Box 146 Montpelier, Ohio 43543 Dear Ms. Lucus: On January 4, 2008, Tanushree Courlas conducted an inspection of the Holiday City Sanitary Sewer Overflow (SSO). Mr. Patrick Oxender, Superintendent, was present and provided information regarding the events leading to and actions taken since the overflow occurred. During our visit, the following was observed: 1: A portable diesel powered pump was in place and directing wastewater from the Pump Station wet well to the discharge line to the Montpelier Sanitary Sewer, System. 2. Residual sludge was observed around the fenced area as well as down towards the ditch. Any remaining sludge, or other overflow debris, needs to be treated with lime and disposed of as required by your National Pollutant Discharge Elimination System (NPDES) permit. 3. Since all three available pumps, including the spare pump, were inoperable during this event, the maintenance plan for the pump station needs to be re- evaluated. A functioning spare pump must be available at all times. The failureof all three pumps needs to be investigated to determine the exact cause and to prevent future un-permitted discharges. The results of this investigation should be submitted to our office by March 28, 2008. 4. During further investigation, it was observed that the manhole upstream of the lift station had also overflowed. This SSO was not reported as required by your permit. This is a violation of your NPDES permit. A 5-day follow-up report should be sent separately for this SSO. Residual sludge was observed around the affected manhole. Dear Ms. Lucus:
