Student Health ServicesPortal Access and Form Completion 1. Access Health Portal via www.miamioh.edu/health
2. Click on Medical Form Web Portal
3. REGISTER – highlighted in red . Using unique ID, birthdate, an email address for password to be created and a security question
4. Upon receipt of email, click the link provided in the email to set up portal password
5. Log in with unique ID and newly created password. This will take you into the portal where they can access the forms to complete and submit
6. There will be 2 forms to complete.
7. Have immunization records available

Student Health ServicesCompliance Highlights Deadline – May 15th Holds Are Temporarily Lifted During Orientation
Failure to Comply With All Required Vaccines and Form Completion Results in a Hold on the Student Registration Account
This Will Prevent the Student From Making or Changing Any Schedules/Classes
They Will Be Prevented from Registering for Classes Next Semester
Non Vaccination Can Result in Class Cancellation

* If a student has a declination of vaccination on file and there is a campus outbreak the student may be required to leave campus during the outbreak.

on Miami University’s Oxford Campus.• Student Insurance Offered by Miami University
for 2016-2017 Through Aetna. • Effective Dates: August 1, 2016 - July 31, 2017.• Annual Cost For Student Insurance: $1,375.00• If Covered by Health Insurance, Student Must Submit
Waiver to Avoid Being Charged for Insurance Offered Through Miami University via Aetna. Waiver available at aetnastudenthealth.com/MiamiOH*** *** it is advised to check with your insurer regarding network coverage benefits

Charged for the Aetna Offered Student Insurance• To Avoid Receiving Reminder Emails Regarding the Waiver
Deadline, Consider Manual Enrollment via the Aetna Waiver Site @ aetnastudenthealth.com/MiamiOH
• Insurance Fee Will Be Applied to the Tuition Bill ( bursar account) in July
