1. Introduction

This document provides instructions for personnel designated to troubleshoot issues with the OpenElect Voting System (OVS) arising on Election Day at the voting precinct. The OpenElect Voting Optical scan (OVO) and OpenElect Voting Interface (OVI) are the OVS components designed to accept voter input at the Election Site.

The OVO is a full-page dual-sided optical scan system that scans and validates voter ballots and provides a summary of all ballots cast.

The OVI enables disabled voters to vote independently. An audio interface, an ADA-compliant keypad, a Sip and Puff device and a set of headphones are provided. The voter uses the keypad, touchscreen or Sip and Puff device to navigate the contests, make selections, and print an official Ballot; but not cast a vote at the OVI.

When the voter finishes a ballot entry and prints an official ballot with the voter’s selections, the voter places the voted ballot into a secrecy envelope while carrying it to the ballot box. The ballot is read into the OVO system to be cast and dropped into the ballot box.

These troubleshooting instructions are for personnel designated to assist election officials at the Election Day Voting Precinct.

2.1.3 Maintenance Menu on the OVO and OVI during Voting

To access Maintenance functions on the OVO during voting, go to the Admin menu by touching the hidden Admin button on the main Voting screen and enter the Election password.

On the OVO Admin menu touch the Maintenance button and enter your Maintenance password.

To access Maintenance functions on the OVI during voting, go to the Admin menu by entering the Election Password and touching the hidden Admin button on the Precinct ID entry screen.

On the OVI Admin menu touch the Maintenance button and enter your Maintenance password.

The Maintenance Menus on the OVO and OVI are illustrated below:

2.1.4 Force Shut Down

If voting is open and there is a hardware failure, or if there has been a fatal error, you may need to force a shut down. Simply depress the power switch on the OVO or OVI case exterior to turn off the system.

Note: Always wait at least twelve seconds before restarting the OVO or OVI.

When the unit is restarted, it will check to see if voting was open and attempt to recover to the last known state.

2.2 Startup Issues

2.2.1 OVO or OVI Won’t Start

Areas to have the pollworker check. • Switch the OVO/OVI off.

o Check the power cable is plugged into the Unit and into the UPS outlet. o Check the UPS unit

Is plugged into the wall outlet Is powered on (LED indicates on in the front of the unit).

o Test the wall outlet to be sure it is working.

• Switch the Unit on again.

• Check if power indicator lights of the Unit is on o Check if the OVI printer LED light is powered on (observe through holes

in the front of OVI unit)

o Check the OVO to determine if LEDs are on: One or more Printer LED lights on (area indicated by yellow

arrow below). The Ballot Reader Indicator is on (area indicated by green arrow

below).

Run through the same checks requested above. If unit still does not respond, replace the OVO or OVI system.

2.2.2 OVO or OVI Does NOT start in Voting Mode

The OVO and/or the OVI is being reported as not starting as normal on election day. One or both units are prompting for a Maintenance Password on starting. This is typically caused by the unit having been closed previously or the System Date and/or Election Information being wrong. There is nothing for the pollworker to do. A maintenance password is required to

proceed further. A troubleshooter should be dispatched to the precinct.

2.2.2.1 OVO Starts at Enter Maintenance Password

Enter your maintenance password. • Select Machine Info on the OVO maintenance menu and Print a report.

o Verify the Election Information printed on the Machine Information Report.

This would include the Title of the Election, Date of the Election, Election ID, and Election Version. If the machine information is wrong, the election loaded on the OVO is incorrect.

o If the Machine Info screen indicates Voting Status is “Not Open”, this means the system has determined the OVO system Date or the Election Date is wrong. To Start Voting, you will need to override the system and start voting.

1) On the Maintenance Screen, Select “Sessions”

2) Touch Override. Touch the Continue button.

3) Enter the Election Password.

4) The system will then prompt the operator is prompted to insert ballot header cards for each precinct into the ballot reader slot. This can be handled by the pollworkers.

o If the Machine Info screen indicates Voting Status is “CLOSED”. The current OVO has been closed.

1) Turn the OVO off.

2) Remove the TM from the current OVO and give the TM and the Machine Info Report to the Inspector to store in the Precinct Results Envelope with a note about the machine being closed.

3) Replace the OVO system with a replacement OVO and a clean TM.

2.2.2.2 OVI Starts at Enter Maintenance Password

Enter your maintenance password. • Select Machine Info on the OVI maintenance menu and Print a report.

o Verify the Election Information printed on the Machine Information Report.

This would include the Title of the Election, Date of the Election, Election

ID, and Election Version. If the machine information is wrong, the election loaded on the OVI is incorrect.

1) Replace the OVI unit with a new unit.

o If the Machine Info screen indicates Voting Status is “NEW”, this means the system has determined the OVI system Date or the Election Date is wrong. To Start Voting, you will need to override the system and start voting.

1) On the Maintenance Screen, Select “Sessions”

2) Touch Override. Touch the Continue button for the OVO (Yes or No for the OVI).

3) Confirm Open Override session by touching YES for the OVI.

4) The system will print an open report and proceed to the Election Password prompt to start an individual voting session.

o If the Machine Info screen indicates Voting Status is “CLOSED”. The

current OVI has been closed.

2.2.3 Display Does Not Come On

2.2.3.1 OVO Touch Screen Monitor Not On

Make sure the Ballot Reader Indicator is lit (area indicated by green arrow below) and one or more Printer LED lights is lit (area indicated by yellow arrow below).

If monitor will not start, replace the OVO system.

2.2.3.2 OVI Touch Screen Monitor Not On

Make sure OVI system has power. Check if printer lights are on and visible through holes in the front side of the unit.

If monitor will not turn on, replace the OVI system.

2.2.4 Printer Error

Screen reads “Error Code 244, Printer Error”. Make sure the paper roll is installed correctly and the lid is closed securely. (Refer to the Election Day Pollworkers Guide).

Replace the OVO or OVI system that has the Printer Error.

2.3 Close Voting Issues

2.3.1 Cannot Close, Enforced Close Time

If Close voting time is being enforced and the system time shown on the Admin menu’s Machine Info screen is wrong, or if there is a need to force Close voting, contact the Election office for assistance.

Enter the maintenance menu and touch the “Change Close Time” button.

An onscreen keypad appears allowing you to enter a new time in 12-hour format and select the AM or PM box. When you enter a new time, the Save button is activated. Touch Save to change the time.

Select Back to exit to the Maintenance Menu, select Back to exit to the Admin Menu. The pollworker may now select Close Voting.

2.3.2 Election Summary Won’t Print on OVO

If voting has closed, touch Admin and use View Summary and Print to print the report. If there seems to be another problem, force a shut down (section 2.1.4), restart the OVO, and print the Summary via the Admin Screen.

Replace the OVO and recover using the original TM. Print the Summary from the Admin or Maintenance menu.

2.4 Ballot Reader Issues The Ballot Reader (scanner) hardware is contained inside the OVO system.

2.4.1 “Ballot Jammed - Remove ballot at Input”

A ballot is being processed inside the unit and the voter or poll worker is trying to insert another ballot. Remove the ballot being inserted and wait for the first ballot to be processed and for the green Ballot Reader Indicator status before re-inserting the next ballot.

2.4.2 Reader Status: “Ballot Jammed”

A ballot is stuck in the Reader. Stop voters from inserting more ballots. If the ballot is not ejected automatically and part of the ballot is visible, gently pull the ballot back out of the slot. Be sure to wait for the green Ballot Reader Indicator status before re-inserting a ballot. It may be necessary to force a shut down and reboot the OVO (refer to section 2.1.4).

Remove the seal (yellow arrow), unlock the back top access area of ballot box (red arrow) and remove the hood cover off the top of the unit (green arrow).

You should be able to access and remove the ballot.

Replace the hood, making sure the flap falls unobstructed into the top of the ballot box.

Lock the hood back into place and reseal the access area.

2.4.3 Election Password Required

After a ballot is inserted, it may be returned to the voter and the Election Password screen presented and the election password may be required to continue operations at the OVO. When this condition occurs, after entry of the Election password, a “Ballot jammed after cast. Special handling required” screen will be displayed:

1) The Ballot returned has been cast. DO NOT reinsert the ballot.

2) Place the ballot that was rejected into a Special Handling envelope (example illustrated to the right) and fill out the information requested on the front of the envelope (Time, Public Count, etc).

3) Include the envelope (with ballot inside) with the

materials returned to headquarters. 4) Press Back to continue with the next voter.

The Admin Log of the OVO will reflect the following information:

2.4.4 Reader Status: “No Reader”

Force a Shut Down of the OVO (power the OVO off, wait 12 seconds, and then power it back on. Voters may continue to vote using paper ballots and return their ballots to a poll official while the OVO is down.

The “No Reader” message indicates a problem with the Ballot Reader or with its connection to the PC component inside the OVO case. If the Ballot Reader Indicator (on the input bezel) is lit, power down the OVO, at least twelve seconds, and power the OVO up. If the message still appears, replace the OVO.

2.4.5 A Ballot Ejects on Restart

If voting was in progress, the OVO was restarted following a problem, and a ballot was ejected from the Ballot Reader slot on startup, it is not certain that the ballot was recorded.

The Inspector holds the ballot in question to deliver to Election Headquarters.

2.5 Audio and Keypad Issue The audio, Sip and Puff input, and keypad hardware on the OVI is part of the OVI system.

2.5.1 Voter Cannot Hear Audio on OVI

Voters can adjust the volume during ballot entry using the Keypad Volume keys:

Volume Increase: The reddish oval (with a +) key at the top left side of the keypad allows the voter to increase the volume of the audio.

Volume Decrease: The black oval (with a -) key at the top left side of the keypad (below the Volume increase key) allows the voter to decrease the volume of the audio.

Check if the Headphones are plugged into the jack on the left side of the OVI Keypad. Test adjusting the volume. Select Admin, select Diagnostics, and use Test Audio to determine if audio is coming through. (Refer to the Election Day Pollworkers Guide for instructions on utilizing the OVI diagnostics.)

Test replacement headphones with the Test Audio diagnostic. Otherwise, replace the OVI system.

2.5.2 Keypad Not Responding

Check the keypad and cable connection. Press Admin, and Diagnostics. Try the Test Keypad diagnostic test. (Refer to the Election Day Pollworkers Guide for instructions on utilizing the OVI diagnostics.)

Replace the OVI system.

2.5.3 OVI does not respond to entry via the Sip and Puff device

Check if the Sip & Puff device is plugged into the jack on the right side of the OVI Keypad (labeled Binary Input). Test the unit. Select Admin, select Diagnostics, and use the Test Keys (circled in red in the diagram on the right) on the Sip & Puff device and the Test Binary Input Test to determine the OVI is recognizing input from the device. (Refer to the Election Day Pollworkers Guide for instructions on utilizing the OVI diagnostics.) If the Sip and Puff device is not working, replace the Sip and Puff device.

If the OVI is no recognizing input from the original Sip and Puff device or the replacement device, replace the OVI system.

2.6 Printer Issues Both the OVO and OVI have a printer. The printer on the OVO is used to print reports and uses a smaller width and diameter paper roll. The printer on the OVI is used to print voter ballots and uses a larger width and diameter paper roll.

2.6.1 Printer Paper Low

The Election Day Pollworkers Guide includes instructions for replacing printer paper. The steps are similar for replacing either the OVO or the OVI paper. The main difference is to access the OVI printer, the OVI is first powered off, the OVI case is unlocked using the OVI access key and the seal broken.

1. Press the printer’s cover release button at the front of the printer.

2. Lift up the printer lid.

3. Remove the paper spool (it will have some paper remaining).

4. Set the new paper roll into the hollow, with the leading edge coming toward the front from under the bottom of the roll.

5. Push down both sides of the printer cover to close. Make sure that the printer cover is securely closed.

For the OVI, relock the Case, replace the seal on the unit, and power the OVI back on.

The OVI will automatically print a Machine Information report when it is powered back on. For the OVO, the pollworker should run the Test Printer diagnostic from the Administrative Menu (Diagnostics function) to make sure the paper is installed correctly. (Refer to the Election Day Pollworkers Guide for instructions on utilizing the OVO Administrator diagnostics.)

2.6.2 Non-Functional Printer

Replace the OVO or OVI if a new Printer is needed.

2.6.3 Printer Jammed

The Operator at the precinct has instructions for clearing jammed paper. For the OVI the seal needs to be broken to unlock the top. For the OVO, the printer is readily accessible.

Open the printer cover and gently remove the jammed paper. Lift out and re-insert the paper roll if necessary. Follow instructions in “Printer Paper Low” on the preceding page for removing and inserting the paper roll.

2.6.4 Reprinting Reports and Ballot Slips

The OVO system always attempts to reprint a report when a printing problem (paper out or jammed) has been fixed.

2.6.5 Print a Ballot on the OVI

The OVI prints a Ballot. If the Ballot fails to print or a 244 error occurs (see Error Code Table), the Operator can power the unit off and clear a jam or insert a new paper roll.

Upon recovery, a new voter session will have to be run if their ballot was not originally printed correctly.

2.6.6 Printing an Election Summary on the OVO

The OVO prints an Election Summary. If the Election Summary fails to print or a 244 error occurs (see Error Code Table), the Operator can attempt to print the report by selecting Admin, View Summary and Print. (Refer to section 2.3.2)

NOTE: Voting must be closed before the Election Summary will print.

2.6.7 Printing an Close Report Summary on the OVI

The OVI prints a Close Report. If the Close Report fails to print or a 244 error occurs (see Error Code Table), the Operator can attempt to print the report by selecting Machine Info from the Admin or Maintenance menu.

If the OVI is “Closed”, the only difference between the Machine Info report and the Close Report is the Machine Info Report does not have the four signature lines. The Machine Information report can be used and the pollworkers can sign the report.

2.7 Replace the OVO Transport Media

If the Transport Media (TM) is unusable or becomes unusable (error code 903 or 906), replace the unit. The following steps take place at the precinct:

1. Switch off the OVO using the case power switch to force a shut down.

2. Open the top access panel of the OVO case under the touch screen monitor. To access the TM compartment, you will need to depress the button on the display assembly (circled below in red). Rotate the display up as shown above. Unlock the compartment.

3. Remove the faulty TM.

4. If voting has begun and the TM contains voting data, give the TM to the Inspector to store in the Precinct Results Envelope with a note about the failure.

5. Make sure the cleared, replacement TM has a label associating it with the precinct OVO system, and then insert it into the TM connector.

6. Close and lock the top access panel and restore the touch screen monitor to the operating position.

7. Switch on the OVO. The system automatically recovers to where voting left off from the data on the hard drive.

2.9 Replace the OVO or OVI System

For expediency, the default solution for system failure or unexpected behavior other than Transport Media failure is to install a replacement system. The faulty OVO or OVI system should be returned to the warehouse to be examined.

2.9.1 Replacing the OVO System

Follow these steps:

1. Stop voters from inserting ballots into the OVO. Voters may continue to vote on paper ballots and hand ballots to an official. Any ballot rejected back to the voter has not been recorded and may be handed to an official. Any ballot accepted by the system was recorded.

2. Force a shut down by switching off the failed OVO.

3. Unplug the OVO case power cable.

4. Unlock and open the top access panel of the failed OVO, under the touch screen monitor, and remove the TM.

To access the TM compartment, you will need to depress the button on the display assembly (circled below in red). Rotate the display up as shown above. Unlock the compartment.

5. Close and lock the top access panel, and restore the touch screen monitor.

9. With a poll official’s help, remove the failed OVO case from the ballot box. Do not disturb the ballots in the ballot box.

10. With a poll official’s help, set and seal the replacement OVO unit on the ballot box.

11. Unlock and open the replacement OVO top access panel, under the touch screen monitor (refer to step 6), and insert the TM from the failed OVO into the replacement OVO TM connector. Close and lock the top access panel.

12. Connect the power cable.

13. Switch on the OVO using the exterior case switch. The system automatically recovers to where voting left off, using the data stored on the TM.

2.9.2 Replacing the OVI System

Follow these steps:

1. Stop voters from continuing with ballot entry on the OVI. If their ballot has already been printed it can be cast on the OVO system.

2. Force a shut down by switching off the failed OVI.

3. Unplug the OVI case power cable.

4. Unlock and open the cover of the failed OVI and remove the TM.

5. With a poll official’s help, remove the failed OVI from its location.

6. Place new OVI in location.

7. Unlock and open the replacement OVI cover and insert the TM from the failed OVI into the replacement OVI TM connector. Close and lock the cover, and attached seals to the OVI.

8. Connect the power cable.

9. Switch on the OVI using the exterior case switch.

10. If a voter was entering a ballot, start their ballot process over.

2.10 System Failure

2.10.1 UPS Sounding Audible Alarm Once Every 5 seconds

When a blackout, brownout, or an overvoltage condition occurs, the UPS will transfer to the battery mode, the On Battery Indicator will illuminate and an audible alarm will sound once every five seconds indicating that the commercial power is lost or unacceptable.

When the commercial power returns or is at an acceptable level, the UPS will automatically transfer back to the AC normal mode. The Pollworker will not have to do anything.

The OVO and OVI will function on Battery Backup for 2 hours.

2.10.2 UPS Sounding Audible Alarm Twice Every 5 seconds

During an extended outage when there is approximately two minutes of backup time remaining the audible alarm will sound twice every five seconds. This Low Battery Warning is letting the pollworker know that they should turn off the OVO and OVI units.

The Pollworker should

1. Tell voters to stop inserting ballots in the Ballot Reader slot.

2. Voters may continue to vote on the paper ballots and turn in their ballots to an official.

3. Push the OVO and OVI case switch to the OFF position.

4. When power is restored, the UPS will automatically restart.

5. Press the case power switch to ON. The system recovers, and voters may begin inserting ballots into the OVO and utilizing the OVI for voting.

2.10.3 Handling Power Failures

If the power goes down during voting, the system can recover on restart when power returns.

The precinct pollworker may handle this situation:

1. Tell voters to stop inserting ballots in the Ballot Reader slot. An accepted ballot was counted; a rejected ballot was not counted. Voters may continue to vote on the paper ballots and turn in their ballots to an official.

2. Push the OVO and OVI case switch to the OFF position.

3. When power is restored, press the case power switch to ON. The system recovers, and voters may begin inserting ballots into the OVO and utilizing the OVI for voting.

2.10.4 Handling a Failed Recovery

In the unlikely event that a system fails to recover, you can assume the data has been corrupted or there are other issues. Replace both the failed unit (OVO or OVI) and TM. Notify Election Headquarters that steps will need to be taken to recover voting data from the faulty unit.

Appendix A – Error Code Table

The errors in the following table could appear during startup or during voting. An error code is either displayed on screen or can be found in the Admin Log. These are not all possible errors. For a complete list of system codes, see Appendix A in the Warehouse Technician’s Guide. Code Description Resolution

111 Invalid Ballot: The ballot is ejected. The ballot style was not found in election data.

Verify that the ballot is valid for this election and has correct markings. Supply correct ballot, if necessary.

206 On Startup: No server was connected on startup. The Diagnostics screen shows an error.

Verify the OVO has valid firmware installed. Check connections and PC hardware. Force a shut down and replace the OVO.

212 Help not found for… Continue using the OVO. Contact Unisyn for updated Help files.

220 On Startup: Displays the Maintenance Password prompt instead of normal open voting prompts. Voting cannot be opened because it is not Election Day.

The current system date is not recognized as Election Day. If the problem is not spread throughout the jurisdiction, log in with Maintenance password and check Machine Info to verify the election version. If the election version is correct, and if no votes are on the system (Zero Count can be verified), select Sessions and Override to start voting at the polls. The Election password will be required.

222 On Startup: Displays the Maintenance Password prompt instead of normal open voting prompts. Voting cannot be opened because it is before Election Day.

Follow the same steps to resolve 220. Login as Maintenance user, verify election version and Zero Count, and use Override mode if the error is due to a faulty system date.

267 Close voting process was incomplete.

Force a shut down. Restart the system and try to press Close again. If the error persists, replace the OVO or OVI reusing the TM. When the replacement restarts and recovers from the original TM, then close voting.

284 Invalid request. An internal error occurred as the tally file was being created.

Replace the OVO and recover from the original TM. Depending on the recovery state, Close voting or request to View Summary from the Admin menu.

289 Tally failed. View Summary function failed to display the tally of votes.

Try View Summary again from Admin or Maintenance Menu. The system may not have been ready. Follow steps for 284 resolution.

350 Password not found. The operator was unable to enter the required Election password.

Verify that the operator has the correct password. If not, try login with correct password. If the password is not correct, replace the system.

367 Invalid Ballot Style or Invalid Ballot Card or No Contests. The Reader was unable to read the ballot.

Try to read the ballot again, and if it still does not read, spoil the ballot. Provide the voter with a new, valid ballot card and instruct the voter to vote again. If all inserted cards return the “No Contests” message, the loaded election has no contests. Replace the OVO.

368 Invalid Ballot. Ballot was rejected. Attempt to insert the ballot into the OVO again and is accepted. If the invalid ballot message is still received, spoil the ballot, if voted. Provide the voter with a new, valid ballot and instruct the voter to vote again. If absentee or provisional ballot, insert into secondary slot on ballot box.

378 Ballot jammed after ballot data was written to the vote file.

The already ‘cast’ ballot may be ejected back to the front of the OVO. Enter the Election password to clear the error. The error screen message says the ballot may have been cast and it should not be inserted back into the OVO, it should be handled manually. Voting can continue as normal.

380 Add precinct failure. Failure to add precinct after inserting a new ballot header card.

Verify that the ballot was valid but failed to initialize the precinct. Verify the ballot is for this election and the precinct ID is valid. Replace the OVO.

384 Precinct not found. The ballot style was not found for a given precinct.

Verify that the ballot is valid for this election. Spoil the ballot, if voted.

901 Failure when writing file to Hard Disk.

Replace the system and recover from the original TM.
