Foreword

Welcome to the South Staffordshire Commissioning Prospectus: A Partnership approach to improving health and wellbeing outcomes. We are pleased to be able to continue our commitment to improve health and wellbeing outcomes through investment in all sectors that build capacity and strengthen the resilience of communities.

We have now been able to bring together investment from a range of partner organisations to enable joined up locality based commissioning of outcomes to support the improvement of the health and wellbeing of the population of South Staffordshire.

The Prospectus is aimed at strengthening community resilience and assets through funding support which promotes good health and wellbeing and helps prevent situations from getting worse for children, families and adults where there are particular needs. This overall prevention role is vital in managing demand for longer-term support and more expensive services, in turn helping to keep the local health and social care economy sustainable.

We are looking for innovative and evidence based proposals for investments to deliver measurable achievements in the key outcomes detailed within this Prospectus which would be considered for approval for funding. Projects will need to aim to identify future replicable or sustainable service models that build social value. Social value is about maximising the impact of public expenditure, considering more than just financial transaction, but including aspects of happiness, wellbeing, health, inclusion and empowerment.The outcomes included in the 2015-2016 Prospectus are drawn from priorities identified by local insight and partnerships, combined with input from a range of stakeholders including voluntary organisations and third sector businesses.

We look forward to working in partnership with successful organisations and innovative, enthusiastic people on new ways to use local resources effectively and efficiently – to deliver the impacts and outcomes that will make a difference to people’s lives.

Five overarching Priorities have been identified for the South Staffordshire Commissioning Prospectus 2015-2016. Alcohol , Obesity, Mental Wellbeing , Dementia and Rural isolation

These have been developed through:

The Staffordshire Health and Wellbeing Boards Strategy

The South Staffordshire Locality Commissioning Partnership with all partners

Local insight - data and public consultations

Co-production with the community and voluntary sector and partners that has enabled us to get a range of sub priorities that highlight the need for specific activity within the locality.

Each of the five priority areas will tackle the main issues for the wider South Staffordshire Partnership. A number of sub outcomes have been attributed to the prospectus. The sub outcomes will ensure the funders of the 2015-2016 commissioning prospectus achieve their outcomes.

Child Obesity is a key issue across the district with many wards having a higher rate than the County and National average.

The number of adults and children who are above a healthy weight is steadily rising within the district

Physical activity levels for adults in South Staffordshire are lower than the national average.

All Children and young people should engage in moderate to vigorous intensity at least 6o minutes and up to several hours everyday

Vigorous intensity activities, including those that strengthen muscle and bone, should be incorporated at least three days a week.

All children and young people should minimise the amount of time spent being sedentary (sitting) for extended periods.

Adults should aim to be active daily. Over a week, activity should add up to at least 150 minutes (2½ hours) of moderate intensity activity in bouts of 10 minutes or more – one way to approach this is to do 30 minutes on at least 5 days a week.

Alternatively, comparable benefits can be achieved through 75 minutes of vigorous intensity activity spread across the week or a combination of moderate and vigorous intensity activity.

Adults should also undertake physical activity to improve muscle strength on at least two days a week.

1 in 4 people have suffered from some form of mental ill health in South Staffordshire.

Feelings of happiness, contentment, enjoyment, curiosity and engagement are characteristic of someone who has a positive experience of their life. Equally important for well-being is functioning in the world.

Experiencing positive relationships, having some control over one’s life and having a sense of purpose are all

4. APPLICATION PACK

What is it for?

The Prospectus is aimed at strengthening community resilience and assets through funding support which promotes good health and wellbeing and helps prevent situations from getting worse for children, families and adults where there are particular needs. This overall prevention role is vital in managing demand for longer-term support and more expensive services, in turn helping to keep the local health and social care economy sustainable

How?

We are looking for innovative and evidence based proposals for investments to deliver measurable achievements in the key outcomes detailed within this Prospectus. Projects will need to aim to identify future replicable or sustainable service models that build social value. Social value is about maximising the impact of public expenditure, considering more than just financial transaction, but including aspects of happiness, wellbeing, health, inclusion and empowerment.

Project Beneficiaries

The outcomes included in the 2015/2016 Commissioning Prospectus are drawn from priorities identified by local insight and partnerships, combined with input from a range of stakeholders including voluntary organisations and businesses. This will enable local resources to be used efficiently and efficiently to deliver impacts and outcomes that will make a difference to people’s lives within the District of South Staffordshire.

Funding bids must be £5,000 and over. If you are looking for a smaller amount of funding please apply to the South Staffordshire Community Budget

If you are planning to submit more than one proposal under this commissioning prospectus, please ensure that you submit a separate Expression of Interest form for each proposal.

Subject to delivery of successful outcomes, projects may be extended.

If you are proposing to deliver your project within more than one district/borough within Staffordshire please note that you may be required to submit a separate application form for each area. It would be expected in this case that you would identify where economies of scale will be applied.

Match funding is not a requirement of this scheme and you can apply for 100% of the cost of your project or activity however any contributions you can make will strengthen your application. Contributions “in kind” (e.g. volunteer time or hall use given free of charge) can be counted towards your contribution. Please base general volunteer costs on £6 per hour per volunteer.

The prospectus will be split into 5 lots, one lot will be allocated for each of the 5 localities. Please identify within the expression of interest form which localities your project aims to be delivered in.

What can be funded?

Any activity, project or event which is for the benefit of the community. This can include capital and/or revenue based costs.

Groups wishing to purchase equipment must demonstrate in their application how they plan to use the equipment in future activities which are of benefit to the community of South Staffordshire

What cannot be funded?

Routine running costs of a group or organisation, for example rent or stationery as part of the normal day-day business of the organisation. However, running costs specific to the project for which funding is being requested can be considered.

Contributions towards ongoing running costs

Any school curriculum based activity.

Any projects for the sole benefit of individuals

Events or activities the principle aim of which is to fundraise for another organisation.

Political or religious activities or events (although secular activities promoted by faith based groups are welcome e.g. mums & toddlers group or community café).

Projects that are being administered through a third party.

Activities or events which have happened prior to receiving notification of an award. Applications cannot be made in retrospect proof of purchase/booking will be required in the evaluation.

Projects occurring outside the local authority boundary

Requirement of Funding

All applications must demonstrate the following:

They have the ability to deliver the project through sound financial planning and project management.

The activity or event promotes good equal opportunities practice.

The funding must be spent in accordance with the purposes for which it was approved. Any changes to the project/event should be agreed with The South Staffordshire Partnership.

Any profits arising from fund-raising activities (e.g. raffle or ticket sales) at the event must be retained for the benefit of the local area in which the activity occurs.

If your project requires planning permission, this must be obtained prior to applying to the South Staffordshire Commissioning prospectus.

The South Staffordshire Partnership reserves the right to award a grant lower or higher than that for which the application has been made.

If you are successful in your application you will be invited to attend a pre funding award meeting. One of the areas covered will be performance management and inputting data onto the Upshot performance management system

Successful groups/organisations must deliver their project/activity/event within 12 months of being awarded a grant unless otherwise stated.

Successful applications must issue press releases upon receipt of this grant, and again upon completion of the project/activity/event. All press releases must be cleared through The South Staffordshire Partnership prior to release.

A video of the project and its outcomes must be added to the Good life You Tube page.

The South Staffordshire Partnership and Goodlife logos including any other relevant partner logos must be included on all publicity material relating to the event or project (press releases, brochures, posters etc.). Successful applications will be informed of logos they need to include as part of their promotion.

Successful Groups/Organisations must serve all appropriate licences and insurances.

Successful Groups/Organisations must ensure that Disclosure and Barring Service Checks (previously CRB Enhanced Disclosure Checks) are carried out on all adults having direct supervisory contact with any children under the age of 18 or vulnerable adults through the project.

Successful Groups/Organisations will receive 20% of the awarded grant up front, 15 % will be awarded each quarter on submission of performance outcomes. The remaining 20% will be retained until a satisfactory completed Evaluation Report is received.

All successful projects will need to link into the Districts Good life vision and any volunteers recruited will be required to attend a minimum of two let’s work together training sessions.

If the group disbands/breaks up any equipment purchased through the Grant Aid Scheme will become the property of The South Staffordshire Partnership.

All groups/organisations should be aware that their project/activity may be audited after completion.

Please ensure any third parties who you have identified have consented to the application submission.

Unless a research project is being submitted the panel requires all back ground work to be completed in advance of the project start date. Projects will be expected to commence delivery from from April 2015

Talk through your project or idea with us before writing your application. We will be able to advise you on what information you might want to include with your application. You might also wish to talk to Support Staffordshire or VAST who can advise you on all aspects of running your group, from developing constitutions and setting up committees, through to developing project plans and funding applications. This is particularly important to ensure you have included all of the information you have to include or wish to be considered. Applicants known to the panel will not receive preferential treatment. All applications will be scored using a set scoring matrix and criteria –your application can only be assessed on the information provided in your application. If you don’t mention it or provide evidence of it, we don’t know it!

Check through the criteria. Do you meet all of the essential criteria? Does your application include a response about everything that will be considered when assessing your application?

Applications which have the best chance of success look at sustainability and how the project will make a difference to the community.

Is your project needed or wanted? Is there a similar activity already taking place in your area, could you work collaboratively with other organisations on your project? Do you have the support of local elected members?

Volunteers can play an important part of any project, especially when you’re looking at sustaining the project/activity long term. Include as much information as you can about volunteers.

When considering the cost of your project, don’t forget to include “in-kind” contributions – the real cost of delivering your project. If you had to pay people to design posters, deliver leaflets, carry out consultation, or if you had to pay to hire a venue – how much would it cost you? This all adds up to your “match contribution” and although it isn’t essential, it is a consideration and you have to consider that your application will be “judged” against other applications.

When completing the budget section, make sure you have read what is needed in each column and for each section.

Include EVERYTHING you need for the project, even if you are not asking us to fund it.

“In-kind” contributions should be listed as both EXPENDITURE and INCOME – think of it as paying for services without actually exchanging cash.

The amount of income you are able to contribute (including in-kind contributions) PLUS the amount you are requesting from us should EQUAL the total cost of your project.

Funding application process:

For 2015/2016 the process will be a three stage application.

A one sided expression of interest will be introduced for 2015/2016. The Funding panel will meet in December 2014 to discuss Expression of Interests. Expressions of Interest must be submitted by 6pm on Monday 1st December 2014.

Once the panel has gone through all expressions of interest You will be notified in December 2014 if your project has been chosen to go through to stage two.

Stage 2 Full Application

If successful at stage 1 you will be invited to submit a full application to be submitted by 6pm on Monday 19th January 2014.

You will receive a letter confirming receipt of your application and your Grant Application Reference number.

The funding panel will meet in January 2015 to score applications. You will be notified in writing of the outcome of your application

within 4 weeks of the funding panel meeting. If your application is unsuccessful, your letter will give you feedback

on the reasons why.

Stage 3 Project discussion

Before funding is awarded successful projects will be invited to attend a pre award meeting In January 2015. This will be an opportunity to discuss your project in more detail and identify and agree what outcomes you expect to achieve and the logistics of how you will do this.

While the funding panel will assess your application and make recommendations, the final decision will be the responsibility of the Locality Commissioning Partnership and the budget holders.

You will be notified within two weeks of the panel meeting if your project has been awarded funding and can commence delivery.

If your application is successful, this letter will also include an Acceptance Form, Contract and BACS Transform Form, all of which must be completed and returned to South Staffordshire Council in order that your grant payment can be processed.

You will be requested to confirm your availability for a training session on the Upshot performance management system which is the partnerships method of supporting performance management.

Dates will be issued when outcome data is required to be submitted each quarter. Funding will be released once all outcome criteria for that quarter have been received. An Evaluation Form will also be sent to you. This must be completed after the project has happened and returned to South Staffordshire Council in order for the final grant payment to be processed
