ABOUT TCOM TCOM: Transformational Collaborative Outcomes Management is an approach grounded in
the philosophy of a shared vision to help people achieve wellness goals as they navigate
healthcare, child welfare, juvenile justice, behavioral health, education, and other complex
systems around the world.
With implementations present in all states in the United States and a strong presence around the world, the TCOM team and our partners focus on the process of change. Our philosophy is rooted in
strategy and tactics. All key decisions should be informed primarily by person characteristics and specific collaborative assessment processes have been developed to support the TCOM
philosophy. These include the Child and Adolescent Needs and Strengths (CANS), Family Advocacy
and Support Tool (FAST) and Adult Needs and Strengths Assessment (ANSA). These commonly
used measures are strategies that fit the philosophy of TCOM and allow us all to use an
understanding of the people we serve as a guide in the decision making process.
ABOUT THE CONFERENCE The annual TCOM Conference is jointly hosted by The Praed Foundation and Chapin Hall at
the University of Chicago. The 13th Annual TCOM Conference, will bring together over 400 attendees
discussing issues impacting all levels in various systems of care.
This year's conference theme is Leadership. All change requires leadership but it takes many forms and often has little to do with formal power and authority. We will look at Leadership in all its manifestation within collaborative processes and complex systems.
