Instructions for Completing this Form
1. Use the HIRAC Planning Tool to plan the risk assessment process, if required. The Developing Safe Systems of Work Guidance Document is also available for assistance.
2. This form is designed to be used as a general risk assessment for an area, task or activity. This assessment can trigger the requirement to perform other specific risk assessments.
3. The Risk Assessment must be developed with a minimum of 3 people to ensure an accurate risk rating. All persons involved must detail their names on page 1 of this form.
4. All risks with a high risk rating must have immediate interim control measures identified. Risks with a high risk rating after controls are implemented shall be escalated to the Executive
Manager.
5. Control measures that require ongoing implementation must have a specific due date and “ongoing” in the due date column.
6. The Manager must ensure that all control measures are assigned to the person(s) responsible, and implemented by the due date.
7. Collect all relevant signatures on the form prior to saving the scanned document in Trim. Update the HIRAC Register accordingly.
Hierarchy of Controls
Control measures can be ranked from the highest level of protection and reliability, to the lowest. Elimination and the other higher order controls must always be considered first. A combination of control measures may be implemented to control a risk, although consider if any new risks are introduced as a result.

Proposed activities the children will participate in: Trash Percussion 10.00am to 11.30am
Trash Percussion is an exciting, vibrant and all-inclusive style of percussion designed with the view that anything and everything can be a musical instrument. Utilising the philosophy that music is a combination of sound and silence, and that everyone can play music, these incursion workshops are like no others as they put the children in the driver’s seat, empowering them with a unique approach to music. Children are encouraged to actively participate in working together as a team in composing what will result in a truly original piece of music on non-traditional instruments.
