    1 INTRODUCTION Smart Staff International Pty Ltd (SSI) was founded over 20 years ago with the objective of providing the resource processing industry with operational and maintenance training material that people would use. This objective was borne out of the founding partner’s first-hand experience of seeing poor training systems result in repeatable failures that led to production losses, excessive costs, serious injuries and the fatality of a close relative.

    Improving processes that frontline employees are required to follow is our core objective and summarised in our principle of “Creating Practical Workplace Systems”.

    This underlying principle remains consistent throughout all of our services and supports our goal of providing workplace training and safety systems that are practical, easy to use, have clear links to the risk exposure they are mitigating, reduce ongoing costs and save lives.

    Our unique approach has seen SSI expand its services from minerals processing to oil and gas, mining, heavy industrial, food processing and utilities industries in the following areas:

    • Design and implementation of front line hazard identification, risk assessment and risk control (HIRARC) processes

    • Technical writing and training material development • Review, design and implement high risk work, hazardous energy control and permit to work systems • Confined space identification and risk assessment • Training and mentoring of safety advisors, permit authorities and isolation officers • Isolation point labelling and P&ID/PFD development and review • Auditing and gap analysis

    2 JOB SAFETY ANALYSIS SSI recognises Hazard Identification, Risk assessment and Risk Control (HIRARC) as being a critical skill and behaviour all companies need embedded in their workforce to enable the effective management of workplace hazards.

    Hazards and risks are often confused and the terms often used interchangeably. For this reason, SSI works closely with clients to identify the hazards (unique or generic) associated to their operations and tailors the development of corporate HIRARC procedures and training packages to ensure effective and end-user focussed training presentations and competency assessments are provided.

    SSI has an extensive team of technical writers, trainers and mentors who have many years of ‘on-the-floor’ experience developing JSA’s to manage hazards and control risks in their workplaces prior to SSI engaging them.

    SSI has 20 years of experience working globally with clients to develop and implement HIRARC systems in both first and third world countries and therefor understand the importance of developing end-user focussed systems and training packages.

    2.1 Our Approach SSI utilises a structured approach to ensure the client is delivered an effective and efficient system and recognise the need to effectively plan and execute implementation projects. SSI provides support throughout all phases of HIRARC projects and provide SME’s to assist clients to not only audit and design systems but train, mentor and competency assess personnel in the field.

    3 TECHNICAL WRITING SSI work in close partnership with the client in designing corporate/site risk management procedures, operating procedures, maintenance procedures, isolation procedures, training and assessment programs with direct application and relevance to organisational requirements, business risk, internal policy and legislative requirements.

    SSI have a suite of templates and generically developed documentation for all types of processes/industries that will decrease the time spent producing documents from the ground up. The use of photographs and diagrams within the documents enhance the learning process providing personnel with a better level of understanding of the task at hand and the organisations workplace standards and practices. Assessments can be aligned to relevant competency units to enable personnel to receive recognised qualifications.

    Smart Staff International currently have a core team of technical writers working across a diverse range of disciplines and industries both in Australia and internationally. Our team consist of “hands on” personnel who all have many years of relevant industry experience behind them. They have been involved in the development and application of systems and procedures at an operational and managerial level and understand what works and how to engage employees.

    3.1 Our Approach Our ground up approach ensures the development of technical documentation, training material and assessments are tailored specifically for your organisation. Over the last 20 years, from the breadth of work and diversity of clients that we have worked with, SSI has identified the key drivers for the successful delivery of a technical writing project. This experience has resulted in SSI developing a technical writing project delivery process which is clearly defined and explained to the client and includes:

    4 PERMIT TO WORK AND LOCK OUT TAG OUT The development and implementation of a new or revised Permit to Work (PTW) and Lock Out Tag Out (LOTO) systems can increase workloads significantly during the development and implementation stage. Additionally, poorly designed systems that do not align with industry leading practices can result in largely inefficient practices being implemented and in the worst case these systems can result in personnel being exposed to uncontrolled hazards.

    Smart Staff International is able to provide experienced personnel to assist with any or all stages of PTW and LOTO system development and implementation.

    Comprehensive systems documentation can be developed from agreed process flow charts by SSI’s technical writers and LOTO and PTW specialists. These include corporate/site procedures, associated forms and all training and competency material.

    Defining and implementing successful PTW and LOTO systems can be complex. To be efficient and effective, a safe system of work requires a comprehensive review of procedures, equipment and training, as well as consideration of how work activities are undertaken. If badly managed the system can be incomplete, confusing, time consuming and in the worst-case neglect employee’s safety and potentially lead to accidents.

    Good PTW and LOTO procedures and practices are not just a question of keeping the company compliant with the latest regulations and legislation. To achieve the best results PTW and LOTO systems must be supported by the entire workforce, generate operational support and enhance the viability of the organisation.

    PTW and LOTO systems are integral in the effective application of safe systems of work. PTW and LOTO systems need to ensure that the following processes are undertaken for all work activities:

    • Appropriate levels of hazard identification, risk assessment and risk control (HIRARC) are undertaken • Safe access is provided to plant and equipment • High risk activities are formally controlled • Interacting activities (SIMOPS) are identified, assessed and managed

    Whilst PTW and LOTO systems must be robust in their entirety to ensure health, safety and environmental objectives are met, they also need to be efficient and practical to apply to generate operational support and commitment.

    Industry Leading Practice (ILP)1 PTW and LOTO systems will provide:

    • Consistent application of the systems • Established and documented controls for personnel safety • Reduction in health, safety and environmental incidents • Planning and control of multiple workgroups working in close proximity to each other • Reduction in lost production (work is known and planned through PTW/LOTO systems) • Efficient processes for managing work activities whether applied at an individual level or managing a

    shutdown situation involving large numbers of employees and external contractors • Increased communication between various disciplines and work parties • Efficient and effective management of maintenance activities through planning of daily, routine and

    campaign tasks

    5 ISOLATION POINT LABELLING AND P&ID/PFD DEVELOPMENT AND REVIEW

    SSI understands the importance of having correct valve identification assigned with unique identification numbers to ensure accuracy of isolation lists and the execution of isolations.

    It is also important to maintain P&ID’s and the isolation point register in a current state, to enable the location of all isolation points and their assigned unique identification number to be readily traced and to ensure that assigned isolation valve numbers are not duplicated.

    5.1 Our Approach Using available on-site technical specification documentation SSI can determine/present the client with:

    a) Type and size of isolation valve labels b) Valve numbering nomenclature c) Red-lined or as-built P&ID’s d) Fitting of isolation valve labels

    5.2 Our Experience Having undertaken numerous of these projects SSI clearly understands project intricacies such as the type and size of identification label to be used and any concerns with specific locational requirements.

    The following table provides some examples of the breadth of SSI’s experience with marking up and “as building” clients piping and instrumentation diagrams (P&IDs).

    Newcrest Mining – Lihir Operations (PNG)

    This project encompassed the entire process plant and included: • assigning unique identification numbers, locating valves and attaching tags • photographing and cataloguing each isolation point (and tag) • developing database (isolation point register) for uploading of isolation valve

    and electrical isolation point details into the Engineering Department plant data system

    Approximately 140 P&ID’s and 6,000 isolation points were covered during the course of this project

    ERA Ranger Uranium Mine (NT)-2 projects

    1. Field audit of Water Operations pipework, develop draft P&ID’s, attach valve labels and develop valve register

    2. Field audit of process plant pipework, develop draft P&ID’s, attach valve labels and develop valve register

    CSF Proteins (VIC)

    This project was completed on time and on budget with 214 manual isolation valves and 163 electrical isolation points labelled and marked up on P&ID’s at the Hi Temp Rendering Plant. Details: • 5 x P&IDs marked up to as built • Development of 183 equipment isolation lists • Development of an isolation point register • Draft, subsequent review and finalisation of isolation list template • Draft, subsequent review and finalisation of isolation permit template

    6 ELECTRONIC PERMIT TO WORK PREPAREDNESS The transition from paper based to an electronic PTW system requires a lot of information gathering from asset registers, P&ID’s, SLD and plant inspections.

    Having undertaken numerous e-Permit transition projects, SSI has developed proven processes to work with clients and their selected e-Permit provider. Our process ensures that all of the required information is collected and presented to the e-Permit provider in a format that allows for efficient uploading of the data into any e-Permit system.

    Concurrently, SSI also identify, in consultation with the client, any gaps that require addressing prior to transitioning. For example:

    • Audits of current paper based processes (high risk procedures and permits)

    • Reviews of drawings for accuracy (ensure they are “as built”)

    SSI achieves this through a combination of our extensive experience, as detailed fully in other parts of this capability statement, in:

    • Isolation point labelling,

    • P&ID red-lining and as-building,

    • Over 20 years of implementing Permit to Work and Lock out Tag Out systems,

    • Assisting clients with the transition from paper based to e-Permit systems.
