SeaGen Secure Program Enrollment is Simple

• Gross family household income for the previous 12 months• PAP enrollment begins on a temporary basis and all shipments are

sent for future dates of service

Complete an enrollment form prior to therapy to assess eligibility

Email: casemanager@seagensecure.com or fax an enrollment form to the SeaGen Secure team

Provide copies of the patient’s income and residency information

Receive a confirmation letter (via fax or email) and phone call from the SeaGen Secure team

Assistance is Also Available for Underinsured Patients

• Income and residency documentation should be submitted along with the enrollment form

• You will receive a confirmation letter and call from SeaGen Secure

Complete an Enrollment Form Prior to Therapy To Determine Eligibility for the Drug Replacement Program (DRP)

• If there is a claim denial, the appeal must be completed within the payer’s appeals window (as stated on the Explanation of Benefits)

SeaGen Secure Must Assist with One Level of Appeal

• Complete this step immediately after each claim denial• Email securely to casemanager@seagensecure.com • Drug is replaced per date of service (DOS) for eligible patients
