• CPRIT – Cancer Prevention and Research Institute of Texas

• Established in 2007 by the overwhelming approval of Texas voters

• Goals are: To expedite innovation and commercialization in

the area of cancer research To enhance access to evidence-based prevention

programs and services

Cancer Survivors• More than 14 million in the United States

Testament to early detection and treatment• The Downside,

Increased risk for second primary cancers Increased risk of comorbidities e.g. cardiovascular

disease, diabetes, osteoporosis Increased risk functional decline

• Cancer survivors in rural West Texas often lack access to healthcare services and support groups that can improve their health outcome

WTCSN – Phase 2 “Connecting Cancer Survivors for Better Health”

• WTCSN- Phase 1 ended August 2012• CPRIT moratorium delayed renewal 12/01/2013• Funded for 3 years; ends December 2016• Phase – 2 is an initiative to expand and enroll

West Texas cancer survivors in a network – To provide cancer survivor information, To enhance quality of life To encourage healthy habits To reduce cancer recurrence and prolong survival

We are looking for cancer survivors!

WTCSN Accomplishments• Enrolled 485 enrollees from predominantly rural

areas in two years• Established infrastructure to support recruitment

to the network Expanded Cancer Nutrition Network for Texas (CNNT)

website to accommodate WTCSN• Educational resources & online enrollment form• Website tracking established for evaluation purposes• Qualified for HONCode certification• Expanded Nsights newsletter to enrollees and partners

Activated toll free and fax numbers to facilitate enrollment, questions, and resources

Developed Cancer Nutrition Toolkit for network enrollees

• English and Spanish language versions• Resources from NCI, ACS, AICR, CNNT Passport, DVD, CD

WTCSN-Phase IIRenewal Grant Goals

• Maintain current WTCSN and expand it to the entire TTUHSC Service Area (aka West Texas)

• Design and implement a Cancer Survivorship Care Plan (SCP) to be used by the survivor and their primary care physician (PCP) to monitor his/her post-treatment care

• Develop, pilot test, and implement tailored health communication messages to improve lifestyle behaviors among cancer survivors

West Texas (TTUHSC Service Area)

TTUHSC Health Service Area

• 108 counties; 50% of Texas land mass• Home to over 2.8 million people or 11.2% of the population

of Texas • 98 counties are rural and 49 of those are considered frontier

counties with fewer than 10 people per square mile• Served by eight metropolitan centers: Abilene, Amarillo, El

Paso, Lubbock, Odessa/Midland, San Angelo, and Wichita Falls

• Contributes 81% of the oil (on-shore); 92% of the wind energy; 86% of the cotton; over 88% of the wheat, sunflower seeds, peanuts; 66% of the cattle; and over 60% of the wine-producing grapes, sheep, and corn to the rest of the state

• Failure to distribute cancer nutrition information and develop survivorship services to this area creates a major obstacle in Texas’ effort to diminish cancer mortality

WTCSN-Phase 2 Specific Aims• Re-establish the Network by reaching counterparts of

existing WTCSN partners and developing new partnerships

• Establish a system of “active referral” with providers, community cancer centers, & partners

• Demonstrate knowledge and utilization of SCPs in post-treatment care by Network enrollees

• Establish baseline knowledge & behaviors of Network enrollees in terms of nutrition, diet, physical activity, QoL, and health status

• Demonstrate positive changes in knowledge & behaviors of Network enrollees in terms of nutrition, diet, physical activity, QoL, and health status

WTCSN-Phase IISurvivorship Care Plans

• Significant addition to our educational mission• Key Element in the 2012 Texas Cancer Plan• Recommended by both the American Cancer

Society, the Institute of Medicine, and other advocacy organizations

• Inform the survivor and make them aware of their post-treatment care

• Individualized to the survivor’s type of cancer and stage of survivorship

WTCSN-Phase IITailored Health Messages

• Focused on nutrition/diet, physical activity, and quality of life (QoL)

• Tailored to 4 parameters: Stage of cancer survivorship Type of cancer Whether overweight or obese Level of physical activity

• Significant in that it has never been attempted in a general population of diverse cancer survivors

• Can impact cancer recurrence and survival
