­65 of volume one and pages 70­72 of volume two, respectively. Athome, the patient may do nasopharyngeal washes, tape diode rings on the mostsensitive spots on the face, or perform Tiger Warmer massage on TW16.

Trapezius muscle pain that spreads along the back is characteristic of sugarimbalance (mid back in particular), respiratory problems, sport injuries or amisaligned spine.Trapezius muscle pain that is concentrated at the neck and shoulders ischaracteristic of visceroptosis, high blood pressure, stress, and weight lifting.Since the trapezius muscle is innervated by nerves at C3 and C4, it is possiblethat cervical or head injuries can create trapezius muscle pain. In this case, the I­Hi­Kon treatment is indicated.The points that release the trapezius muscle directly are the “Inner Yin” points,located on the Kidney meridian, one palm width superior to Kid10. If the pain is found to be due to a sugar imbalance, in which case the trapeziusmuscle will be most tender directly over GB21, then the sugar imbalancetreatment is indicated. In particular, SP9 is the point that reduces pressure painat GB 21.
