To help the community better understand what is required to obtain a Certificate of In- dian Blood and what duties Vital Statistics performs; following information has been provided. The Vital Statistics department conducts an ongoing interoffice assessment of commu- nity files to ensure all documentation is up to date. This includes Birth Certificates, So- cial Security numbers , Acknowledgment of Paternity, Marriage Certificate, Marriage License, Death Certificate, Wills and all other vital documents requested or required to be on file per tribal authorities. This is sometimes accomplished by an annual file up- date hosted by Vital Statistics. However, it is ultimately your responsibility, as a commu- nity member, to ensure your file is up to date. My staff and I encourage you to come into our office and ensure your file is accurate. - The information collected by vital statistics is used in a variety of ways; some may impact your eligibility for certain benefits. Therefore it is critical to bring in your documents for verification for tribal benefits such as permits for wood, housing repairs, turkey Iham distributions and voting privileges. When information is received, my staff will maintain a hard copy file, in addition we will convert all information provided to an electronic format for preservation and easy trans- action through e-mail and other electronic means. Greetings Santa Clara Pueblo,

I would like to take this time to wish you all a very Merry Christmas and a Happy New Year.

To help the community better understand what is required to obtain a Certificate of In­dian Blood and what duties Vital Statistics performs; following information has been provided.

The Vital Statistics department conducts an ongoing interoffice assessment of commu­nity files to ensure all documentation is up to date. This includes Birth Certificates, So­cial Security numbers, Acknowledgment of Paternity, Marriage Certificate, Marriage License, Death Certificate, Wills and all other vital documents requested or required to be on file per tribal authorities. 1. Vital Statistics works with the U.S. Census Bureau to ensure Santa Clara Pueblo is accu­rately represented during the decennial census surveys and all other surveys the bu­reau may be conducting on Santa Clara Pueblo land,

2. Provide information to agencies requesting member verifications, for example the BIA Form 4432, Tribal Verification (used to determine job eligibility and eligibility for Na­tive American collage grants, etc.),

3. Conduct research on individuals requesting membership using ancestral decedent documentation and Help community members develop family trees.,

4. Provide tribal departments with Vital Statistic Information generated from the U.S. De­partment of the Interior Labor Force Report, submitted bi-annually by the Vital Statis­tics department.

5. Provide assistance with death certificates for individuals who pass away on Santa Clara Pueblo land.

6. Assist the Santa Clara Pueblo Social Service in researching Indian Child Welfare Act (ICWA) case

7. And enforce the Non-Member Residence Code.

When a request for a new or existing Certificate of Indian blood or any needed file is re­ceived, Vital Statistics works closely with the Governor to ensure the following process is implemented. The Community must make certain the below information is provided.

Process in Requesting a New or Existing Certificate of Blood:

• A written request may be submitted containing the following information: • .Full Name • Maiden Name • Date of Birth • Description of request • Date submitted • Phone Number • Current Address • Signature

For new Certificates of Indian Blood requests, the following documents will be requested at the time the request is submitted: • Birth Certificate • Social Security Card • Parents Marriage Certificate or Acknowledgment of Paternity (For membership deter­

mination) • Parent, who is not Santa Clara Pueblo but is Native American a Certificate of Indian • Blood, is needed to determine blood quantum.

For current Certificate of Indian Blood request, a written letter is needed specifying the above information. A copy will be given unless it is documented an original is being requested. At the time the letter is received the individuals requesting a current Certificate of Indian Blood will be asked to bring in any documents lacking from their file such as, Birth Certificate, Social Security, Parents Marriage Certificate or Acknowledgment of Paternity. Once all documentation is re­ceived, the request will go the Governor for approval. Please be aware this process may take up to a week depending on the Governor's schedule.

If you do not want to write your own request letter, the Vital Statistics Office has a pre-made form/letter for your convenience.

The following is a description of what my staff is responsible for:

Elaine Filbert, Data Entry Technician................................................................ ext. 1287 [email protected]

Elaine is a Data Entry Technician; she's responsible for all hard copy files. Elaine is an active member of the community and has been serving the community for 6 years 3 of which have been spent in Vital Statistics. Her duties include:

• Cataloging current tribal records in alphabetical order and ensuring all files are up­date with supporting documents.

• Updating individual records and family groups that date back to the 1936 base roll. • Updating census data and electronic upload into the vital statistics database • Conduct research for membership, and look up family ancestors.

Geraldine Gutierrez, Data Entry Clerk/Program Assistant................................. ext. 1342 [email protected]

Her responsibilities include working with all electronic files and assisting Elaine with hard copy files. She has been serving the community for 4 years I of which has been with)1ital Statistics.

She is honored and privileged to, work for our Pueblo of Santa Clara and our people. Her duties include:

• Assist Vital Statistics staff • Membership requests (Certificate of Indian Blood) • Assist Director with data information requests from departments, Tribal members and

other agencies. • Our staff assist Governor's office and Council with research and information. • Assist non-member Resident Committee • Membership verification from other Agencies and Santa Clara Pueblo departments. • Update of all members, blood affiliated non-member, and non-native files held in the

office of Vital Statistics. • Assist individuals at the direction of the Vital Statistics Director to fill out and submit a
