    EXPLANATORY NOTES

    APPLICATION FOR PERMANENT RESIDENCE FORSPOUSE AND/OR CHILDREN OF A SINGAPORE CITIZEN/

    PERMANENT RESIDENT

    Who may apply

    1. Spouse and unmarried children (below 21 years old) ofa Singapore Citizen/ Permanent Resident.

    Application Form (Form 4)

    2. Application Form 4 is to be completed (1 original copy).

    3. Applicant is required to complete Part A to Part E andPart G. Local sponsor to complete Part F and Part H ofthe application form.

    Photograph Requirement

    4. A recent colour passport-sized photograph of the mainapplicant is to be pasted onto the space provided on

    page 1. Photographs of children included in theapplication are to be pasted onto the spaces providedon page 6.

    5 The photograph must be:

    35 mm wide by 45 mm high without border andtaken within the last three months;

    taken full face without headgear, unless theapplicant habitually wears a headgear inaccordance with religious or racial customs. Theheadgear must not hide the applicant's facialfeatures;

    the facial image must be between 25 mm and 35mm from chin to crown; and

    taken against plain white background with a matt or

    semi-matt finish.

    Documents to be produced

    6. The originals and a photocopy of each of the followingitems are required. Official translations are requiredif they are not in English. Original documents willbe returned immediately after sighting.

    (a) Valid travel document of applicant. Photocopies ofpages showing personal particulars, officialdescriptions and Singapore immigrationendorsements are required.

    (b) Work Pass of applicant, if any.

    (c) Identity cards of both applicant (if any) andsponsor.

    (d) Applicants Birth Certificate and householdCensus List.

    (e) Official Marriage Certificate of sponsor

    (f) Educational certificates of both applicant andsponsor.

    (g) Official Birth Certificates of children showing bothparents names and childs name and adoptionpaper, if applicable.

    (h) Death certificate or divorce certificate and thecustody papers for the children in respect ofapplicants/spouses previous marriages (if any).

    (i) Employers letter from sponsor and applicant (ifapplicable) stating the date of employment,position held, salary per month for the last 6months with breakdown into basic, overtime andallowance per month (see item 7 below). Theletter, addressed to the Controller of Immigration,should be dated within 1 month from the date ofsubmission.

    (j) Income Tax Notices of Assessment* of sponsorand applicant (if applicable) for the last 3 years.

    (k) Statement* from the CPF Board showing thesponsors monthly CPF contribution history forthe last 12 months.

    [Note: *Alternatively for (j) and (k) above, sponsorand applicant may choose to complete Appendix

    2 (PR-FTS) to give consent for ICA to obtainand verify financial information provided in respectof this application with the Inland Revenue

    Authority of Singapore and Central ProvidentFund Board of Singapore directly instead.]

    (l) Sponsors valid Business Registration Certificate(for self employment) with names of partnersshown and Valid Vocational licence, if applicable.

    (m) Deed Poll or Change of Name Certificate ofapplicant and sponsor.

    Basic Salary

    7. When completing the application form, please note thatbasic salary does not include the following:-

    (a) Additional payment by way of overtime, bonusor commission.

    (b) Travel, food or housing allowance.

    (c) Any sum paid to an employee or contract workerto defray special expenses incurred by himowing to the special, unusual or hazardousnature of the employment.

    (d) Any contribution paid by employer orcontractor to any pensions fund or providentfund.

    (e) Any gratuity payable on discharge or retirement.

    Submission

    8. Please note that the submission of PR applicationis by appointment only. To make an appointment,please visit our website at http://www.ica.gov.sg. Onthe date of your appointment, please arrive punctually

    to submit your application together with supportingdocuments personally to:

    9. All information contained in the form must be complete,accurate and verifiable.

    10. Please submit all the required documents together atthe time of application. Incomplete submission mayresult in non-acceptance of the application at thecounter.

    11. Submission of application by post will not beaccepted.

    12. Applicant and sponsor may be required to furnishadditional documents and information whenevernecessary.

    National Service Liability

    13. Under the Enlistment Act, all male Singapore Citizensand Permanent Residents, unless exempted are liableto register for National Service (NS). Following thecompletion of full-time NS, they will be required toserve up to 40 days of Operationally Ready National

    Service (ORNS) per year for the duration of theirORNS training cycle till the age of 50 years (forofficers) or 40 years (for other ranks).

    14. Main applicants who are granted PR status under theProfessionals /Technical Personnel and SkilledWorkers (PTS) Scheme or the Investor Scheme areexempted from NS. Male children who are granted PRstatus under their parents' sponsorship are liable forNS under the Enlistment Act. They are required toregister for NS upon reaching 16

    years old and will

    be scheduled for enlistment at the earliest opportunityupon reaching 18 years old. If they are pursuing full-time studies, deferment from full-time NS may begranted only if they meet the deferment conditions

    stipulated by the Ministry of Defence which can befound at http://www.ns.sg.

    All NS-liable males aged 13 years old and above, arerequired to obtain an Exit Permit (EP) when travellingoverseas for 3 months or more.

    15. NS-liable PRs are expected to serve NS. Renouncingor losing one's PR status without serving or completingfull-time NS would have an adverse impact on anyimmediate or future applications to work or study inSingapore, or for Singapore citizenship or PR status.

    16. Males who are granted Singapore PR, and who werepreviously Singapore Citizens or Singapore PermanentResidents, are liable to be called up for NS regardlessof the type of PR status they have been granted. Forfurther enquiries, please write to:-

    Important Notes

    17. The grant of permanent residence does not exempt anindividual from registration with the relevantprofessional bodies or any other statutory conditions asrequired by legislation governing the regulation of theirprofessions (e.g. doctors, lawyers, etc.).

    18. Ex-Singapore Citizens and Ex-Singapore PRs whohave withdrawn their CPF monies would have to refundthe full CPF amount withdrawn if their PR application isapproved. A letter of clearance from CPF Board is

    19. All correspondences will be sent to the sponsorsresidential address as registered in the sponsorsSingapore identity card.

    20. ICA may share your personal information with otherGovernment agencies to process any applications youhave made or to render you a service, so as to serve

    you in an efficient and effective way, unless suchsharing is prohibited by legislation.

    Warning

    21. If you knowingly and willfully falsify or conceal amaterial fact or submit a false document in relation toany application, action will be instituted against youand you will be denied all immigration facilities. Inaddition, you will face severe penalties provided by thelaw.
