INTRODUCTION

The Nye County Nuclear Waste Repository Project Office (NWRPO), with the assistance and cooperation of Yucca Mountain Site Characterization Office (YMSCO), will implement the Early Warning Drilling and Testing Program (EWDP) to collect data and develop a database for the saturated zone, down gradient of Yucca Mountain. The EWDP is part of Nye County's mission of independent verification testing and oversight drilling with the requirement that any activities at Yucca Mountain will be performed in a manner to protect the integrity of the site. Nye County and its contractors will conduct work under the Nye County Quality Assurance Program Plan (QAP-P) and its subordinate Work Plans for Drilling and Well Construction, Borehole Testing, Geophysical Logging, and Sample Management. The Nye County QAPP has been reviewed by the Nuclear Regulatory Commission (NRC) and found to conform to relevant criteria of Title 10 of the Code of Federal Regulations (10 CFR) Part 60, Subpart G and 10 CFR Part 50, Appendix B. The Office of Civilian Radioactive Waste Management (OCRWM) has not approved the Nye County QAPP as equivalent < to the Project at writing of this Field Work Package (FWP).

The U.S. Department of Energy (DOE)/OCRWM activities associated with the EWDP are to be performed in accordance with the Quality Assurance Requirements and Description (QARD) requirements related to the data collection, testing, and monitoring of boreholes, with the agreement of Nye County. Affected Management and Operating (M&O) organizations are responsible for conducting M&O field work in accordance with the Project Requirement Documents and this implementing FWP. It is the affected organization's responsibility to determine the QA program applicability for related activities in accordance with the OCRWM approved QA program. The Test Coordination Office (TCO), with agreement of Nye County, may initiate addendums to this FWP to provide additional detail regarding the M&O activities described and controlled according to AP-5.2Q.

This FWP, implements M&O activities associated with the Nye County EWDP Phase II and the Alluvium Testing Complex (ATC) activities budgeted in the DOE Yucca Mountain Site Characterization Project (YMP). All references to tasks performed by Nye County are Non-Q references and do not mandate the county to perform work to this FWP. The Nye County work plans should be used to confirm planned work activities. This FWP will be revised for major changes to Nye County work plans for the EWPD Phase II and ATC drilling activities that restrict the YMP activities to be performed.

All activities conducted, and data/samples collected by Nye County will be considered Non-Q. Activities conducted by personnel qualified under the YMP using approved YMP procedures will be considered Q. The DOE or M&O management will determine which Nye County work will be overseen by YMP personnel based on the needs of the Project. The Project Management, in the future, may decide to attempt to qualify Nye County data through an approved YMP qualification process, as they would any other data sets not originally collected as Q.

1.0 SCOPE AND DESCRIPTION

1.1 GENERAL SCOPE DESCRIPTION

This FWP defines the process controls utilized by the TCO to manage and coordinate the DOE/OCRWM activities, associated with the Nye County EWDP Phase II and ATC.

The following Site Characterization Plan (SCP) and Process Model Reports (PMR) document the YMP data needs for Site Recommendation (SR) and License Application (LA) associated with this FWP:

SCP 8.3.1.2.2.7 Aqueous-phase chemical investigations. 8.3.1.2.3.1 Characterization of the site saturated-zone ground water

flow system. 8.3.1.2.3.2 Characterization of the Saturated Zone Hydrochemistry. 8.3.1.3.2.1 Mineralogy, petrology, and chemistry of transport pathways. 8.3.1.4.2.1 Characterization of the vertical and lateral distribution of

stratigraphic units within the site area.

PMR S9000 Saturated Zone Flow and Transport Model

1.1.1 Planned Test Activity, Location, and Site Improvement Description

The Nye County EWDP plan includes the drilling of approximately 23 new boreholes down from the hydrologic gradient of Yucca Mountain. The boreholes are divided into categories of shallow boreholes drilled to a total depth less than 305 meters (1000 feet), and the deep boreholes equal or greater than 305 meters (1000 feet), or as designated by Nye County. Due to drilling methods (e.g., drilling with polymer mud) for the deep boreholes, Nye County may drill piezometer borehole adjacent to the deep borehole using air for the drilling medium. The piezometer drill hole will fulfill data collection in the unsaturated zone, in an acceptable drilling environment, which correlates to the upper portion of the deep borehole. This new drilling and testing program will be a phased program of planned drilling, testing, well completion, longterm monitoring, and data collection taking place over a three-year period. Nye County plans to establish an "Early Warning Monitoring Network" and create a comprehensive baseline of critical aquifer parameters.

In the EWDP phase II, Nye County NWRPO will drill an additional 3 or 4 boreholes to support an ATC south of Busted Butte near FortyMile Wash, on, or near the Nevada Test Site (NTS). Nye County will perform pump testing (48 hour) in the boreholes at completion of the drilling operations. The TCO will provide the interface between Nye County and the YMP for associated YMP testing activities to Nye County pump test (48 hour). After Nye County pump test, the M&O and U. S. Geological Survey (USGS) will perform single-well hydraulic and injection pumpback tracer testing.

A separate FWP will be developed for the single-well injection pumpback tracer testing (identified in "Proposal to Conduct Hydrologic and Tracer Testing in Saturated Alluvium to the South of Yucca Mountain, Nevada," dated 05/27/99, Paul Reimus, [LANL], and M. J. Umari [USGS] Principal Investigators [Pis]).

In FY 01, the YMP plan to perform cross-hole hydraulic and conservative/reactive tracer injection tests.

The Nye County EWDP and ATC drilling and testing is contingent on extra budget approval by the DOE-forM&O/USGS and the Nye County (identified in "Cooperative Agreement DE-FC-08-99NV12083" dated 11/05/98, from Lisa D. Ray [DOE]).

The USGS and the M&O (i.e., Sandia National Laboratories [SNL], and Los Alamos National Laboratory [LANL]) will conduct hydraulic and tracer testing in FY00 & FY01 with the following principal objectives:

1. Identify/verify radionuclide transport processes in the saturated alluvium.

2. Obtain flow and transport parameter estimates representative of the saturated alluvium south of Yucca Mountain.

After the ATC testing completion, the ATC boreholes will be returned to Nye County for integration into the EWDP monitoring network.

The Nye County EWDP and ATC plans include road enhancement, drill pad construction, drilling, sampling, perched-water testing, saturated zone testing, geophysical logging, installation of instrumentation, long-term monitoring, and data collection taking place over a three year period. Nye County is responsible for the EWDP activities and drilling the ATC boreholes.

Nye County EWDP Phase II and ATC plans include drilling 10 new boreholes, rework existing boreholes, and deepening boreholes (i.e., boreholes NC-EWDP-03D & 02D), in FY00.

The affected M&O organizations are responsible for the following: (1) documenting actions relating to site integrity concerns,

• (2) collection/receiving and curation of cuttings/core/water samples, (3) observing perched-water testing, water table testing, and water

level measurements, (4) observing geophysical logging and handling data, (5) surveying borehole locations (GPS), and (6) Eh/pH testing.

The TCO/Sample Management and Drilling Department(SM&DD) will collect/receive cuttings and core samples provided by Nye County at the wellhead for the Project. Core will be taken as needed. The cuttings and core samples for Nye County and the Project will be transported to the Area 25 Sample Management Facility (SMF) for storage.

The Pis or Designee will be on site to collect water samples in the water table, perched-water interval, and pump testing conducted by Nye County, as needed. Also, the PIs will be on Site to interpret stratigraphic units, as needed. The Pl's interpretation of stratigraphic units should not impact drilling rates. A separate FWP will be developed for the detailed interface between Nye County and the M&O/USGS for the single-well/cross-hole hydraulic and tracer injection testing activities at the ATC.

The PI will perform Eh/p'H measurements in each borehole after borehole drilling is completed to total-depth (TD). The TCO and PI will evaluate the borehole conditions to determine if the borehole is suitable for running Eh/pH tools downhole.

The planned borehole locations are from south of Busted Butte to Highway 95, and east/west along Highway 95. All planned boreholes are located outside the Conceptual Control Area Boundary (CCAB) of Yucca Mountain.

If a planned borehole location changes by more than 60 meters (197 feet), the TCO shall contact the Safety Assurance Department (SAD) for possible initiation of a Determination of Importance Evaluation (DIE). Attachment 2 shows a map of the Nye County EWDP borehole locations and Table I below identifies the boreholes identification, coordinate, elevation, and planned TD.

1.1.2 Objectives

The Nye County Work Plan identifies the primary mission to investigate the saturated zone hydrology down gradient of Yucca Mountain. The purpose of the EWDP is to develop a shallow monitoring system to acquire aquifer parameters of the alluvial and upper tuff aquifers and the deep monitoring system to define the deep carbonate aquifer.

The YMP ATC primary purpose is to address the needs for additional Saturated Zone (SZ) datain the alluvium. The ATC will develop/validate conceptual models of radionuclide transport, flow, and transport parameter estimates, in the alluvium south of Yucca Mountain.

The affected M&O organizations and USGS will collect samples critical to stratigraphic, hydrology, hydrochemistry, laboratory testing, and Eh/pH data (identified in "Nye County EWDP FWP Data Requirements Input [REV]", dated 7/24/98, from Ezra Wasson).

This information will provide input to the SZ geologic and hydraulic

flow model to support SR and LA.

1.2 SPECIFIC SCOPE DESCRIPTION

1.2.1 Test Plan-and Organizational Responsibilities

TEST PLAN

Road Enhancement Nye County is responsible for all road enhancement activities.

Pad Construction Nye County is responsible for all pad construction activities.

Borehole Drilling The drilling of the EWDP and ATC boreholes is the responsibility of Nye County. The Nye County Drilling Contractor (NCDC) plans to drill all boreholes utilizing dual wall reverse circulation (DWRC), with dry air medium, to the water table and an alternate drilling method (i.e., air foam and mud-rotary) penetrating the water table. [Refer to the Nye County Drilling and Well Construction Work Plan]. The Nye County on-site representative has overall control of the drilling site along with the drilling contractor supervision. Nye County is responsible for drilling the EWDP/ATC boreholes. Nye County will drill the EWDP/ATC boreholes to controls identified by the DOE/M&O/USGS to support the YMP ATC Program. See Attachment 3 "Alluvium Testing Complex (ATC) - Borehole Vital Characteristics For Test Implementation".

The TCO will provide oversight and will coordinate all Project Scientific/Engineering Personnel activities with Nye County, on behalf of the Project.

Borehole Sampling Nye County sampling plans consist of collecting continuous cuttings, specific core intervals, and water samples by contracted geologists.

The Nye County Sample Management Plan provides its contractor with instructions on collection and splitting the cuttings/core samples at the wellhead, Nye County (50%) and the Project (50%). The Sample Plan identifies packaging and identification requirements (i.e., well, depth interval, and other pertinent information for storage). Nye County may core approximately the first 120 meters (394 feet) of diatomite deposits of borehole NC-EWDP-07S.

All YMP samples will be collected by YMP qualified personnel using YMP QA procedures to obtain Q samples. The TCO/SM&DD geologist will support collection of YMP samples at the wellhead, on a shift by shift basis. The TCO/SM&DD geologist will provide collection/receipt of samples for the Project in accordance with Project procedure. The TCO/SM&DD geologist will provide depth control. The Nye County and Project samples will be stored on site in a controlled facility or area. The Nye County and Project samples will be kept separately within the controlled facility or area. The TCO/SM&DD/Designee will pickup samples on a routine schedule and deliver them to SMF for processing and storage.

The PIs will analyze cutting samples for laboratory determined hazardous minerals from both the unsaturated and saturated zone for stratigraphic data. The water samples will include hydrochemical and isotopic analyses. The PIs or Designee will be on location for specific drilling/coring intervals for stratigraphic interpretation for the Natural Environment Program Operations (NEPO), as needed.

The TCO/SM&DD may provide sample containers, labels, and other materials to obtain YMP samples; process, store, and transport samples as needed. The TCO/Designee will coordinate all YMP field sampling activities with Nye County.

Borehole Testing The PI or Designees, may observe Nye County perched-water testing, water table testing, geophysical logging, perform water level measurements, and collection of water samples. The Nye County Borehole Testing Plan includes sharing half of all water .samples with the Project. The PI or designee will be on site to provide oversight for the collection/receiving of samples from Nye County at the well for laboratory analysis, as needed.

The PIs will perform saturated zone hydraulic and tracer testing in the ATC boreholes. The detailed plans for the ATC activities will be identified in a separate FWP. The M&O/USGS will provide an equipment procurement list (e.g., instrumentation, pumps, tubing string, packers, injection string, and power cable) for the ATC program.

The M&O Geophysical Logging Coordinator (GLC) will be on site during the logging by the Nye County geophysical logging

contractor. The TCO/GLC will use a scientific notebook to report and record the field geophysical logging activities.

The PI will conduct Eh/pH measurements from pumped samples through the flow through cell and by using a wire line unit to lower and raise a test probe in the borehole. The probe will feed measurements to surface recording equipment. The TCO and PI will evaluate the borehole condition before running the Eh/pH probe downhole. Eh/pH testing is planned for all EWDP boreholes. The TCO or Designee will coordinate all field Eh/pH testing activities with Nye County.

NYE COUNTY BOREHOLE SAMPLINGIMONITORING SYSTEM The Nye County EWDP includes the design, installation, maintenance, and data collection of the borehole sampling and monitoring system in accordance with the Nye County Work Plans.

TECHNICAL DATA MANAGEMENT SYSTEM (TDMS) The TDMS Management will establish a system for access and control of data from Nye County NWRPO and the University of Nevada, Las Vegas (UNLV).

Organizational Responsibilities The organizations involved in implementing work under this FWP, as currently assigned by the DOE, the YMP Planning and Control System (PACS) are as follows:

TEST COORDINATION OFFICE The TCO is responsible for coordinating and monitoring YMP test activities in support of participants and the YMSCO. The TCO shall assign a FWP Records Coordinator (FWPRC) to monitor the FWP records process. In conjunction with Nye County, the TCO has the responsibility to schedule and coordinate YMP field activities with all affected organizations of the Project, to define requests, and control YMP field work scope that fall within the approved scope of this FWP. These responsibilities, when shared with the sponsoring recognized organizations, will ensure that data and information gathered from the M&O test activities described herein will meet requirements for site characterization and performance assessment, and be consistent with the M&O Safety & Health Plan and environmental requirements. The TCO is responsible for YMP test coordination and field implementation at the sites, with direct contact through the TCO Field Office at the Exploratory Studies Facility (ESF) Pad or borehole location when YMP test-related activities are occurring. The TCO will maintain Field Test Coordinator (FTC) oversight at the drill site with Field Test Representatives (FTRs)/Designee, as needed. In addition, the TCO ensures DIE controls are passed along to affected organizations. The TCO is responsible for managing all M&O field work associated with EWDP activities. All affected M&O/USGS organizations must coordinate work through the TCO. The TCO will schedule, maintain, and coordinate M&O/USGS activities and personnel assigned to the field, on a daily basis. The TCO or Designee will provide oversight of the Nye County EWDP operations, on behalf of the DOE.

NATURAL ENVIRONMENT PROGRAM OPERATIONS/ PRINCIPAL INVESTIGATORS The NEPO and USGS will provide Scientific PIs, scientific staff, instrumentation and equipment necessary for providing, maintaining, and calibrating all required scientific equipment and instrumentation, accepting and instrumenting the scientific borehole, conducting, monitoring, and reporting the field tests. The Pis will request all field test support functions through the TCO. The Pis are responsible for ensuring that the YMP data and information gathered during test activities described herein are acceptable to meet requirements for site characterization, and are qualified in accordance with QA requirements.

SITE FACILITIES OFFICE No Construction/Craft Management Department support activities have been identified in this FWP. Necessary Field Engineering Department (FED) survey support, Level of Effort (LOE), will be identified by the TCO and initiated by AP-3.14Q.

GEOPHYSICAL LOGGING SUPPORT The TCO/GLC will conduct field oversight to the Nye County geophysical logging operations, on behalf of the YMP, to ensure valid parameters are incorporated to allow future analyses of the logging data, as needed. The TCO/GLC will submit geophysical logging information to the NEPO and USGS Scientific Staff and the RPC.

SCIENTIFIC PROGRAMS SUPPORT The SM&DD will provide SAMPLE COLLECTION personnel to be called out by the TCO. Informally, the FIELD DRILLING ENGINEERING (FDE) personnel will support these activities (e.g., fishing operation for lost equipment in the hole by YMP) as subject matter experts for drilling engineering, as directed by the TCO. Sample Collection personnel will provide appropriate identification, packaging, and shipping support for any samples or core collected as identified by the TCO. The on site SM&DD personnel duties will routinely include on site FTR assignments - FTR/Designee.

TECHNICAL DATA MANAGEMENT SYSTEM (TDMS) The TDMS will provide data coordinators to process data submittal and incorporate Nye County NWRPO and the UNLV database links into the TDMS.

1.2.2 Field Testing Equipment

The PI organizations will provide the testing equipment required for field activities associated with testing Nye County boreholes, and will have approved procedures and/or scientific notebook procedures to follow when using this equipment. PIs are responsible for maintaining documentation for testing equipment, calibration, and methods used to collect data. Specific testing equipment is identified in the Pl's procedures.

No data manipulation software is identified at this time. No manipulation of raw data will occur in the field during data collection; unless performed by the P! under approved procedures or documented in the Pis scientific notebook.

1.3 IMPLEMENTING FIELD DOCUMENTS

This FWP provides the process controls utilized by the TCO to manage the YMP activities associated with the Nye County EWDP and ATC that include sampling, testing, and oversight.

1.4 DATA AND DELIVERABLES

1.4.1 Data Submittals

The site characterization and process model reports results obtained from testing these boreholes will provide information to confirm the 3-D Geologic Framework Model and Hydrologic Model. The Pis have the responsibility for the collection, analysis, submittal, and reporting of data in compliance with Project and participant plans and procedures for their specific testing activities as described in this FWP. Data record responsibility is addressed in Section 6.0 of this FWP. All transfers of data between participants or to outside parties shall be conducted in accordance with AP-SIII.3Q, "Submittal and Incorporation of Data to the Technical Data Management System," and other applicable plans and procedures.

1.4.2 Test Deliverables

See M&O Long-Range Plans/integrated Project Schedule for deliverables associated with these activity elements.

1.5 PLANNED TFM USAGE

There are no QA Controls for the Nye County EWDP activities from DIE BAAOOOOOO-01717-2200-00101 REV 01. No Q Controls for Tracers, Fluids, and Materials (TFM) apply as all work is outside the CCAB and facilities are Non-YMP. See Attachment 1, Document Input Reference Sheet.

2.0 SAMPLING PLAN

The YMP requirements for core samples, and the collection of cuttings will be specified by the affected PIs and provided to the TCO, before start of borehole drilling operations. All YMP core and cuttings will be processed through the Sample Overview Committee (SOC) in accordance with YAP-SII.IQ. The SOC in conjunction with the PI must authorize changes. In the event that insufficient cuttings recovery is obtained to allow a 50/50 sample split, the NWRPO sample requirements will be given priority over YMP requirements.

The current version of YAP-SII.4Q and applicable PI procedures shall be used to document sample collection and provide traceability of all water samples taken from the borehole.

In addition, Pis will collect water samples as per scientific notebook procedures. Stratigraphic units identified in core and cuttings will follow the stratigraphic hierarchy and nomenclature as defined in the YMP Reference Information Base.

PI organizations will provide all non-standard sample packaging materials, transportation containers, and any associated equipment prior to the sample collection activity. PI organizations wil set criteria for all sample types in accordance with YAP-SII.1Q. Sample Collection Support Personnel will provide all standard core and cuttings sample packaging materials for YMP, transportation containers, and any associated equipment for geologic sample collection.

2.1 CONTINGENCY PLANS FOR SAMPLING

No contingency plans for sampling have been identified for this FWP.

3.0 WORK IMPLEMENTATION AND CONTROL

3.1 IMPLEMENTATION

The following list of activities includes those that both implement QARD requirements and management guidance that does not implement QARD requirements. Items flagged with QA: are recognized as being quality affecting unless specifically graded out. Grading is conducted in accordance with QAP-2-0 or an equivalent procedure. Items identified with a QA:NIA do not implement QARD requirements and are therefore considered administrative in scope. Items flagged with NC: are recognized as being the non-quality affecting responsibility of Nye County, and are provided only to demonstrate how YMP and Nye County activities are integrated. The sequence of QA activities may be modified by the TCO FTC based on conditions in the field, provided affected organizations concur and those modifications are documented.

The controls provided in the following sections are based on TCO communications with the NEPO scientific investigators and NWRPO.

The Surface-Based Testing Facilities Requirements Document (SBTFRD)

The YMP is responsible for any item lost in the borehole during the YMP activities. The DOE, M&O, and Nye County will evaluate and pursue recovery of any YMP lost item from a borehole, using fishing operations, and if not recoverable, it shall be reported and recorded in the Daily Operations Reports (DOR).

The TCO/Designee will perform a surveillance and document the condition of the ATC sites prior to the YMP setup and ATC testing. At completion of the YMP ATC testing, the EPD will perform a second surveillance and document the condition of the site. If environmental conditions are such that the site requires cleanup, the M&O will implement environmental cleanup activities and restore the site to acceptable conditions prior to returning the sites to Nye County - identify in future FWP.

3.3 PREREQUISITES AND HOLD POINTS

Affected organizations must inform the TCO if quality-related work elements cannot be conducted as described in this FWP. The TCO, will if applicable, stop work on those elements. If FWP revisions are required, work on effected elements will be stopped until the modifications have been completed and controlled by the Project. Any YMP employee who discovers a condition or practice that creates or if allowed to persist would create, any imminent danger to workers, the public or the environment has the authority to stop work. The YMP personnel rights relating to Safety and Health (S&H) imminent danger conditions are described in Section 1.8.3 of the M&O Safety and Health Plan.

4.0 ADMINISTRATIVE (NON-QA) INSTRUCTIONS

4.1 ENVIRONMENTAL, SAFETY, AND HEALTH

4.1.1 Environmental

(This section is applicable only to YMP Personnel performing YMP work.)

Site disturbing work requires YMSCO/Assistant Manager, Office of Project Execution (AMOPE) approval. All work shall comply with applicable environmental stipulations as found in YAP-30.2. To establish and maintain a high degree of environmental awareness on the YMP, all organizations and employees involved with YMP activities must clearly understand their roles and responsibilities in maintaining compliance with all applicable environmental requirements. All work shall comply with environmental requirements as found in YAP-30.2 and associated approval letters. In order to carry out and meet these environmental requirements plus fulfill this commitment, environmental requirements and stipulations will be presented in the initial Safety Toolbox meeting conducted in association with any field activity related to this FWP, and periodically presented in subsequent Safety Toolbox Meetings.

Additional land access/environmental compliance approvals are necessary for activities that require the use of tracers and need to be brought to the attention of the M&O EPD by the TCO. Tracers must be approved prior to use on a case by case basis. Each tracer approval will contain stipulations regarding tracer usage; i.e., tracer concentrations, rates, and reporting requirements. Any use of tracers in excess of State approved levels must be reported to the EPD by the TCO immediately.

Archeological Upon identification of any important resource such as Native American artifacts by field personnel, stop work and report the finding to the Field Operations Office (FOC), "Ranch Control" immediately by telephone at 295-5915. The M&O EPD also shall be notified.

Biological The Affected Organization Lead, or the TCO, shall notify the M&O EPD at least five working days before field work will begin. This notification allows the M&O EPD to notify M&O Environmental Sciences Department (ESD) so that preparations can be made, if necessary, to re-survey the site(s).

Protected Species Project supervision and site supervisors will be responsible for ensuring compliance with those environmental stipulations and ensuring that desert tortoises are not harmed. All YMP work at a specific site that may endanger a desert tortoise shall be suspended if a tortoise is found on that site. Work shall not resume until the tortoise moves out of harms way or an M&O ESD biologist moves the tortoise.

Vehicular and Equipment Traffic Vehicular and equipment traffic is restricted to specifically designated roads. No off-road travel is permitted unless specifically authorized.

Radiological Safety All applicable items, equipment, and materials that have been in use on the NTS, or will be used by Affected Organizations, shall be surveyed by Radioactive Material Control prior to entering the YMP Site area. This is to identify potential radiological contamination and such shall not be used if the survey results indicate the presence of contamination greater than levels established by the NV/YMP Radiological Control Manual for release of material for unrestricted use. A report of the survey results shall be sent within 10 working days to the Affected Organizations and the YMSCO/ AMOPE/Environmental Field Coordinator (EFC), with a copy to the M&O Radiological Control Manager and M&O EPD. All surveys shall be completed prior to moving an item onto the YMP Site area. An indication of clearance is to accompany each item moved onto the YMP Site area.

All conditions mandated in the M&O Radiation Protection Program and M&O-RI-001, and authorization for the Use of Radioactive Materials or Ionizing Radiation Producing Equipment must be met. Requests for use of radioactive materials, including sealed sources, shall be made to the M&O Radiological Control Manager.

Releases of Hydrocarbon or Hazardous Materials All spills or releases of hydrocarbon materials or hazardous materials shall be immediately reported in accordance with YAP-30.1, Occurrence Reporting and Processing of Operations Information, which includes contacting the TCO, FOC, M&O EPD, and the DOEIYMSCO/AMOPE/EFC.

Compliance with General Environmental Permits The following Environmental Permits control work on the YMP Site area. These Permits may change from-time to time in content, requirements, and/or scope: Class II Air Quality Permit AP9611-0573: If work will generate dust

or other air emissions or if it requires the use of equipment rated over 250 horsepower, conditions in this permit must be followed.

Underground Injection Control Permit UNEV89031: In addition to tracer approvals, requirements for environmental water sampling and handling of discharges from drilling activities and pump tests are provided to this permit. The M&O TCO, Construction Management, or the PI of the Affected Organization shall notify M&O EPD a minimum of two days before a discharge is planned. All potential water discharge shall be sampled for analysis according to this permit.

Groundwater Appropriations Permit: If a pump test is proposed, a groundwater appropriations permit may be needed, or requirements in an existing groundwater appropriations permit must be followed.

A separate FWP will be developed for single-well/cross-hole

hydraulic and tracer injection testing.

ENVIRONMENTAL, SAFETY, AND HEALTH REVIEW

1.0 INTRODUCTION

The Environmental, Safety, and Health (ES&H) Review of the Field Work Package (FWP) for Nye County Nuclear Waste Repository Project Office (NWRPO) Early Warning Drilling and Testing Program (EWDP), has been compiled by the Test Coordination Office (TCO) ES&H Specialist. This ES&H review strives to incorporate the seven guiding principles and five core functions of Integrated Safety Management (ISM).

This review has been conducted to ensure ES&H functions have been integrated into the activities described by the FWP.

The purpose of this ES&H Review is to: (1) provide a Preliminary Hazard Analysis (PHA), which identifies and lists the hazards; and (2) recommend engineering, administrative, personal protective equipment (PPE), and work practice control measures for coordinating and conducting scientific characterization activities at Nye County EWDP sites. This ES&H Review is also being compiled to ensure that information about hazards and control measures will be transmitted to all affected organizations on the Yucca Mountain Site Characterization Project (YMP) in order to integrate ES&H into all scientific characterization activities, processes, and operations.

Scientific Characterization Managers and Supervisors should read/review this document, and work with ES&H to evaluate work processes and operations where Job Safety Analysis (JSA) will need to be conducted and documented. Scientific Characterization Managers and Supervisors should contact the Environmental Programs Department (EPD) to ensure that the necessary environmental permits have been applied for and approved in accordance with Environmental Programs Department PRO-EP-001 "Environmental Permit Compliance".

Scientific Characterization Program Managers sending personnel to collect samples/data at Nye County EWDP sites should be aware that a "Medical Needs Analysis" that is in accordance with PRO-SH-012, "Conducting a Medical Needs Analysis" should be completed and on file for each site.

1.1 WORK AND SCIENTIFIC CHARACTERIZATION SCOPE

Nye County plans include the services of a "Construction/Drilling Subcontractor" to drill approximately 23 new boreholes down from the hydrologic gradient of Yucca Mountain. These new boreholes are divided into categories of shallow boreholes drilled to total depth of less than 1,000 feet, and deep boreholes equal or greater than 1,000 feet. In addition Nye County EWDP Phase II, plans include 3-4 additional boreholes for an Alluvium Testing Complex (ATC) south of Busted Butte, on the Nevada Test Site (NTS). The United States Geological Survey (USGS) and Los Alamos National Laboratory (LANL) will conduct

hydraulic and tracer testing. The work will include; road enhancement, drill pad construction, drilling, collecting geologic samples, perched-water testing, saturated zone testing, geophysical logging (both Video and Neutron), installing of instrumentation, long term monitoring, and data collection. Nye County is responsible for performing work to their own Work Plan, Quality Assurance Plan, Safety and Health Plan, and will manage and coordinate all of the above activities.

This ES&H Review will be limited in scope and only provide guidance for scientific personnel collecting rock cutting/core samples, pump testing, water level, and Eh/pH measurements from the Nye County "Construction/Drilling Subcontractor" at EWDP sites.

A detailed description of all scientific characterization activities, including borehole location, for Nye County EWDP can be found in FWP-SBD-99-001.

2.0 HAZARDS

2.1 PRELIMINARY HAZARD ANAYLSIS (PHA):

3.0 HAZARD CONTROLS

"Dry" Drilling: Some Nye County EWDP activities may require that boreholes be drilled "dry" using compressed air as the drilling fluid. Respirable dust containing Crystalline Silica could be produced as a result of these types of operations.

"Dry" drilling should only be conducted according to the Work Practices as outlined in Section 5.2 of the M&O Safety and Health Procedure PRO-SH-014, "Silica Protection Program". PRO-SH-014 requires scientific characterization managers and supervisors to implement effective engineering controls, such as atomizing water spray, Highly Efficient Particulates and Aerosols/Air (HEPA) filtration system, dust diverter, Torit dust collector and/or administrative controls (rotation of personnel) and work practices to control Silica Dust.

The M&O Silica Protection Program requires employees to attend a training course on the hazards and control measures for Silica exposure. PRO-SH-014 recommends that workers exposed to high dust level work areas receive a chest x-ray and "uniforms" to wear. In addition, as part of this program, employees may be required to wear a respirator as a secondary line of defense.

Employees will follow the guidance/requirements that can be found in the M&O Safety and Health Procedure PRO-SH-009, "Respiratory Protection Program", should respiratory protection be required as part of Nye County EWDP activities. Supervisors and employees wearing respirators have several specific responsibilities/actions under this procedure. Employees must have completed a physical examination within the past year and have no facial hair that interferes with the sealing surface of the respirator. M&O Safety and Health Procedure PRO-SH-007, "Health Surveillance Program" describes how M&O employees can arrange for physical examinations, on site through Bechtel Nevada Medical. To schedule physical examinations call 5-2957.

Respiratory equipment, fit testing, and training for M&O Yucca Mountain Site Characterization Project (YMP) and Scientific Characterization Personnel is supplied by M&O Safety and Health Industrial Hygiene (IH). Contact the IH Staff (day shift only) in Trailer #5 on the ESF Pad (5-6269) for respirator fit testing, equipment issue, and training support. You must call and make a reservation/appointment in advance.

Currently, YMP employees are required to wear occupational respiratory protection, i.e., at a minimum, 2 mask HEPA Filtered Air Purifying Respirator whenever dry drilling activities are being conducted that generate dust above the Action Level (AL). Once the respirator has been donned, personnel may only lift the mask to take a short drink of water, spit, wipe their nose, and/or briefly communicate. Smoking and chewing

of tobacco is not allowed around drill rigs where dry drilling is being conducted and respirators are required.

M&O IH sampling and monitoring program which includes both the sampling of employees breathing zones and work areas may be conducted during operations where dry drilling is taking place. Engineering controls are being constantly evaluated and additional engineering controls will be recommended when they are required.

Extended working hours, extended driving hours to remotelisolated areas: During some scientific characterization activities at Nye County EWDP sites, extended working and extended-driving hours will be encountered. Whenever possible, personnel conducting activities at Nye County EWDP sites should utilize the "buddy system". Transportation, back and forth to borehole sites, should be done in pairs (or more) riding together in a single vehicle. Find someone to ride with, who will stay awake and monitor your driving. Driving alone, back to town, after working at remote/isolated sites for more than two shifts is not permitted. Scientific characterization personnel are required to conduct a "radio net" and/or telephone check-in/check-out with Ranch Control for a "head count" that is required on a daily basis. Personnel working at Nye County EWDP sites should never venture into the field without a radio (YMP Net #1), first aid kit, food, and water.

Nevada State Law requires you to wear seat belts, and the U.S. Department of Energy (DOE) requires you to wear a seat belt if you are driving a government vehicle. Drivers of vehicles are responsible for ensuring that passengers wear their seat belts.

To report life threatening injuries or medical emergencies, on the NTS Radio Network call "Mayday, Mayday, Mayday", emergency services will answer, and will request information on the nature of your emergency and support/assistance you may require. Ranch Control also has to be notified.

Working outside in inclement weather and temperaturelhumidity extremes: Personnel conducting scientific characterization activities at Nye County EWDP sites could encounter severe inclement weather (rain, lightning strikes, and/or high winds) with little or no warning.

In the summer, temperatures are routinely above 110 degrees F. In the winter, snow could even be encountered with little or no warning.

Sunburn is a very definite possibility if you are going to be outside most of the day. Always wear "sun screen", long pants and long sleeved shirt.

NTS Net Radio Control ("900") monitors the National Weather Service for conditions on the test site, and broadcasts them over all radio nets on the NTS, including the YMP site. Personnel should have YMP NET #1 radios with them when they go into the field in order to monitor "900" weather control. If severe weather warnings and/or lightning strikes are issued, personnel should suspend work and take cover inside vehicles or leave the site altogether. At certain times of the year, severe cold and hot

temperatures could be routinely encountered at Nye County EWDP borehole sites.

In cold environments: In order to keep Nye County EWDP scientific personnel from experiencing the effects of cold stress, the deep body (core) temperature must not fall below 36 degrees C (96.8 degrees F). For a single, occasional exposure to a cold environment, a drop in core temperature to no lower than 35 degrees C (95 degrees F) is permitted.

Lower body temperatures will very likely result in reduced mental alertness, reduction in rational decision making, or loss of consciousness, with the threat of fatal consequences.

In protecting Nye County EWDP scientific characterization personnel from the effects of cold stress, the following items should be noted:

Pain in the extremities may be the first early warning of danger to cold stress.

Suits and cold weather gear should provide whole body protection, with emphasis on hands, feet, and head from cold injury.

The higher the wind speed and the lower the temperature in the work area, the greater the insulation value of the protective clothing required.

If fine work is to be performed with bare hands for more than 10-20 minutes in cold conditions below 16 degrees C (60.8 degrees F), special provisions should be established for keeping employees' hands warm (i.e., fuel burners, Warm air jets, electric radiators).

Nye County EWDP scientific characterization personnel should not be involved with the use of evaporative chemicals/liquids (i.e., gasoline, alcohol, or cleaning fluids). These types of chemicals/liquids require special precautions to be taken to avoid soaking of clothing or gloves with the liquids, because of the added danger of cold injury due to evaporative cooling.

If scientific characterization work at Nye County EWDP site needs to be performed contiguously in temperatures that are below -7 degrees C (19.4 degrees F), then a "Work-Warming Regimen" must be established and implemented.

Controlling Cold Stress: Personnel conducting scientific characterization activities at Nye County EWDP sites can find guidance for working in cold environments and for work/rest regimens in the latest edition of the Threshold Limit Values Booklet for Chemical Substances and Physical Agents published by the American Conference of Governmental Industrial Hygienist (ACGIH).

In hot environments: Temperatures in Area 25 can reach 120 degrees F in the hottest part of the summer. Working in these hot work conditions/environments with elevated humidity readings will produce elevated heat stress levels. In conditions such as these, three types of

emergencies can occur that involve heat stress; heat cramps, heat exhaustion, and heat stroke.

Personnel conducting scientific characterization activities at Nye County EWDP sites should be aware of the following symptoms of heat stress and accompanying first aid treatments:

Heat Cramps: S5ymp-toms: 1. Muscle cramps in legs and abdomen. 2. Pain accompanying cramps. 3. Profuse sweating. 4. Faintness. First Aid Treatment: 1. Move to cool (air conditioned) place. 2. Sip salted water (1 teaspoon of salt in 1 quart). 3. Massage cramped muscles. 4. Obtain medical treatment.

Heat Exhaustion: Symptoms: 1. Profuse sweating. 2. Intense thirst from dehydration. 3. Cool, moist skin (clammy and pale). 4. Fatigue, weakness, dazed. 5. Dizziness. First Aid Treatment: 1. Move to cool (air conditioned place). 2. Loosen tight clothing and remove excess clothing. 3. If conscious, sip salted water. 4. Treat for shock, lay on back and raise feet slightly. 5. Stay with the victim until medical aid arrives.

Heat Stroke: (Please note: Heat Stroke is a medical emergency) Sympto~ms: 1. Can occur suddenly, with little warning. 2. Dizziness, raging headache. 3. Hot, dry, flushed skin. 4. Full and fast pulse. 5. Breathing deep at first, later shallow breathing. 6. High temperature (107 degrees or higher). 7. Confused delirious behavior. 8. Muscle twitching, growing into convulsions. 9. Loss of consciousness or coma. Emergency Care: 1. Heat Stroke is a true medical emergency, arrange transport to a medical facility without delay. 2. Move to cool (air conditioned) place. 3. Strip to underclothes. 4. Lay on back, head and shoulders raised slightly. 5. Assure breathing airway is open. 6. Put ice or cold wet cloth on head. 7. Cool body with water or wet cloth.

8. Do not give coffee, cigarettes, or a stimulant.

Controlling Heat Stress: Personnel conducting scientific characterization activities at Nye County EWDP sites should:

"* Follow scheduled work/rest cycles. Guidance for work/rest regimens can be found in the latest edition of the Threshold Limit Values Booklet for Chemical Substances and Physical Agents, published by the ACGIH.

"* Workers should alternate between light and heavy Work. "• Where possible, rotate duties among several workers. "* Drink plenty of water. Drink at least 1-7-ounces about an hour before

and then 5 to 7 ounces every 15 to 20 minutes during work. Some people find electrolyte drinks (i.e., Gatorade) effective instead of, or in addition to water.

"* Encourage workers to wear loose fitting, light-colored clothes whenever possible.

Personnel conducting scientific characterization activities at Nye County EWDP sites should drink plenty of liquids and take frequent breaks. M&O Safety and Health Procedure PRO-SH-008, Occupational Heat Stress, provides guidance for dealing with potential heat stress conditions and establishes responsibilities within the M&O.

Working around drilling, and heavy equipment. Drilling equipment: When and if scientific characterization activities at Nye County EWDP sites require personnel to conduct work around electrical circuits or "certain" types of mechanical systems/components (i.e., drilling systems, conveyer belt systems), the systems/components must first be de-energized, isolated, and rendered inoperative before employees can begin work. This process of de-energizing, isolating and rendering inoperative is known as Lockout/Tagout. Lockout/Tagout is an Occupational Safety and Health Administration (OSHA) program requirement that seeks to ensure that the electrical circuits and electrical control circuits to "certain" mechanical systems/components have been locked and tagged (identified) in order to prevent accidental activation.

The Lockout/Tagout program is not required for working around "all" equipment (i.e., automobiles, pick-up trucks, front-end loaders, and forklifts). However, scientific personnel conducting activities at Nye County EWDP locations where heavy equipment is being used, should check-in with the "Construction/Drilling Service Provider" personnel operating such equipment before attempting to work in the area, and to ensure that the operators/miners/teamsters know of your presence.

Heavy equipment: Working around heavy equipment presents respirable dust hazards and safety hazards from contact with and being struck by moving equipment. For example, a "front-end loader's" safety hazards would include both pinch points and being struck or run over by the equipment. When scientific characterization activities require work in the same area as heavy equipment, personnel should be aware of the following:

When heavy equipment is producing visible-dust and the "Construction/ Drilling Subcontractor" cannot control it with water, leave the area. Do not stand behind mobile equipment when it is running. (The operator may not be able to see or even hear you).

All mobile equipment (i.e., front end loaders, forklifts) have warning systems installed on them that sound a loud alarm whenever the equipment/machine is placed in reverse. Pay attention to these alarms and watch for moving equipment.

Most mobile equipment is operated with a-tender (a driller and/or another operator), who watch the machine as it moves, and warn the operator of foot travelers in the area. Pay attention to the commands of the operator and/or the tender.

Pay attention to your surroundings and follow all directions of the Nye County EWDP Personnel and/or "Construction/Drill Subcontractor's" personnel while accessing sites on the YMP where heavy equipment is being used.

Working in High Noise Levels: Personnel conducting scientific characterization activities at Nye County EWDP sites need to be aware that drilling operations have been designated as high noise level areas. Engineering controls should be implemented when feasible to reduce the noise levels, however, the designation means that hearing protection (earplugs and/or earmuffs) must be used around drilling operations where the noise level is 85 dBA or higher. Earplugs and earmuffs are available from your employer. During certain operations, (i.e., jack hammering . operation), dual protection, both earplugs and earmuffs, may be required. Contact the TCO S&H Specialist for information on Nye County EWDP activities where dual protection may be required. PRO-SH-004 "Hearing Conservation Program" specifies the engineering, administrative, and PPE control measures that are required for employees working in high noise level areas. All employees working in these areas must be in a "Hearing Conservation Program" and, among other things, obtain a baseline and annual audiogram (hearing test). Occupying rodent and/or insect infested buildings, trailers, or work areas.

Hantavirus: An outbreak of a potentially fatal illness has occurred in the Southwest, primarily in New Mexico and Arizona, although three cases have been reported in Central Nevada. The cause of the illness has been identified by the Center for Disease Control (CDC) as the Hantavirus. Rodents such as pocket mice, deer mice, canyon mice, and kangaroo rats are the primary carriers of the virus.

As a precaution, all scientific personnel should be aware of the possibility of exposure to the Hantavirus and follow this advice:

" Avoid human contact with rodents, rodent droppings, and rodent nesting materials. Infected rodents carry the virus in saliva, urine, and feces.

"* The virus can infect humans through breathing the dust of dried out rodent feces and urine, and/or contact with rodent feces/urine through skin that is cut, dried, or broken. Avoid breathing the dust from rodent infested areas; avoid skin contact with rodent infested areas. "* If a facility or job site in which scientific personnel are working has visible signs of "heavy" rodent infestations-(i.e., rodent excreta, and/or rodent nests), the rodents should be trapped/removed and the facility cleaned and disinfected.

" Scientific personnel should make no attempt to remove or clean-up rodent infested areas. The M&O Industrial Hygiene personnel have trained CRMD craft personnel in pest control techniques. They have the expertise, equipment, and supplies to trap and clean-up heavy infestations.

Insects, scorpions, black widow spiders, and poisonous snakes: Work areas that have signs of insect infestations should be sprayed with pesticides. CRMD craft personnel have been trained by IH to perform this service, and only the craft personnel should be conducted pest control spraying operations. Do not bring your own insect spray onto the site. Scientific testing personnel should be vigilant when walking near areas including stacking pipe, stored drilling equipment, or stored materials as these are favorite nesting places for spiders, scorpions, and poisonous snakes. Seek immediate medical attention, if bitten. Fire: Scientific characterization personnel conducting work at Nye County EWDP sites need to be aware that the high fire hazard season at the NTS runs from spring to fall. Brush fires have been started by lightning strikes. Fires have also been started by vehicle's exhaust pipes coming into contact with brushes while off-road driving. Vehicle exhaust pipes can reach 500 degrees F or more. While the likelihood of such fire hazards is remote, personnel conducting scientific characterization activities need to remain alert and report any fire or smoke immediately to "900" Radio Net Control. Personnel going to the field locations should carry fire extinguishers and shovels.

Follow the direction of the Nye County Personnel and/or the "Construction/ Drilling Subcontractor" supervisory personnel in the event of a fire emergency at an EWDP site.

Walking, working on wet, uneven surfaces: In most cases, personnel conducting scientific characterization activities at Nye County EWDP sites will only be collecting samples and data from the "Construction/Drilling Subcontractor". However, scientific personnel should be aware that most borehole sites have rocky, gravel, or dirt surfaces, which also have irregularities on the walking surfaces. Slips, trips, and falls could be hazardous.

Working with chemicallhazardous materials: Any chemical tracer, fluid, or material (TFM), or hazardous material that could be used on the YMP as part of the Nye County EWDP activities, must be approved by the M&O Environmental Programs Department through the "Authorization to Purchase and use Regulated Hazardous Materials" PRO-TS-007. An Authorization to Purchase and use Regulated Hazardous Materials form must be submitted and approved before chemicals and materials can be transported and used on the project.

The requirements for identifying, characterizing, tracking, and documenting the disposition, management-, and disposal of nonhazardous waste (including empty and used containers and used lead acid batteries) generated during Nye County EWDP activities on the YMP shall be in accordance with Environmental Programs Department Procedure PRO-EP-002 "Non-Hazardous Waste Management".

Federal, State, and the U.S. Department of Energy (DOE) requirements mandate that regulated and hazardous materials are managed to minimize the potential of their release during transport, storage, and use. Environmental Programs Department Procedure PRO-EP-004 "Spill Management", directs those who transport, store, and use regulated hazardous materials to use appropriate engineering practices, develop plans to be followed in the event of a release or spill, be capable of responding to such a release, and be able to notify appropriate authorities.

The use or removal of testing related TFM by affected organizations must be identified and reported to the TCO prior to such use or removal. The TCO ES&H Specialist coordinates the annual inventory of chemicals and hazardous materials as required by OSHA regulation and the M&O Safety and Health Procedure PRO-SH-003, "Compliance with the Occupational Safety and Health Administration Hazard Communication Standard". The affected organization must know the location of the Material Safety Data Sheets (MSDS), and the proper storage, use, transportation, and PPE requirements for all hazardous chemical use. The MSDS should always be reviewed before using any product/material underground. Contact the ESF TCO ES&H Specialist, if there are questions regarding TFM usage on the YMP.

Neutron well logging: As part of Nye County EWDP activities, Neutron sources could be used to log boreholes on the YMP. A subcontractor to Nye County may perform this activity or it may be conducted in-house by personnel from the M&O.

Neutron logging activities conducted by a subcontractor will require written permission from the state of Nevada. The subcontractor is also required to be licensed by the Nuclear Regulatory Commission and the Department of Transportation (DOT). Regardless of whether the Neutron Logging is conducted by a subcontractor or in-house authorization for the use of ionizing radiation producing materials will be through the M&O Radiation Control (RADCON) Manager, and will be in accordance with the NV/YMP RADCON Manual and the M&O Radiation Protection Program (RPP).

All Neutron logging wireline measurement work on the YMP involving M&O personnel will be conducted under FWP-ESF-96-013, "Borehole Wireline Measurements for ESF Testing Activities". A Radiological Work Permit (RWP) will be issued before any activity involving the use of radioactive materials/tools is permitted. Radiological Health Surveys will be conducted during the use of these radioactive materials/tools. Radioactive Material Areas (RMA) with controlled access will be established during deployment. Signage, posting, and barricade requirements will be in effect. Access will be restricted or limited during calibration and insertion activities inside and around the RMA. Any employee required to be inside the RMA-will;-at a minimum, have General Employee Radiation Training (GERT) and Rad Worker I Training. Any employee inside the RMA will have the following personal monitoring devices; NTS Whole Body Thermoluminescent Dosimeter (TLD) and Neutron Dosimeter. Personnel handling the tool and related accessories will wear finger TLDs.

4.0 ROLES AND RESPONSIBILITIES - SAFETY AND HEALTH 4.1 Nye County Controlled Area

Nye County will establish and maintain control over all EWDP Sites. A fence will be erected around all Nye County EWDP Sites, access to this "controlled area" will be the responsibility of the Nye County EWDP Personnel and/or the "Construction/Drilling Subcontractor". All work performed inside this "controlled area" will be the responsibility of Nye County and/or the "Construction/Drilling Subcontractor". Scientific characterization personnel will access the "controlled area", but only under the direction of the EWDP personnel and/or the Construction/ Drilling Subcontractor. A Point of Contact will be established with the Person-In-Charge (PIC) for the Nye County EWDP and/or the "Construction/Drilling Subcontractor" and the PIC for scientific characterization activities. All scientific characterization samples/information/data collected inside the "controlled area" will be coordinated by the PIC for scientific personnel with the designated PIC from either Nye County EWDP or "Construction/Drilling Subcontractor". (A description of the PIC's roles and responsibilities is provided below). Inside this "controlled area" all work will be preformed according to the Nye County Safety and Health Plan.

4.2 Safety and Health Roles and Responsibilities:

In order to fully implement the principles of ISM, each work site is controlled by an M&O department, a subcontractor or an M&O participant. The organization that controls the work site will establish the appropriate safety procedures for the conduct of activity on that site. Nye County controls the work site and will determine the appropriate safety procedures for activities inside the EWDP sites. Outside the Nye County controlled area, the CRWMS M&O is in charge and has the responsibility for S&H for all teammate organizations, employees, visitors, and personnel from other organizations when they are on M&O controlled worksites. Clear roles and lines of responsibility,

authority, and accountability are established at all levels of the. organization to ensure protection of workers, the public, the environment, and property.

4.3 Specific Group and Individual Safety and Health Roles and Responsibilities: The following groups and individuals are responsible for:

Nye County EWDP "Construction/Drilling Subcontractor: Nye County is responsible for all activities inside the "controlled area" at each EWDP site. Nye County is responsible for work to their own Work Plan, Quality Assurance Plan, and Safety and Health-PRan, to manage and coordinate the activities for drill pad construction, road enhancement, drilling, testing, and long term monitoring.

Test Coordination Office: The TCO is responsible for selecting the PIC for the work area outside the Nye County- EWDP "controlled area". The TCO is responsible for the implementation of the requirements of this FWP and for coordinating all scientific testing activities with Nye County and/or the drilling subcontractor. The TCO and other scientific organizations are responsible for the safety and health of their employees, outside the "controlled area" at Nye County EWDP sites. The TCO is also responsible for conducting scientific characterization activities/operations that are in compliance with the M&O Safety and Health Plan, the TCO FWP ES&H Review, and their own organization/laboratory standards, codes, regulations, procedures, and programs. The TCO is responsible for coordinating and documenting the "daily" Toolbox Safety Meeting between scientific characterization personnel conducting activities at Nye County EWDP sites.

Line Managers and Supervisors: M&O line management and supervision is responsible and accountable for the protection of workers, the public, the environment, and property. M&O line managers and supervisors are responsible for evaluating their work operations, activities, or processes that present new or unusual hazards, then conducting and documenting a JSA. A JSA is a structured, step-wise method to identify discrete tasks in a job, recognize the hazard(s) involved in each task, and specify mitigations to eliminate or reduce the hazard(s) to an acceptable level (i.e., engineering, administrative, or PPE controls, employee S&H training). All JSAs must be conducted and documented in accordance with M&O Safety & Health Procedure PRO-SH-011, "Conducting a Job Safety Analysis". All JSAs must be reviewed and approved by M&O S&H. Contact the TCO ES&H Specialist for assistance in determining which scientific characterization activities require JSAs. Once the JSA has been produced, reviewed, and approved, this will be used as a task guide for employee training purposes, and this training will be documented. In addition, as previously mentioned, line management and supervision should be aware that a "Medical Needs Analysis" that is in accordance with PRO-SH-012, "Conducting a Medical Needs Analysis" should be completed and on -file.

Individual Employees: M&O employees (once they have been trained and understand the requirements), regardless of their employer, are responsible for understanding and following the requirements of the ES&H programs of their employer and specific YMP ES&H programs (i.e.,

Occupational Respiratory Protection, Noise-Control & Hearing Conservation, PPE). M&O employees are responsible for ensuring that the ES&H training they have received is followed and implemented, regardless of whether the training was received from their parent organization or on the YMP. M&O employees are responsible for immediately notifying the PIC and then their M&O organization supervisor of unsafe acts, conditions, and/or equipment.

Person-In-Charge: The PIC is responsible for ensuring the specific work (construction/testing support or scientific testing) activity is conducted in accordance with M&O established S&H procedures. The PIC is responsible for ensuring that the Toolbox-Safety Meeting is conducted at the beginning of each shift, on a daily basis. A PIC shall be present in the specific work area during all construction/testing support or scientific testing activity, operation, or process. The PIC will determine if the workers present have the experience,- knowledge, skills, and abilities needed to perform their work safely and competently. The PIC will be a point of contact for all visitors to the specific work area. (i.e., all workers in the specific work area should be able to immediately identify the PIC). If the PIC needs to leave his/her assigned specific work area, a new PIC will be selected and the change will be communicated to all individual workers in the specific work area. If an issue arises, the PIC will be responsible for notifying the appropriate manager(s) and supervisor(s) and serve as the focal point of contact for issue resolution.

4.4 Written Safety and Health Roles and Responsibilities - TCO ES&H Review:

The ES&H Review is an attachment to this FWP and contains both a PHA and a section recommending control measures for all identified hazards.

The ES&H Review is specific to the construction/testing support and scientific testing activities found in this FWP. Each organization's line management and supervision shall read the ES&H Review and use it as both guidelines and minimum requirements for informing, educating, and implementing control measures for all identified hazards. A JSA may be assigned by the TCO to be generated by another organization and will be incorporated in the work described in this FWP. A copy of the ES&H Review will be available at the ESF TCO field office, the Las Vegas Office, and will be transmitted to test and construction departments working on the YMP.

M&O Teammate organizations and employees will still perform work that is authorized by their respective FWPs, Work Instructions (WIs), work plans, and/or work procedures. Teammate organizations perform their work as an "integrated group" to the ES&H polices and procedures as set forth by the M&O.

M&O Teammate organizations and employees will continue to conduct construction/testing support and scientific testing work on the YMP through the umbrella of the M&O Safety and Health Plan and the listed Environmental Plans that can be found in the FWP.

The "M&O Safety and Health Plan" (B100000000-01717-4600-00016), establishes implementing guidance/requirements through the S&H procedures.

5.0 EMPLOYEE TRAINING

Personnel requiring access to the YMP site must have completed or be escorted by an individual with General Employee Training and First Aid training. Scientific Characterization Personnel will not require access inside the fence at Nye County EWDP sites.

The PPE is required for all persons entering-any other construction site, besides the Nye County EWDP sites on the YMP (i. e., hard hat, steel toed shoes, approved [ANSI Z87] safety glasses, and/or hearing protection plugs or muffs). Casual dress (shorts, no shirt, sandals) is not allowed at YMP borehole sites. Work shirts (short sleeve or long sleeve if you are worried about sunburn), long pants are the normal attire. PRO-SH-002 "Procurement of Required Personal Protective Equipment" describes how M&O employees go about obtaining prescription safety glasses and approved footwear through the M&O purchasing system.

All scientific characterization personnel shall adhere to the PRO-SH-001 "Accident Investigation, Reporting, and Recordkeeping" for guidance on reporting "recordable injury and illness".

5.1 OTHER TRAINING

All personnel entering Nye County EWDP M&O controlled work sites, who have not received the toolbox safety meeting briefing, are to ask the M&O PIC for current conditions at the site before entry. This is to ensure compliance with applicable OSHA Standards.

6.0 EMERGENCY RESOURCES LOCATION AND CONTACTS:

For weekend work in remote locations, scientific characterization personnel at Nye County EWDP sites will need the approval of the TCO. A "Safety and Emergency Procedure Description and Plan" must be completed by the TCO. The "Safety and Emergency Procedure Description and Plan" lists participants and emergency contacts and is distributed by e-mail to Project participants and the Site Facility Office.

6.1 EMERGENCY REPORTING

PRO-SH-005, "Emergency Management", was developed for supervisors who have responsibilities for a facility or operation. In case of an emergency, involving scientific characterization personnel, telephone: Dial 911. Or, over the Radio: "Mayday, Mayday, Mayday", then give name and net number, YMP NET #1 Radio Control will then make contact and get details of assistance required.

6.2 NTS RADIO NET

The NTS Radio Net (Station 900) is manned 24 hours a day, every day of the year. Scientific Characterization Personnel conducting activities at Nye

County EWDP sites can contact 900 over the radio and be patched into all other Net Systems on the NTS.

6.3 NEAREST HOSPITAL OR CLINIC

Mercury, Nevada (approximately 40 to 45 miles, depending on the specific Nye County EWDP site).

6.4 CONDUCTING A MEDICAL NEEDS ANLAYSIS: A medical needs analysis for scientific characterization activities, as called out by PRO-SH-012, is on file.

6.5 NEAREST POTABLE WATER

Carry potable drinking water when traveling to remote Nye County EWDP sites on the YMP. Orange "Gott" Drinking Water Coolers, with ice, are available from the front desk personnel at the FOC in Area 25.

6.6 SUGGESTED EMERGENCY EVACUATION ROUTE AND MEETING AREA(S)

Situations may occur at Nye County EWDP sites that will require immediate evacuation of the operation. These situations may include, but are not limited to fire, explosions, and hazardous chemical releases or spills. If such a situation occurs, notify 900 Radio Net Control and Ranch Control immediately. Follow the direction given.

7.0 TCO PERSONNEL

Some TCO Personnel carry hand held radios and are available on the YMP Net #1 or NTS Net #5.
