All visitors requiring access to a tenant space must be registered with www.115lasalle.com or www.111monroe.com, our web-based visitor registration web site. When a registered visitor shows his/her identification to the Visitor Receptionist located in the main lobby of 111 West Monroe Building (1 East/Center) or on the 3rd floor of the 115 South LaSalle Building (3 West), it takes only a few seconds for the receptionist to print a guest badge and your visitor will be directed to your floor.

6. The protection of building corridors, doors, stairwells, elevator floor coverings, public areas, lobbies and service areas is the responsibility and expense of the tenant and/or moving company. More specifically:

Tenant employees that are authorized to request services have been given user names and passwords to access the building’s work order request system which is called Electronic Tenant Solutions (ETS). We recommend only authorizing one or two people per tenant to request service. All requests for work should be submitted through the ETS online system, unless there is an emergency. If there is an emergency (smoke smell, leak/flood, etc.) call the Office of the Building at (312) 357-0100 immediately!

Any charges for work requests that are billable will be billed to the tenant on their monthly work order invoice.

GENERAL MAINTENANCE

Many general maintenance items can be handled by our on-site 24/7 engineering staff.

To submit a work order request, go to the building’s website www.115lasalle-111monroe.com and click on Login. Log-in to the Workspace site, click on Service Requests > New Service Request. If you are entering the work order on behalf of another employee, please be sure to enter their name and phone number so that our engineers can locate the person/area that needs work. Please make sure to enter a detailed description of the problem including the specific location (e.g. NW corner, John Doe’s cubicle, Room #2095, Ladies Restroom).

If an individual has been authorized by your company to enter work request but do not have a log-in, please contact the Office of the Building at (312) 357-0100. Detailed instructions on how to use the ETS online request system have been given to all tenant contacts. It will be the responsibility of the administrators to forward this document to all company approved users. Please contact the Office of the Building, if you should need to obtain the instructions.

Note for BMO Harris Bank Employees: BMO Harris Bank employees are required by BMO Harris Bank policy to send all work order requests to the Corporate Real Estate Help Desk at (888) 333-7627 with the above information, as well as the cost center to charge for billable service related to the work.

Hot/Cold Calls: Please note that if you are experiencing a problem with the temperature in your area, it is very important to notify the Office of the Building. We will make every effort to address comfort calls within 30 minutes. Space heaters are strictly prohibited and will be confiscated if found. In addition to being a safety hazard and against City of Chicago fire code, space heaters can disturb the accuracy of building thermostats and cause employees in the surrounding areas to be too cold.

Light Bulb Replacement: Some tenants have agreements in place to have the engineering staff perform regular light sweeps within their space. If you would like to set up a similar agreement, please contact the Office of the Building at (312) 357-0100. If you do not want to set up regular agreements but need bulbs changed, please submit a work order using the ETS online request system. The contracted 115 South LaSalle | 111 West Monroe Security is responsible for security in the lobbies, dock area and all common areas within the building. Please note that tenants occupying a full floor are responsible for the security of all areas of the respective floor.

Building Security Officers are not armed and are only intended to serve as protection for the building and as a deterrent to crime. Actual enforcement of local laws is the role of the Chicago Police who should be contacted along with the Office of the Building if a crime or situation requiring the use of force occurs.

115 South LaSalle | 111 West Monroe Tenant Information Manual

  17

Please remember that security is everyone’s responsibility. Valuables should be stored in a safe place and doors locked after hours. The Office of the Building or Building Security should be contacted if any strangers behaving in a suspicious manner are noticed in the office areas. Only with tenant cooperation can a secure building be maintained.

SECURITY TIPS

Building Management takes many precautions to protect tenant property; however, tenants are wholly responsible for the security of all persons and property within their suite. By following a few simple rules, much can be done to eliminate or reduce incidents of theft or intruders.

Never leave a suite door unlocked while the reception area is unattended, even if it is only momentarily. This may be the single, most important rule to prevent thefts and intruders from entering your suite. They know it is not uncommon for reception areas to be left unattended, especially when a firm is opening up for the business day and at the close of the business day.

Rear doors or secondary entrances should be kept locked at all times.

Hang coats and wraps away from the entrance to the office to reduce the possibility of their being easily stolen while you are busy. Keep valuables out of sight at all times. A minimum amount of cash should be kept in the office. Both stamps and cash should be locked in an office safe.

Keep purses and gift packages out of sight and locked inside a cabinet or desk if possible.

Lock desks when not seated at them.

The building prohibits all solicitors and peddlers. While there are many legitimate solicitors, most tenants do not desire to be bothered. Please call the Office of the Building immediately to report all solicitors and peddlers so they may be properly escorted from the building.

Do not let persons other than your employees and clients into building restrooms.

Building personnel are always ready to properly identify themselves. Persons posing as working for the building which you may not readily recognize should be reported to the Office of the Building or the Security Control Room immediately. Every tenant has the right to question all those who enter their suite for proper identification.

Receptionists should be instructed to keep all visitors in the reception area, and ask the person being visited to come out and greet the visitor. Delivery and service personal should be escorted to their work location rather than directing them. This will reduce the possibility of unauthorized people entering unauthorized areas within your office.

Following your own instincts is critical. If there is ever any doubt or you don’t feel right about an individual in your suite, hallway, elevator or restroom, immediately call The Office of the Building to investigate.

FIRE AND SMOKE

When evidence of fire is detected, remain calm and dial 911. Be sure to use the address, 111 West Monroe or 115 South LaSalle Street, when speaking with the 911 operator.

1. After contacting the Chicago Fire Department, immediately contact the Office of the Building at (312) 357-0100. Be prepared to tell the receptionist or the security officer your building address, floor and suite number, the location of the fire, its type and severity, if possible.

2. Inform your Floor Captain. Alert all others in the immediate area.

3. Do not attempt to fight a fire that appears to be out of control or threatens your safety. If possible, close the door to the room where the fire is located and all doors in the immediate vicinity on the floor. This will help contain the spread of fire and smoke. If the fire is small enough to be controlled by fire extinguishers, hand-held chemical fire extinguishers are located on each floor. To operate the extinguisher, follow these simple instructions:

Remember PASS: P -- Pull locking pin A -- Aim at base of fire S -- Squeeze trigger S -- Sweep spray back and forth at the base of fire

4. Listen for instructions over the emergency communication system. You will receive emergency instructions that will be broadcast to the entire building or to individual floors from the Emergency Control Center.

Remember; never use an elevator in a fire situation. Always use the nearest stairwell.

EVACUATION

In the event of a full floor evacuation due to fire, the fire floor, two floors directly above and five below will need to relocate. Relocation should be achieved using the building stairwells. Do not use elevators for emergency evacuation. Evacuation of additional floors will be by order of the Chicago Fire Department once they are on site.

During the evacuation process, please follow the instructions listed below:

1. Listen for instructions over the building's emergency communication system and your Emergency Preparedness Team members.

2. Do not return for personal belongings.

3. Close, but do not lock, doors behind you.

115 South LaSalle | 111 West Monroe Tenant Information Manual

  27

4. Check for stragglers, especially in the perimeter offices and washrooms.

5. Before opening any doors, feel the surface for heat. If the door is hot, proceed to another exit. Heat may be an indication of fire on the other side of the door.

6. Keep calm and walk quickly. Relocation will be through one of the five stairwells in the building. ALWAYS RELOCATE DOWN – NEVER ATTEMPT TO GO TO THE ROOF.

7. If smoke is present, stay low and crawl along the floor. The cleanest air is near the floor. If possible, cover your nose and mouth with a wet cloth such as a handkerchief or towel.

8. If you are instructed to evacuate the building entirely, proceed to the nearest stairwell and go down to the ground floor; then proceed to your reassembly point. Reassembly points should be at least two (2) blocks away from the building.

Once the Fire Department deems it is safe to return to your work area, you will be advised to resume normal business operations.

Immediately after you have contacted the Chicago Police Department, contact the Office of the Building at (312) 357-0100. For the protection of personnel and company assets, tenants should:

1. Stay out of the main corridors of the building.

2. Stay away from exterior windows.

3. Lock the main entrance door if the demonstrators are on your floor.

4. Notify all employees and visitors about the civil disturbance and warn them to avoid personal contact with the demonstrators; do not make any comments or statements to anger the demonstrators.

5. Ask all employees and visitors to avoid leaving the building.

6. If demonstrators penetrate your floor, warn employees and visitors to be alert for "unattended” and "suspicious" items that were carried in by the demonstrators, i.e. clothing, knapsacks, bags, etc. Also warn them not to touch, move, disturb, or cover any suspicious items that are left. If any suspicious items are discovered, contact the Office of the Building immediately and follow the procedure outlined in the "Bomb Threat - Physical Evidence of a Bomb" section of this manual.

BOMB THREAT

It has been clearly proven that the vast majority of these calls are indeed false alarms, meant only to disturb the normal work of a person or company. However, at no time should any call be regarded as just another false alarm. The following guide will be useful. When a call is received, there are several things to do:

1. Keep the caller on the line as long as possible.

2. If possible, signal a co-worker to dial 911. Tell the 911 operator to have the call traced. Be sure to use the address of 111 West Monroe or 115 South LaSalle Street when speaking with the 911 operator.

3. Obtain as much information from the caller as possible.

a. Time bomb is to explode. b. Location of bomb. c. Description of bomb. d. Time call is received. e. Exact wording used by caller. Write it down if possible. f. Voice of caller (male, female, dialect, etc.). g. Reason for threat. h. Background noise.

4. The Bomb Threat Telephone Report which follows this section may be useful in helping you record exactly what the caller is saying.

5. After the caller has hung up, call the Office of the Building at (312) 357-0100 immediately to report the threat.

6. Inform your Floor Captain of the situation.

7. The Floor Captain, their Emergency Preparedness Team members, and the Building Staff will make a complete search of the suspected area. It will be the responsibility of the Floor Captain to identify any suspicious items or packages which do not belong in the space. If a suspicious item is identified, the Chicago Police Department police will then investigate the object.

8. Building staff personnel will search the buildings command areas, restrooms, stairwells, vacant spaces and building closets.

9. The decision to evacuate is ordinarily left to the tenant unless physical evidence of a bomb is found.

10. If you determine that your employees and visitors are in imminent danger -- and you cannot reach the Office of the Building by telephone in a reasonable length of time -- you may determine it prudent to exercise your independent judgment and move or evacuate your personnel without being given specific routes to follow.

Should the Building or another tenant receive a bomb threat, you will be advised and informed of any specific details. You may then decide whether you will evacuate.

PHYSICAL EVIDENCE OF A BOMB:

In the event that you discover an item that you suspect to be a bomb, do the following:

1. Call 911 and advise the Chicago City Police Department. Be sure to use the address of 111 West Monroe or 115 South LaSalle Street when speaking with the 911 operator.

2. After phoning the Chicago Police Department, contact the Office of the Building immediately at (312) 357-0100.

3. Do not touch the item in any manner.

4. Do not use radio equipment to transmit messages.

5. Inspect your work area, but do not touch or remove any suspicious objects.

6. If you determine that your employees and visitors are in imminent danger -- and you cannot reach the Office of the Building by telephone in a reasonable length of time -- you may

OTHER EMERGENCIES

Tornadoes or Severe Weather: In most cases, advance warning in the event of a tornado, severe storms or high winds is not likely. If we become aware of impending severe weather, building personnel will monitor the weather and tune into local news cast for additional information. If conditions warrant, building security will notify building tenants via the building Public Address System that severe weather is in the area and for tenants to begin moving into the core areas of the building (i.e., stairwells, restrooms, elevator lobby) taking cover.

Whenever possible, do not enter the lobby or leave the building. The greatest danger will be that of flying glass and objects; therefore, attempt to relocate where the maximum numbers of walls are between you and the exterior of the building. Once weather conditions improve, building security will give all tenants in the building the “all clear” announcement.

Power Failure or Flooding: In the event of a Power Failure or Flooding, the closing of the building may be recommended, depending on the severity of the problem. The building is equipped with a backup generator that will provide emergency lighting and power to one elevator per bank of elevators so tenants can exit the building. If evacuation is necessary, tenants will be given instructions via the buildings Public Address System.

Other Safety Hazards: Please contact the Office of the Building at (312) 357-0100 immediately when any of the following occur:

Storm Damage

Water Leaks

Smoke

Odor or other evidence of escaping natural gas or chemicals

Explosion

Theft/Vandalism
