Hello, West Virginia nurses! I hope the increasing sunshine and warmer weather are bringing you the energy and vitality I always associate with this time of year. There is a lot to share, so let’s get to it!

Unity Day 2017April 5, 2017, was Nurses

Unity Day at the West Virginia Legislature, as 300 nurses and nursing students attended the special day at the Capitol. Senate Resolution 68 – sponsored by Senators Rucker, Stollings, Takubo, Plymale, Unger, Boso, and Beach – officially designated the day in recognition of WV nurses. Senators Rucker, Stollings, Takubo, and Beach shared wonderful, supportive sentiments as they spoke to the Resolution. Amy F. Bruce, MSN, RN, NE-BC, is a senior nurse lecturer in the WVU Institute of Technology (“WVU Tech”) Department ofNursing. Ms. Bruce started hernursing career in 1999, when she received a BSN from the University of Charleston, and followed that degree with an MSN in nursing administrationfromMarshallUniversityin2002.She is currently a student in the Doctor of Nursing Practice program at West Virginia University School of Nursing.

Amy Bruce, MSN, RN, NE-BC

Amy’s professional experience includes fifteen years of critical care nursing – incorporating open heart recovery, plastic surgery, and wound care management – as well as education, and now authorship. Her co-authoredarticleentitled“TheUseofTeledermoscopyin the Accurate Identification of Cancerous Skin Lesions in the Adult Population: A Systematic Review” waspublished in the Journal of Telemedicine and Telecare in January of 2017. She presented her poster, “InfluentialFactors Relating to SunProtective Behaviors inAdults,”at the International Conference of Nursing Science and Practice in Dallas, Texas, June 26-28.

Some of Amy’s other notable accomplishments include writing a proclamation signed by then-governor of West Virginia Earl Ray Tomblin for Melanoma SkinCancer Awareness Month in May of 2015, and beingnamed a Jonas Nurse Leader Scholar in fall of 2016.

Amy is newly appointed to the leadership board of the West Virginia office of the Arthritis Foundation aschair of volunteer development, and elected as a West VirginiaLeagueforNursing(WVLN)boardmember.Sheis a member of several organizations, including WVNA, WVLN, and the Future of NursingWest Virginia ActionCoalition Leadership Team. Additionally, she volunteers for the American Cancer Society.

Amy Bruce currently resides in Sissonville with her husband and children.

Unity Day is central to our purpose at WVNA, and we could not hold that event without our sponsors. At the same time, the largest number of attendees at Unity Day are nursing students. Faith Community Nursing Project

Angie Gray, BSN, RN, our Public Health Nurse Leader, is in the final stagesof evaluating the RWJF Project in preparation for her report and graduation inSeptember in Atlanta, GA. The Faith Community Nursing Toolkit and statewidesupport network will continue as part of the current FONWV strategic plan. It isexpected that Angie will continue to lead the Team as the project expands into phase 2. A number of nursing-related bills were proposed in the 2017 West Virginia legislative session. It was a busier than anticipated session, as several of the nursing bills were unexpected, and concerns arose about their possible unintended negative consequences on our profession.

First, let’s focus on thepositive. WVNA championed two bills that passed this year.

House Bill 2282/2219These are the final rules for advanced practice

registered nurses (APRNs) related to the passage ofHouse Bill 4334 in the 2016 legislative session. These rules allow APRNs to have prescriptive authority without a collaborative agreement after three years of prescribing. It also removed several prescriptive restrictions from the prior rule, such as the ability to prescribe Schedule III medications for 30 days with no refill and the removal of the 72-hour limitations on benzodiazepine prescribing, among others. Additionally, for those prescribing in the first three years of practice, APRNs may now collaborate with duly licensed physicians in West Virginia, in any of our contiguous states, or within the Veterans Administration. The final rule became effective 5/15/2017. All APRNs should read the final rule found at http://apps.sos.wv.gov/adlaw/csr/ruleview.aspx?document=10105

House Bill 2522The enhanced nurse licensure compact (enhanced

NLC, or eNLC) bill relates to interstate registered nurse and licensed practical nurse licensure. This law will go into effect the sooner of 26 states passing the enhanced NLC legislation, or December 31, 2018. What the NLC will permit is obtaining a license in one’s home state of residence and then being permitted to practice under that license in any of the states that have passed the enhanced NLC legislation. This will save RNs and LPNs licensure fees, allow for innovative models of nursing care such as telehealth across state lines, and will make it easier to bring RNs and LPNs from other states into West Virginia. As of the writing of this article, there are 23 states that have passed the enhanced NLC, and five states with pending 2017 enhanced NLC legislation. To learn more about the enhanced NLC and

Once again, as was done in 2016 with the APRN bill (HB4334),yourWVNAwashappytoteamupwithAARPand West Virginians for Prosperity to support both of these ultimately successful bills.

Next, let’s discuss the bills of concern that did NOT pass.

House Bill 2345/House Bill 3109House Bill 2345 started as a combination of the

RN and LPN Boards but – in the House GovernmentOrganization Committee – was replaced by an originating bill from that committee that combined the RN, LPN, Medical Imaging and Radiation TherapyTechnology, and Nursing Home Administrators Board. The dilution of professional nursing expertise on this combination board was of significant concern to WVNA. While the committee substitute did pass the House Government Organization Committee, thanks to theengagement in contacting legislators to oppose this bill, it was halted when it was placed on the House Calendar, and never saw a full House of Delegates vote.

House Bill 2504This bill would have prohibited the state nursing

boards from limiting the number of students in nursing programs. Currently, the BON can and does set a maximum student-to-faculty ratio. If passed, the individual schools would have the authority to set that ratio instead. Leaders of nursing education and regulation in West Virginia were concerned that this deregulation could negatively impact nursing education and patient safety. This bill was not placed on the House Health and Human Services agenda, which was the committee of first reference, and thus did not advance.

House Bill 2667This bill related to the approval and accreditation

of nursing schools. As originally written, it would have exempted nursing programs from maintaining Board of Nursing (BON) approval if they were accredited bythe Accreditation Commission for Education in Nursing(ACEN) or the Commission on Collegiate NursingEducation (CCNE). Nursing regulators and a numberof leaders in nursing education in the state expressed concern over this bill and the impact on the quality of nursing education and the agility of the accrediting bodies to act on programs whose outcomes were declining. The bill was amended to permit the BON approval exemption only if schools had been accredited byACENorCCNEfortwoaccreditationcycles,andaddeda clause allowing the board to assist nursing students in filing complaints to a national accrediting body for those programs exempted from board accreditation. This

bill passed the House Health and Human Resources Committee, but was not placed on the agenda of subsequent House committees referenced, and thus did not advance.

Finally, there are two interesting resolutions thatpassed, both proposing to study specific nursing issues during the upcoming Interim legislative session.

House Concurrent Resolution 115 requests the JointCommitteeonGovernmentandFinancetostudycombining the Board of Examiners for RegisteredProfessional Nurses and the Board of Examinersfor Licensed Practical Nurses, including examining the benefits of merging the boards to facilitate participation in national compacts, the cost savings of combining the boards, and the overlap of occupations licensed and representation on the boards.

House Concurrent Resolution 138 requests the JointCommitteeonGovernmentandFinancetostudythe nursing shortage in West Virginia. At the time of this writing, the Legislature has not selected the issues they intend to study. The WVNA will keep you posted on the status of these studies.

Nursing Workforce InitiativeAs evidenced by the Study Resolution on the

nursing shortage, the legislature is particularly interested in West Virginia’s nursing workforce, as are all health care stakeholders throughout the state. The Future of Nursing WV (of which WVNA is one ofthe triad, sponsors along with the WV Organization of Nurse Executives and the WV Hospital Association) isplanning a summit on September 15, 2017, focused on addressing this important and challenging problem. (See the Future of Nursing WV update on page 6.)The issues, problems, and gaps that contribute to an apparent nursing shortage as identified by practicing nurses, nurse educators, nurse employers, and policy makers will be examined related to both nursing recruitment and to nursing retention. This is a complex issue that has many confounding factors and consequences unfavorable to nurses, health care systems, and communities.

During the neonatal period, preterm infants are at risk for illness such as chronic lung disease, severe brain injury, retinopathy of prematurity, necrotizing enterocolitis, and neonatal sepsis. Children who were preterm infants are at risk for motor and sensory impairment, learning difficulties, and behavioral problems. The average daily cost for NICU care is $3,000. When comparing the average cost for a term infant’s care – $2,830 – to that of a preterm infant – $41,610 – the difference is obvious. These cost differences persist during the first five years, if not for the lifetime of the child.

While the U.S. is a developed country, it has an above-averagepretermbirth rate.Data fromthe2016MarchofDimes Report Card indicate that the U.S. preterm birth rate has dropped over recent years to 9.6%, while the preterm birth rate in West Virginia is above that average, at11.3%,earningourstatea“D”gradeforthe2016report.Reducing total preterm births to 8.1% by 2020 is one of the Leading Health Indicators of the Healthy People 2020 goals, a designation to indicate high-priority initiatives (www.marchofdimes.org). With these numbers and goals inmind,theWestVirginiaPerinatalPartnership,theMarchof Dimes, and the WV Department of Health and Human Resources have developed the “PretermBirth ReductionProject”todecreaseWestVirginia’spretermbirthrateandto achieve the national goal.

PTB Reduction PlanThe West Virginia Perinatal Partnership’s Preterm

Birth Reduction Project has several strategies aimed at reducing PTB:

1) Provider education and awareness regarding tobacco cessation and progesterone therapy

2) Providing implementation strategies

3) Removing barriers to care

4) Assessing the impact of strategy implementation

This project will be targeting primary care providers who deliver prenatal care, such as those who practice in rural health clinics, family physicians and nurse-midwives, obstetrics providers and nurses, and radiology providers. There are two teams of two nurses who are visiting provider offices and hospitals, bringing current information and aiding in removing barriers to care. Patient brochures and posters for the offices will be dispersed.

Preconception counseling can empower motivated patients to improve pregnancy outcomes. Counseling on nutrition, recommended spacing of children, prepregnancy weight gain or loss, tobacco use, drug cessation, and maternal stress management can be offered to mitigate risk prior to pregnancy.

Tobacco CessationWest Virginia has the highest rate in the U.S. of smoking

during pregnancy. Maternal smoking during pregnancyranges from 1.8 percent in California to 27.1 percent in West Virginia,accordingtotheCDC(2014).Provisionalnumbersfrom DHHR’s Health Statistics Center indicate the rate of smoking during pregnancy in West Virginia has dropped to 24.2 percent in 2016, a significant improvement but still ahead of the national rate of 8.4 percent.

Adverse outcomes linked with tobacco use, such as prematurity and low birth weight, as well as fetal and infantmortality(suddenunexpectedinfantdeath,SUID),have been well researched. Maternal morbidity andmortality has been shown to be elevated in women who smoke during pregnancy. The Perinatal Partnership has initiated evidence-based tobacco cessation outreach education programs targeted at health care professionals who want to learn motivational interviewing and other tools to help patients quit smoking. Programs such as “Help2Quit”assistmothersandtheirfamilies incuttingbackorquitting. The recent launchof theManagementof Maternal Smoking (MOMS) Initiative includesrepresentatives from DHHR programs, the Perinatal Partnership, CDC, Substance Abuse and Mental HealthServices Administration (SAMSHA), U.S. Department ofHealthandHumanServices,andtheCentersforMedicareandMedicaidServices.

Long Acting Reversible Contraception (LARC)Recently the Partnership has been providing outreach

education aimed at reducing unintended pregnancy by advocating for the use of LARCs, a highly effective method of family planning. Insertion of the intrauterine device or the intradermal implant can be initiated during the teen years, or any time when pregnancy would not be desired. Often incarcerated women and those in day report centers are eager to have this long-lasting method of birth control.

LARCs can also be inserted immediately postpartum, in the case of an IUD, or before the mother leaves the hospital, in the case of an implant in the upper arm. This strategy has been shown to decrease preterm birth, as it lengthens the interval between pregnancies. The barriers to obtaining LARCs are well documented, and include physician lack of knowledge, financial constraints, and logistical issues such as obtaining the LARCs in the labor and delivery unit or freestanding birth center, and payor issues. The Partnership, WV Public Employee InsuranceAgency (PEIA), and Medicaid payors have workedtogether to unbundle the charges from delivery, with the goal of providing LARCs at a convenient time to reduce unintended pregnancies.

Recurrent Preterm BirthIn 2012, the American Congress of Obstetricians and

Gynecologists (ACOG) and the Society forMaternal andFetal Medicine (SMFM) issued updated clinical practiceguidelines for PTB risk screening and progesterone use to prevent recurrent PTB. These practice bulletins strongly recommend using synthetic 17a-hydroxyprogesterone caproate (17P) supplementation for singletonpregnancies in women who have experienced previous spontaneous PTB, and vaginal progesterone supplementation for singleton pregnancies diagnosed mid-pregnancy with a short cervical length and with no history of spontaneous PTB. It is estimated that administering 17P (trade name Makena®) to an eligiblewoman will reduce her chances of delivering prematurely by33percent.Eachyearalmost30,000pregnantwomenare considered eligible for 17P.

Recently published studies have suggested that maternity care providers may not be aware of these recommendations. There may also be barriers with obtaining the weekly injections, or the patient may have transportation issues, live in a rural area, or may not desire weekly intramuscular injections. Nurses in the provider offices, hospitals, and community may need to counsel those at-risk patients and their families about the benefits of progesterone treatment, strategizing creative ways for the woman to receive weekly injections near home in a pharmacy, by a home health nurse, or in a nearby community clinic. Nurses administering pregnancy tests, or those working in neonatal intensive care units, or with women who have just delivered before 37 weeks spontaneously, can educate patients about the availability and effectiveness of 17P and urge them to set up their first prenatal appointment early, advising their midwife or physician of a previous preterm birth.

Early access to initial prenatal appointments isimportant, since insurers require a preauthorization for Makena – a very costly drug, as much as $695 perinjection, although insurers often negotiate price. Before commercial availability, 17P was available only through compounding pharmacies. Due to an FDA ruling, “apharmacist may not compound regularly or in inordinate amounts any drug products that are essentially copies of Makena.” The Amag pharmaceutical company doesprovide some clientswithMakena orwith part of theirMakena copay if they are eligible for the “Makena CareConnection”program.

Shortened Cervical LengthTransvaginal cervical length (TVCL) measurement

between 16 and 24 weeks gestation can be utilized to monitor risk in women with a history of prior preterm birth as well as to identify women without a history of preterm birth who are at increased risk for preterm birth during the current pregnancy. Transvaginal measurements are considered more accurate than transabdominal scans because of better consistency and are unaffected by maternal weight, position of the cervix, obesity, and fetal position. TVCL screening strategy in women without a prior preterm birth may be considered, since daily vaginal progesterone supplementation has been shown to lower the risk of preterm birth in women with a very short cervical length, considered to be below 2.5 cm as measured upon transvaginal ultrasound.

The Preterm Birth Reduction Project plans to facilitate educational opportunities for providers and staff on current best practices in tobacco cessation, LARCs, and the indication and prescription of the two types of progesterone supplementation.

Hospitals in West Virginia are working to meet the needs of patients who have been sexually assaulted. Ideally, every county in West Virginia should have an accessible and responsive service delivery system that includes providing medical forensic exams by trained medical providers.

It is essential that all patients who present to medical facilities with reports of sexual assault be thoroughly evaluated. Treating injuries alone is not sufficient in these cases.Medicalpersonnelor SexualAssaultNurseExaminers (SANEs) should be trained and clinicallyprepared to provide comprehensive forensic evaluation while maintaining chain of custody using the WV Sexual AssaultEvidenceCollectionKit(SAECK).

In sexual assault cases, the medical facility should immediately call a victim advocate from the nearest rape crisis center. The importance of having a victim advocate from the local rape crisis center available to a sexual assault patient cannot be overemphasized. Research has shown that, when advocates are called to the hospital and can explain their role, in most cases, patients accept advocacy services. Advocates are specially trained to

Sexual Assault Nurse Examiner (SANE) Programprovide patients with free, confidential, non-judgmental support, information, and resources, so patients can make informed decisions about their care following a sexual assault. The immediate involvement of victim advocates at the hospital is crucial to the recovery of many sexual assault patients. When consent has been given, advocates stay with the patient throughout their time at the medical facility.

If the patient decides not to accept advocacy services, the medical provider or SANE should provide contactinformation for the local rape crisis center for future reference.

The policies for conducting medical forensic examinations by the SANE or other trained medicalproviders should be written in hospital protocols and available in the emergency department procedure manuals.

Once the kit is collected, it will be turned over to law enforcement if the patient has signed a consent form releasing the evidence to law enforcement.

If the patient chooses to have a medical forensic examination but does not want to initiate or participate in any investigation with law enforcement, the medical provider or SANE must have the patient sign andcomplete the consent form for a “non-report to law

enforcement.” The kit is sent to Marshall UniversityForensic Science Center (MUFSC), where the forensicsamples will be stored for 24 months. It is important to note that if liquid samples were collected as a part of the toxicology kit (i.e., bloodorurine), the sampleswillhave a limited life span and will degrade over time. All forensic samples collected as part of the medical forensic examination(swabsorsmears)willhaveanunlimitedlifespan if collected and dried properly.

Should the victim later decide to initiate an investigation, they may contact law enforcement and provide the kit tracking number. Law enforcement would request the kit from MUFSC by submitting a“RequesttoTransfer”form.

NOTE:There isno statute of limitations on reporting sexual assault in West Virginia.

Opioid abuse is devastating our state, and too often it starts with something as seemingly harmless as the prescription of an opioid-based pain medication.

Powerful painkillers, such as oxycodone or hydrocodone, are prescribed to provide temporary relief, but do nothing to address the source of pain.

Long-term prescription opioid use creates a tremendous problem where patients develop a tolerance and dependence for pills that share characteristics similar to heroin. This ultimately turns the search for temporary relief into addiction, accidental overdose, coma, and death.

West Virginia tops the list for overdose deaths in the nation, and this cycle must change.Theanswerlieswithpatientsandprescriberschoosingsafer,non-opioid(non-narcotic) alternatives.

That’s why my office created the first substance abuse fighting unit by an Attorney General in West Virginia. It promotes a focused effort on enforcement, preventionthrough education, and the pursuit of alternative treatments for injury or pain.

The process must begin with a thorough physical examination and the prescriber considering every available alternative to opioid medication. The price of anything less is potential addiction, overdose, and death.

Patients must feel empowered to ask for non-opioid based medication and treatment. These options include physical and occupational therapy, chiropractic care, osteopathic manipulation, massage therapy, acupuncture, and over-the-counter medication.

Such a strategy follows our office’s best practices for prescribing and dispensing opioid drugs. This initiative garnered support from more than 25 national and state stakeholders. It also laid the foundation to cut prescription opioid use by more than 25 percent.

But the key to achieving that goal is use of non-opioid alternatives. Highly addictive pills can no longer be the first choice in treating aches and pain.

Physical and occupational therapy, non-medication approaches to physical rehabilitation and improving functionality, offer an interdisciplinary approach to pain management that focus on physical, social, and emotional health. In some cases, these techniques have been shown to improve a patient’s overall health by assessing performance issues in everyday living.

Chiropractic care and osteopathic manipulation therapy have been shown to be as effective as exercise therapy or standard medical care for pain, particularly for lower back pain. This type of treatment also improves functionality in the case of injury.

Massage therapy has proven effective in the short-term management of pain,particularly pain isolated to one area of the body, such as the lower back or neck. One clinical study found few adverse effects resulting from massage as a pain management technique.

Acupuncture offers short-term comfort as well as increased functionality of the injured body part, according to some studies. One study found patients undergoing total joint replacements reported feeling a 45 percent reduction in pain after receiving acupuncture treatments.

Over-the-counter pain medications are shown often to be more effective in treating pain than medications that can lead to addiction or death. Acetaminophen, ibuprofen, or a combination of these two readily available pain relievers, can reduce pain without the risk of addiction and can be prescribed in larger doses as needed.

These non-opioid alternatives provide West Virginians a much safer option. They also cut down on the selling or sharing of pills, and prevent medications from accidentally falling into the wrong hands – such as those of a child.

Our goal is to reduce misuse of prescription medication while preserving legitimate patient access to necessary opioid treatment, such as during active cancer treatments or palliative and end-of-life care.

Myofficewillnotrelentinworkingtowardahealthier,drug-freeWestVirginia.Our state has a great deal of potential, but drugs decimate our workforce, shatter

ambitions born in adolescence, rip families apart, and leave too many to mourn the passing of those who overdosed one too many times.

With caution, education – and a sensible approach to pain management – I have faith that West Virginia can curb its opioid epidemic and we can build a safer, brighter future for our state.

Painkiller Alternatives May Reduce Risk of Prescription Opioid Abuse

Each boxon the site represents community projects in each county designed to inspire behavior change and healthier options. There are farmer’s markets, bike trails, walking clubs, projectsteachingkidstogarden,andmuchmoretoinspireeffortstogetWVoffthe“worsthealth”lists.Communityteamscanapplyformini-grantstogetaprojectstartedandthenleverage that money to expand their efforts.

Over 300 people came together June 2-3 at WV Wesleyan College in Buckhannon for the fourth annual Try This Conference, and nearly 150 health professionals attended the firstTryThis:ForHealthcareProfessionalsthedaybeforetheconferencestarted.Speakersoffered practical strategies for engaging patients and communities in movement, healthful food, and stress management. Hearing what is happening in health practices across the state was inspiring and motivating, since everyone shared how they implemented changes and offered support to others to do the same.

Thehealthprofessionals’daywasacollaborationbetweentheWVUSchoolofMedicineEasternDivision, Try ThisWest Virginia, The FutureofNursingWV, theWVAcademyofNutrition and Dietetics, and other WV providers, institutions, and community leaders. Health professionals shared evidence-based strategies for identifying, preventing, and reversing obesity and diabetes, and dietary approaches for opioid dependence.

FollowinganintroductionbyKateLong(co-directorofTryThis),ClayMarsh,MD,vicepresident & executive dean for health sciences at WVU, inspired the audience to think

abouthealthinanewway.GaryTaubes,scientificjournalist,andauthorofThe Case Against Sugar (2016),challengedcurrentbeliefsabout“whywegetfat”and“whattodoaboutit.”

Participants heard about innovative strategies like the Wheeling FARMacy, offeredthroughHealthRightWheeling–afreeclinic–andGrowOhioValley–afoodadvocacygroup. In this program, patients are given a prescription for healthy food. as well as a coupon to purchase fresh fruits and vegetables at the local farmer’s market.

C. Donovan “Dino” Beckett, DO, named by the National Rural Health Association(NRHA)astheir2017RuralHealthPractitioneroftheYear,sharedhowtheMingoCountycommunity of Williamson mobilized to implement several projects to improve the health and well-being of its residents.

Other conference topics were how to engage kids in healthy activity, motivational interviewing, and stress-busting techniques.

Nurses, physicians, anddieticians received sixhoursof continuingeducation (CE) forthe health professionals’ day and had an opportunity to receive six more for qualified workshops during the next two days at the Try This Conference.

Try This WV is a movement that can make all West Virginians proud and hopeful about the future of building a culture of health in our state.
