VA Voluntary Service, VA Central California Health Care System (VACCHCS), has the distinct honor and privilege of offering Veterans the opportunity to have their names engraved on the “Walk of Honor” Memorial located at 2615 E. Clinton Avenue, Fresno, CA 93703.

Veterans and their loved ones may submit applications for the “Walk of Honor” to pay tribute to the men and women from all branches of the United States military who have been Honorably Discharged, Killed in Action or Missing in Action.

WALK OF HONOR APPLICATION• Applications will not be accepted without required Veteran documentation. (DD214 or other forms of

verification indicating honorable discharge – see Walk of Honor FAQ for assistance.)• Multiple applications must be submitted at the same time if applicant wishes for names to be placed

next to each other on the Walk of Honor• VA Voluntary Service reserves the right to refuse to engrave any message or material determined

inappropriate, such as telephone numbers, political messages, or suggestive wording.• Each plaque may have three lines with up to 20 characters per line. (This includes spaces.)• The three-line message on the plaque will be centered in capital letters.• Only one letter or punctuation symbol per space is allowed.• Use one blank space between words.• Veterans who were Killed in Action (KIA) will be added at no cost, and the status will be indicated by a

gold star. (Documentation required)• All applications are due by August 15, 2019.• Inscriptions must be verified by the sponsor by August 31, 2019, or the application will be pulled.• Please make check or money order ($85 per block) payable to VA Voluntary Service.

Walk of Honor Frequently Asked Questions

What type of payment is accepted? All checks can be made out to VA Voluntary Service “Walk of Honor.” Below are the forms of payment accepted for the Walk of Honor.

• Cash

• Check

• Money Order

• Electronic Donation (receipt must be provided with application and documentation ofeligibility) at https://www.pay.gov/public/form/start/53164095/

What is the cost of the plaque? $85.00 USD

What documentation is needed to submit?

• Completed application

• Payment

• DD-214 or other verification of Honorable Discharge.

• Verification of Killed in Action (KIA), if applicable

Why do I have to submit my Verification Papers/DD-214? To verify honorable discharge and plaque inscription for accuracy i.e., Purple Heart, Silver Star, etc.

Who qualifies for a plaque? Any Veteran that is or was;

• Honorably discharged

• Killed in action

• Missing in action

Where can I obtain a copy of family members DD-214 or discharge papers? There are four options that you may pursue.

• If Veteran has received care at the VA Hospital in Fresno, you may visit “Release ofInformation” in the basement of the hospital, Room B1A01, for a copy of the documenton file.

• A Standard Form 180 (SF-180) may be submitted to the National Archives Office(available online at https://www.archives.gov/files/research/order/standard-form-180.pdf).

o All the information of who to mail or fax this document can be found within theSF-180 itself.

o The request may also be submitted online athttps://www.archives.gov/Veterans/military-service-records.

**Note: Submitting to the National Archives will take some time to receive the DD-214 or military records.

• You can reach out to Veteran organizations that your loved one may have been part ofi.e. American Legion, Veterans of Foreign Wars, etc. to see if they may have a copy of their DD-214 or military records on file for you to obtain.

• You may check with the county clerk to see if the Veteran filed a copy of his/her DD214 there. NOTE: Due to privacy laws, some states will not release the information if you are not eligible to receive it.

• If you are unable to obtain records using the listed methods, but know that the Veteran was treated at another VA facility, you may ask Voluntary Service to request a report for that Veteran by stopping by their office (Bldg. 1, Room 1A13) and filling out the necessary paperwork.

Where do I turn in my application? The Voluntary Service Office on the first floor, east wing, between the main lobby and the breezeway in Room 1A13, across from the Veteran Service Officers.

Is this for Veterans both deceased and alive? It is available to ALL Veterans who honorably served in the armed forces during peacetime, time of war, overseas, stateside, no matter whether they are alive or deceased.

When will the inscriptions be completed for viewing after submission of application? You will receive an invitation to the dedication and unveiling from the Voluntary Services office.

Can I submit my application without a DD-214 or discharge papers? No, a DD-214 or discharge must accompany the application, when submitted. Things may have been done differently in the past, but these are the guidelines that have been set forth from current VA Leadership.

Will the plaque inscription be in mixed case letters? No, the inscription will be in all caps.

I missed the deadline, when can I get an inscription added? You can have your name added during our next enrollment period. There is a wait list that you can be added to, see someone in room 1A13 to fill out an application and learn about time lines.
