The BYO plans allow you to pick the plan that is right for you. The Smart Choice plan option (detailed below) offers low premiums, low office visit copays, and is worth considering and comparing with the other plans to see if it’s right for you. The Smart Choice plan features a $15 PCP/$50 specialist copay, $1,000 individual/$3,000 family deductible with 80% coinsurance. The out-of-pocket maximum is $2,000 individual/$4,000 family.*Includes Engineers, Maintenance Instructors, Flight Instructors and Chelsea Food Services employees.

Step #1Choose a PCP n You can choose any PCP from the

Aetna Select network by visitingDocFind at www.aetna.com/united. Your PCP is the doctor you go to first.He or she will help you learn aboutyour health and how to manage it.

n Choosing a doctor is a personal decision — that’s why each memberof your family can have his or herown PCP.

n You can change your PCP anytime. Just call Aetna Member Services at 1-800-334-0110, or log in to theAetna Navigator® secure member website at www.aetna.com/united.

Step #3Pay your copay n After you visit your doctor, you will

have to pay a copay. This is a flat dollar amount or a percentage ofyour bill, depending on the design of your health benefit plan. Copaysvary by location.

n Through Your Benefits Resources,which can be accessed by going to Flying Together > EmployeeServices > Benefits, you can findthe covered services and copayments for medical, mental health/substanceabuse and pharmacy benefits covered by Aetna. Step #2Visit your PCP for caren Go to your PCP for checkups or

whenever you are sick or hurt.n Your PCP will help you decide

if you need care from another doctor. If so, your PCP will give you a referral.**

n Sometimes you may need care that requires approval from Aetnabefore your visit. Your PCP andother network doctors will ask for this approval for you.

The Aetna DMO plan provides generous coverage for preventive services that keep your teeth healthyand catch problems early. You’llselect a primary care dentist (PCD)from the Aetna network for yourselfand each covered family member.Your PCD will provide most of yourdental care and referrals to specialistswhen you need them.

In an EmergencyThe plan provides coverage for emergency dental services (such as pain relief) both in and out of theAetna network. You should always contact your PCDfirst for treatment. If you are treated by a non-networkdentist while more than 50 miles from home, the planwill cover emergency services up to $100, subject tostate requirements.
