In late May we conducted a DC member-wide survey regarding the association’s quarterly newsletter, The Virginia Voice.

After three distributions of the electronic survey, we ended up with a healthy 14% response rate -- and a number of member respondents took the time to give us some real feedback and suggestions.

Format Preferences

Over 81% of the respondents voiced a preference for an online version of the newsletter. 34% of these would be satisfi ed with an online pdf of the traditional format (as you are reading here), while the others would prefer it be specifi cally formatted for mobile devices and the web. About 10.5% would like to receive it in both online and mailed formats. Less than 6% voiced a preference for printed and mailed only. Printing of the newsletter costs $2400-$2900 per issue -- $9600-$11,600 per year -- and paper and ink costs continue to rise. prominence in the newsletter. Effective with this issue, we have reduced the

point size of the classifi eds and moved them to the back of the issue. We will watch the value of articles contributed by supporting suppliers to ensure value and relevance. Most members who provided comments about the publication and its continuing improvement were positive. One member expressed that he/she felt it is critical that we maintain a print copy. Other ideas including increasing the number of articles designed for the offi ce staff, adding sample offi ce forms, and including regular local, state and national legislative summaries. patients. I have a staff member who is afraid to get any adjustment. She thinks I may break her neck. But I can’t fi re her because of that? (Dr. Andrew)

This state is an “at will” state. At-will employment is a term used in U.S. labor law for contractual relationships in which an employee can be dismissed by an employer for any reason; that is, without having to establish “just cause” for termination, and without warning. So technically, you can fi re someone for having bad breath. In regards to unemployment benefi ts, that may be another thing. That being said, I would let her go as soon as possible. She is like a bad apple in a barrel. I mean what if a patient asks her, “Do you get adjusted?” and she says, “Oh, no, I am afraid he will break my neck”….seriously? (Dr. Cynthia)

She will break your practice! Find a way to either educate her regarding the safety of chiropractic or fi nd a way to let her go. If she is not getting adjusted there is no way she can help grow your offi ce -- which is a primary role of CAs. (Dr. Mike)

care, massage and acupuncture?”Actually, we ask these questions in

phone interviews and if the response is negative, as in, “Oh, no, I’d never let someone do that to me,” the candidate would not be granted an in-person interview. I don’t mind if the candidate has never been a chiropractic patient, but her attitude and willingness to learn more about it can be a deal breaker. If she has never had chiropractic care, I would say something like “our employees receive complimentary care here as part of their benefits and we encourage staff members to become patients, not only to improve your health, but also because it is easier to answer patient questions if you’ve had experience.” (Dr. Sara)

As per a conversation I had with a VEC agent several years ago, an employer has 30 “working days” to determine if an employee is working out. Before that, if the person is fired then he/she cannot collect unemployment. After 30 days, he can collect unemployment, even if his employment was part-time. A “working day” was defined to me as “the employee shows up to work.” Even if they clock in and then clock out 1 minute later, it is considered a working day. And it’s 30 working days, NOT 30 calendar days. I hope that is some useful information for people. (Dr. Mark)

I have probably fired more staff than most of you have hired. Am I a jerk? Debatable. ;-) Actually, I have high standards and the funny thing is as I move lesser staff out of my office, I make room for someone better. If we keep poor staff then we will never have good staff because the poor staff is taking up that space. But the point I really wanted to make is that unemployment is NOT expensive. (Dr. Chris)

Most importantly, a poor staff member will ruin your good ones. THAT is expensive!!! (Dr. Bill)

Funny, if it were not true. I hired a lady a couple of years ago and, as with all my staff, part of her training was going through the new patient process as the patient so she can “walk a mile” in the patient’s shoes. So I go through the exam, etc. and have a

ROF and then say, “Alright, let’s go get you adjusted.” Granted, she had been smiles and head nodding the whole time as we went through the ROF and what an adjustment entails (and I use Activator exclusively!). So she looks at me and says, “Seriously?” I looked at her, just as confused at that point, and said, “Yes. You seemed to understand what we found and that there was some room for improvement. Is there something that you do not understand?” She replied, “Well, I had a really bad experience with a chiropractor a few years ago and I was in terrible pain for weeks and bedridden…” I empathized and stated that that is a rare situation and unfortunate, but that I would not be twisting her neck (not that I believe that is bad to do; I was empathizing in the moment) but to no avail. There was no way she was going to be adjusted and I was certainly not going to “force” anyone to be. I simply shook her hand and told her that I respect her choice but that I could not have anyone in work in the office that could not genuinely encourage patients to also seek care, particularly when they are not feeling well. I liken it to a waiter at a restaurant never eating at the restaurant. How can they respond to questions from the people they are serving if they do not believe in the product themselves?Firing staff is easy, not firing them when you should is painful. They are being PAID to perform a service. You owe them nothing. They are there to perform a service and will be compensated for their work. That said, whatever is working for you is your call, if you are happy with the results. (Dr. Chris)

Thank you for sharing your story, Dr. Chris. I primarily dislike staff who lies and is lazy. This lady was honest and a scared cat. She works hard. I am getting patients through other sources. I felt offended when she said that, but I feel that it is her who is unlucky not getting help from me. My other staff members always love to get treatment from me. (Dr. Andrew)

You are signing the paycheck, Doctor, so if you are happy then that is all the truly matters. Your compassion is certainly not a bad quality to have. (Dr. Chris)

Statins function to inhibit the enzyme HMG-CoA reductase. This is the

rate-limiting enzyme that converts HMG-CoA into precursors of cholesterol. Thus, by inhibiting HMG-CoA reductase, the synthesis of cholesterol is inhibited, which is supposed to save us from dying of heart disease. But if you look at the CDC’s website about heart disease, there is no mention that there has been a substantial reduction in heart disease and deaths from heart attacks due to the use of statins. Each year since 1999, about 610,000 Americans have died from heart disease 1. This begs the question: is there any real benefi t from taking statins?

JUPITER Trial Misrepresentation

The JUPITER trial2 was publicized in the media as providing conclusive evidence that statins prevent heart disease. The study was lauded so much that we were subjected to the notion that we should all be taking statins for preventive purposes. A look at the study suggests that such a claim is without merit. A total of 8901 men and women took a placebo while another 8901 subjects took 20 mgs of rosuvastatin per day. The trial was stopped at a median follow-up time of 1.9 years and a maximum of 5 years. The primary end point was the combination of nonfatal myocardial infarction, nonfatal stroke, arterial revascularization, hospitalization for unstable angina, or confi rmed death from cardiovascular disease. Of the 8901 subjects in each group, there were 142 primary endpoint events in the statin group and 251 in the placebo group. In other words, 1.6% of the statin subjects and 2.8% in the placebo subjects had an event – this is known as absolute risk. This irrelevant 1.2% difference between the two groups supports the reason why deaths from heart attacks has not changed since at least 1999.

Side Effects of Statins

Most patients taking statins will develop symptoms of some kind, such as muscle pain, which can be of varying degrees. Patients are led to believe that the side effects are worth the “great” protection they are receiving from statins. However, from the above we can see that there is no signifi cant cardiovascular protection and the side effects can be much greater than just a little muscle pain. Additional potential side effects of statins include fatigue, erectile dysfunction, memory loss, cognitive defects, ALS-like symptoms,

aggression, irritability, polyneuropathy, peripheral neuropathy, muscle cramps and muscle weakness3. Researchers are now suggesting that statins may actually cause atherosclerosis and heart disease. It is time to seriously consider the benefi ts of statin therapy?

Statins as a Cause of Heart Disease?

Those of us who are suspicious of statins and other medications that disrupt normal metabolism would not be surprised to discover that statins may be promoters of heart disease. But can you imagine the anger many will have when they realize that they have suffered unnecessary side effects while taking a medication that may actually cause the disease they were trying to treat/prevent? Unless you have a library subscription that includes Expert Reviews in Clinical Pharmacology, you will only have access to the abstract, which contains enough information to make one think4: “In contrast to the current belief that cholesterol reduction with statins decreases atherosclerosis, we present a perspective that statins may be causative in coronary artery calcifi cation and can function as mitochondrial toxins that impair muscle function in the heart and blood vessels through the depletion of coenzyme Q and ‘heme A’, and thereby ATP generation… Thus, the epidemic of heart failure and atherosclerosis that plagues the modern world may paradoxically

be aggravated by the pervasive use of statin drugs. We propose that current statin treatment guidelines be critically reevaluated.” The authors also explain that statins inhibit the synthesis of vitamin K2, which is known to protect against arterial calcifi cation.

What Should We Do?

It is not likely that patients will be taken off statins after the publication of this one article. However, there is a greater likelihood of this happening if patients normalize their blood Continued on page 8

glucose, triglycerides, and LDL and HDL cholesterol by avoiding sugar,

fl our, and trans fats. In the meantime, it seems reasonable for statin takers to also take coenzyme Q10 and vitamin K2.

when I hear and read the ubiquitous chiropractic ads for symptom-relief. Even if treatment of “back pain, neck pain and headaches” was all we had to offer, would we be the best at it? In reality, we place a meager 4th behind our old friends needles, drugs and knives. Is this all we aspire toward? Is this what we envisioned when we fi nished school? Is this all we offer to a public whose health continues to decline at an alarming rate? Are we subscribing

to a truly Chiropractic paradigm or a medical model? When we do alleviate pain are we engaging our patients in a process that will lead to better health or simply providing a temporary escape-hatch that recycles them into the same minimalist mindset that brought them to us initially? Do we as a profession have the will, determination, strength and character to communicate in a clear, consistent and shameless way the true potential that chiropractic offers so that instead of using it as a 4th choice for pain, they view it as a 1st choice for better health; a way to minimize the likelihood of crisis and maximize their quality of life? The questions we need to ask ourselves are these; Is Chiropractic an off-ramp or a road to a better life? a time-out or a chance to take control? a band-aid or a means of empowerment? a knee-patch or a remake? a fi nger in the dyke or a new functional repair? And as DCs; are we pacifi ers or game-changers? tag-a-longs or leaders? whisperers or loudspeakers? fi re-crackers or bombs? I realize that it’s a hard corner to turn. Offering more than pain-relief requires an

uncompromising faith in what my father called “the healing power of Chiropractic.” The good news is this: if we can, as individuals and as a profession, garner the strength to be true to our mission, we have the ability to offer something spectacular; a service that is absolutely unique. One that is unequaled in its ability to impact the world; an opportunity to throw off the shackles of hamster-wheel care and step through a doorway that opens up into a whole new realm of possibilities. This happens by making the philosophy of Chiropractic the cornerstone of your practice and the adjustment part of the education instead of allowing the big picture to be relegated to an ”oh yeah, by the way” part of the adjustment. By doing this we take our best shot at helping our patients in a way and to a degree that no one else has. We become Chiropractic distance-runners rather than sprinters by designing care around incremental pro-active changes over time that enable people to incorporate chiropractic into their long-term game plan instead of using it as an episode-based “treatment” or ”non-medical intervention.” We become winners rather than also-rans. Is it worth the effort and the risk? Can we afford not to? The future of our profession and its impact on the world of health-care is literally in your hands. It’s time to shift gears.

Dr. Vincent Joseph Hosts Legislative Open Housein Newport News

Dr. Vincent Joseph of Rebound Chiropractic in Newsport News and his wife hosted a successful open house in July for Virginia Delegate David Yancy. Dr. and Mrs. Joseph recognized the importance of having several hours of actual “face time” with their legislator. The casual atmosphere provided an opportunity for Dr. Joseph and other attending chiropractors to bring to light some statewide issues with respect to Chiropractic. Yancy’s aid is a strong Chiropractic supporter, so he knows the challenges this profession faces. In addition, the Josephs raised money and recruited poll workers for the campaign. They were able to invite business leaders in their community who donated most of the money. Because of their generosity, neighbors, chiropractors and others could attend at no charge. Dr. Joseph noted, “The afternoon event was much easier to organize than we had initially thought. The delegate’s staff provided incredible support in the preparation of the open house. All we had to do was provide a list of invitees, the venue and food. We are hoping the ease and success of our event will encourage other Chiropractors to become more involved in supporting the candidates who make a difference in our state.”

shared information regarding the imaging center. There was also plenty of socializing and catching up, along with the great food!

(ECA-IPN) Continues Advancing in Virginia,

Maryland and Pennsylvania

Over the last several months, the ECA-IPN

has made signifi cant strides in the organization’s strategy development and marketing. Eric Osterberg, DC has been hired as the ECA-IPN’s Director of Marketing to implement these strategies as it jumps head-fi rst into marketing the IPN to Virginia’s, Maryland’s and Pennsylvania’s health insurers. Dr. Osterberg provides the IPN with a valuable combination of insights derived from his success as a practicing DC in Pennsylvania, an effective board member of the PCA, and in-depth understanding of the current healthcare marketplace. According to Tom Wetzen, DC, the IPN’s Vice President and Unifi ed VCA’s President, “The fi rst phase of marketing our ECA-IPN is to raise awareness of our network and the value that we are offering to payers. In this stage, we are reaching out to all of the health insurers, PPOs, HMOs, TPAs and large employer groups in our three states. We are sharing information, sending letters, making phone calls, scheduling meetings and answering questions.” Dr. Wetzen asks all Virginia DCs for the following assistance:
