TheaimsandobjectivesofMostonGrangeCareHome.TheaimforMostonGrangeistoprovideappropriate,highquality,nursingcarethatmeetstheneedsofServiceUserswithvariousformsofdementia.Weaimtoprovidethisviapromotingpersonalgrowthanddevelopmentwithinapersoncentredapproachthatrespondstoanychangesintheindividual’sneedswhilemaximisingtheirabilitiesandpotential,inawarm,friendly,welcomingandhomelyenvironment.ObjectivesTheprimaryobjectiveistofacilitateasnormalaspossiblealifeforourserviceusers,takingintoaccounttheirillness,individualneedsandwishes.Weprovidearangeoftreatmentsforourserviceusersthroughprovidingapersoncentredapproach,whichensuresthatneedsaremetforthoseserviceuserswherededicatedtreatment(s)willenhancetheirprospectsforrehabilitation,forthosewhocanimprovetheirskillsandqualityoflife,orforthosewhoareunabletoprogressbutwillcontinuetoneedaplacementforlife.Treatmentandcareisdeliveredwhereserviceusersareinvolved,whereverpossible,inthedevelopmentoftheirindividualcareplansandhaveanamednursetoensureandenhancetheindividualnatureoftheircare.Agreatdealofattentionisgiventoprovidingafriendlyandhomelyatmosphereoneachunit,sothatserviceusersfeelcomfortablewithintheirenvironment.TheRegisteredProviderandRegisteredManager

OverviewoftheServiceEHCMostonGrangeisawholly-ownedsubsidiaryofEquilibriumHealthcareLimited,whichownsandoperateshospitalsandcarehomesspecialisinginthetreatmentandcareofpeople,withvariousformsofNeurologicalDisorder,forexample:

• EarlyOn-SetDementia• HuntingDisease• Korsakoffs• AcquiredBrainInjury• MentalHealthDisorder• DegenerativeDisorders

AccommodationAccommodationisarrangedoverfiveunitswithinthetwoserviceserialblocks.Eachunitprovidessinglebedroomswithen-suitefacilities,withatotalof59.Thereareadditionalbathroomandshowerroomsandtoiletfacilitiesoneachunitandeachhasakitchen,diningroom,loungeandleisurefacilities.Thereareseveralotheractivity/leisureroomsavailablewithintheadministrationblockforServiceUsers’use–includingabeauty/hairdressingsalon.MostonGrangeisanosmokingfacility.TherearedesignatedexternalsmokingsheltersforServiceUsers,staffandvisitors.DeanvaleandMapledeneUnitsHerecareisprovidedforbothadultmenandwomen.TheunitprovidesstaffsupportforServiceUserswhoareabletoproactivelyengageandsociallyinteract.Eachunithaveaccesstotheirindividualenclosedgarden.WoodsideUnitTheunitfacilitatesadultmenwithaNeurological/DegenerativeDisorder.Ittoohasanenclosedgarden.HollybankUnitThisunithasbeensplitintotwosmallerenvironmentstoenablestafftocaterforthemorechallengingserviceusersinaspacioushomelyenvironment.HollybankViewThisunitprovides5bedswhichareforserviceuserswithchallengingbehaviours.HollybankValeThisunitprovides6forbothadultmenandwomen.TheunitprovidesstaffsupportforServiceUserswhoare able to proactively engage and socially interact. Each unit have access to their individual enclosedgarden.

The home is situated within walking distance of Newton Heath and Failsworth, where local facilitiesincludeshops,restaurantsandrecreationalamenities.TheHomeisadjacenttoDeanLanetramstation, isconvenientforthemainbusroutestoManchestercitycentreandtheM60motorway.PersonalSpaceAllbedroomshaveen-suiteWCandwashingfacilitiesandarefurnishedwithbedandbedding,bedsidecabinet,chestofdrawers,wardrobe,chairandcurtains.ServiceUsersareactivelyencouragedtopersonalisetheirbedrooms,inordertohelpdevelopasenseofbelongingandprivatepersonalspace.Allbedroomdoorshavealock,forwhichServiceUsershaveakeyandwhichisaccessibletostaffinemergencies.Allbedroomshavealockablespaceforstorageofpersonalitemsandmedication(ifappropriate).Allfurniture,furnishings,beddingorcurtainsbroughtintothehomemustbefireretardant.Anyelectricalequipmentmusthaveasealedplug,andifoverayearoldmustbePATtestedbeforeitcanbeapprovedforuse(staffwillarrangethis).CateringOnadmissiontheCookwillbegiveninformationaboutanyspecialdietaryrequeststhattheserviceusermayhave(e.g.lowfat,vegetarian,kosher,halal,fortified,soft,lowsugar,glutenfreeetc.),andanylikesordislikesthatheorsheexpresses.TheCookwillbehappytodiscussindividualrequirementswiththeserviceuserand/orrelativeorotheradvocate.Ifaserviceuserneedsanyassistancewiththeirdietaryintakethiswillbeidentifiedandactionformulatedwithintheindividualplanofcare.Mealsarevariedandmealtimesreasonablyflexible.Achoiceofatleasttwooptionsisalwaysavailableandallmealsareattractivelypresented.Achoiceofhotandcolddrinksaswellassnacks,includingfruit,isavailablethroughoutthedayviathekitchenetteoneachunit.LaundryMostonGrangeprovidesanon-sitelaundryforserviceusers’clothing,butcannotacceptanythatrequiresspecialists’techniques–e.g.dryclean/handwash/donottumbledry.TelephonesServiceuserscan,iftheywish,havetheirowntelephoneinstalledattheirowncost(installation,linerental,callcharges.)SmokingandAlcoholMostonGrangehasanosmokingpolicy,tominimisetheriskoffireandthedetrimentaleffectsofsecond-handsmoketootherserviceusersandstaff.Therearedesignated,coveredsmokingsheltersinthegardenareasforthosewhowishtosmoke.

Theconsumptionofalcoholisdiscussedwiththeserviceuserandassessedonanindividualbasis.RecreationalFacilitiesTheactivityco-coordinatorsareresponsiblefortheplanningofaweeklyprogrammeofeventsthattakeintoaccountserviceusers’individuallifestyles,interests,physicalandmentalabilities.Onadmissiontothehometheactivityco-ordinatorwillmeetwiththeserviceuseraswellasrelativeand/oradvocatediscussingtheirinterestsandtheavailablefacilities.VisitingVisitorsarewelcomedatanytime,aslongastheysignthevisitor’sbookandabidebythehouserules–e.g.smokeonlyintheallocatedgardenareas,respectfellowservicesusersandstaff,etc.Wealsoaskthat,whereverpossible,theyavoidthetimessetasideforstructuredactivities.MostonGrangerespectsandsupportstheserviceusers’righttoexpressapreferenceastowhetherornottheywishapersontovisit.StaffwillprovideassistancetoServiceUserswhoneedhelptoresistunwantedvisitorsandassistancetovisitorstoresistunreasonabledemandsforvisiting.ServiceUsersaresupportedandencouragedtomeetwiththeirvisitorsinanenvironmentinwhichtheyfeelcomfortable;thisincludescommunalareasortheserviceUser’sbedroom.IftheserviceUserwishestomeetinaprivateroom,thiscanbearranged(withsufficientnotice).WhereserviceUsersreceivevisitsfromchildren,itisnecessarytoensurethatthisoccursinasafe,comfortableandpositiveenvironmentthatenhancesfamilywell-beingandisconsistentwithandsupportedbyindividualcareplans.ReligiousSupportServiceUsersareopenlyencouragedtoattendreligiousserviceormeetwithareligionsadvisoroutsidethehome,inaccordancewiththeirindividualwishes.IfaserviceUserwishestobevisitedbyareligiousadvisororhaveaprivateservicewithinthehome,thenanappropriateroomcanbeprovided.AdvocacyAserviceUsermayfeelthattheywouldlikethesupportofsomeonewhocanspeakontheirbehalfandexpresstheirindividualwishesorbeliefs.ServiceUsersareencouragedtoexpresschoiceaboutwhotheywouldprefertocarryoutthisrole,beitafriend,relative,nurse,doctor,healthprofessionalorsomeonewhoiscompletelyindependent.StaffwillbehappytodiscusstheoptionsavailableorassisttheserviceUsertomakecontactwithanindependentadvocate.RespectandDignityRespectanddignityisexpressedbytheactionsandbehaviourofstaffwhichinclude;knockingondoorsbeforeentering,discussingissueswithServiceUsersinprivate,treatingeachserviceUserasanindividual,offeringactivitieswhichenablethemtoexpresshimorherselfasanindividual,andspeakingwiththeserviceUserbytheirchosentitle.

ConsultationwithServiceUsersServiceUsers’consultationmeetingswillbefacilitatedatleastonamonthlybasis;thepurposeofthesemeetingsistoencourageServiceUserstotakeanactivepartwithinthedevelopmentoftheservice,byvoicingopinionsandsuggestions.CommitmenttoCarers,RelativesorSignificantOthersMaintainingcontactwithrelativesandfriendsisrecognisedasanimportantelementinanindividual’streatment.MostonGrangeaimstoworkinpartnershipwithcarers,relativesandsignificantothers,listeningtoandrespectingtheirrightsandopinions.Tothisend,carers,relativesandsignificantothersareactivelyencouragedtoparticipatewithintheindividual’scareplans.Furthermore,carers,relativesandsignificantothershavetheopportunitytoinfluencetheoperationsofthehomethroughtheFamilyForum.Thisforumisheldevery3monthsMinutesareproducedanddistributedtothecarer,relativeorsignificantother.Copiesarealsoavailableontheinformationboardlocatedintheunitsandreceptionarea.ServiceUsersareencouragedandwherenecessaryassistedtomaintaincontactwithrelevantothers,vialetter,telephoneand/oremail.ServiceFeedbackSurveysBothServiceUsersandtheircarersorrelativemembersareencouragedtocompletetheservicefeedbacksurvey.Servicefeedbacksurveys(whichcanbecompletedconfidentially)aresenttotheserviceUserandtheircarersofrelativemembersannuallytoseekviewsabouttheservice.SupportwillbeofferedtoServiceUserstocompletethesurvey.TheresultsofthequestionnairesarethenanalysedandbroughttotheDeputyManager,whodecidesonwhatactionstotake.Theresultingactionplanisavailabletoallandwillbedisplayedontheunitandinthemainreceptionareaofthehome.

StaffToensurewemaintainthehighestqualityservice,recruitmentandretentionofhighcalibrestaffisofprimaryimportanceatMostonGrange.Theselectionprocessisrigorousandonlythestrongestandmostsuitablecandidateswillprogress.Onceonboard,afullstafftraininganddevelopmentprogrammeisputinplacetoensurethattheyarekeptup-to-dateandfullyequippedwiththetoolsandknowledgetheyneedfortheirparticularroles.TheDeputyManagerregularlyreviewsthestaffingarrangementstoensurethatstaffinglevels,skillsmixandstaffingratiosareoptimised.Eachunithasaqualifiednurse,supportworkers.Inaddition,therearethreefulltimeactivityorganiserswhoworkacrossallunits.

ExperiencePoliciesandproceduresareinplacetosupportandguidestaffwithintheirroles.Theseareavailableuponrequest.

StaffTraining&DevelopmentMostonGrangeregularlyreviewsthestaffingarrangementswhilstensuringthattheskillmixandstaffingratiosaremaintainedtomeettheclinicalneedoftheservice.Staffjoiningtheteamundergoaninductionandtrainingprogrammeduringthefirstsixmonthsofemployment.Trainingmodulesinclude:

• SafeguardingofVulnerableAdults• Basicfoodhygiene• Teamteach(positivehandling)• ManualHandling• Basicfirstaid(includingCPR)• Riskassessment• Basicprinciplesofcare• Firesafety• Healthandsafety• MentalHealthAct• MentalCapacityAct• DoLS(DeprivationofLibertySafeguarding)• Dementiaawarenesstraining• Cultureanddiversitytraining

Thislistinnotexhaustive,andeachmemberfollowshisorherownindividualiseddevelopmentplan,whichisformulatedandagreedwithinthestaffperformancereviewsystem.Inlinewithclinicalgovernance,itisessentialthatMostonGrangeoperatesawell-definedandrobustsystemofclinicalsupervision.Teamclinicaldiscussion/reflectivegrouplearningaswellasindividualresponsibilityforContinuingProfessionalDevelopment(CPD)shouldsupportthis.Formalclinicalsupervisionwilloccurasaminimumofonceeverymonthwithinvariousapprovedformats.TherapeuticPractitionerServicesTheremaybetimeswhenserviceusersrequireinputfromotherpractitioners,forexample;

2. TheorganisationalstructureofMostonGrangeCareHome3. ThekindsoftreatmentandanyotherservicesprovidedforthepurposesofMostonGrangeCareHome,the

rangeofneedswhichthoseservicesareintendedtomeetandthefacilitieswhichareavailableforthebenefitofpatients

ServiceUserCriteriaItisourpolicytoadmitpeoplewhomeetthefollowingcriteria:

• HaveaNeurologicalDisorder/DegenerativeDisorders• Areover18years’old• Maleorfemale• Havefundingagreedandinplace• Mayormaynotdisplaychallengingbehaviour

ThehomecannotacceptreferralsonbehalfofpeoplewhoarecompulsorydetainedunderthesectionsoftheMentalHealthAct1983(withtheexceptionofpersonsonguardianshiporders,conditionaldischarge,CommunityTreatmentOrderandextendedleave).

ReferralsandAdmissionsAtMostonGrange,webelievethateverypersonwhoisreferredandadmittedshoulddosoasaplannedprocessofcare.Theyshouldbesupportedandempoweredthroughtheprocess,ensuringthatanappropriatecarepackagecanbeprovidedfortheindividual.TheDeputyManagerwillconsiderreferralswithreferencetoeligibilitycriteriadescribedbelow:

• Acomprehensiveneedsassessment(includingdiagnosis,livingskills,disabilities,inpatientandhospitalhistory).

• Acurrentcareplanorcareprogrammeapproachsummary.• Ashortsocialreporttoincludesocialhistory,criminalhistory,medialhistoryandpsychologicalhistory.• Currentriskassessmentormostrecent.•

Theywillalsobemadeawareofthedocumentationthatwillbemaintainedrelatedtotheircare.FireSafety,HealthandSafetyandSecurityMostonGrangehasanaddressablefirealarmsystem,whichidentifiesexactlywhereanalarmordetectorhasbeenactivated.Firechecksarecarriedoutweeklybyourownstaffand6monthlybyqualifiedfireequipmentengineers.Staffreceivefireawarenessupdatesonasixmonthlybasis.Theentrancedoorstoeachunitwithinthehomehaveautomaticdoorlockingsystems,whichpreventaccesswithoutanauthorisedelectronicswipecard.Thesedoors(andfiredoorsheldopenelectronicallyinthenormaloperationofthehome–e.g.corridordoors,diningroomandloungedoors)areautomaticallyreleasedintheeventofthefirealarmbeingactivated.Alldoorsarefiredoorsandwhenclosedshouldprovideprotectionfromfireandsmokeforatleast30minutes.Intheeventofthefirealarmsounding,occupantswouldbeadvisedtomovebehindthenewfiredoorawayfromthesource,ifindoubttheyareadvisedtoleavethebuildingviathenearestfireexit,whichareclearlysigned.Thefireassemblypointislocatedwithinthecarparktotherearofthebuilding.Toassistwithfireprevention,smokingiscompletelyprohibitedforbothserviceusersandstaffanywherewithinthebuilding,thoughtheymaysmokeinthedesignatedsmokingareasinthegardens.
