Introduction
A comprehensive assessment of learning needscan be extracted from nursing historyandphysicalassessmentfindings along with information fromthe clients family. The assessment also includesclient characteristics might affect the learning
process, such as readiness to learn, motivation forlearning, and literacy levels. In addition toextracting data through interview, the nurseshould also observe the capability and clientsneeds. Learning needs can also be identified fromclient to nurse questions about things they do notknow or are not skilled in doing so.
