ORDER

Administrative approval and financial sanction is hereby accorded for 'Infrastructure

Strengthening of 11 (Eleven) nos. of Sub-Centres to be upgraded into Su-Swasthya

Kendras (Health &::: Wellness Centre) at Birbhum District' at an estimated cost of

Rs. 84,87,680.00 (Rupees Eighty Four Lakh Eighty Seven Thousand Six Hundred & Eighty)

2. The work will be executed by the Engineering wing of District Health &::: Family

Welfare Samity, Birbhum in conformity with the norms prescribed in Finance Department

G.O. No. 5400-F(Y) Dated. 25/06/2012 read with G.O No. 3060-F(Y) Dated. 11/07/2014.

3. Fund will be released after submission of requisition of fund along with the statement

of work awarded cost for each facility in favour of District Health &::: Family Welfare Samity,

Birbhum as the implementing agency.

4. The released amount shall be treated as a pool fund for the said activity.

S. The expenditure shall be booked in the monthly FI\IR &: will be submitted to this end

in due course.

6. No price escalation will be allowed in any circumstances.

7. No other work will be taken up or implemented other than the actual sanctioned

scheme

R\... The awarded work will be completed within ~ (T \\'0) months from the date of issuance

of order.

9. State Tearn will make periodical onsite 'isits to review the progress of the work &:

check the quality of work being implemented. No. paYlnents will be made until the clearance

on the quality of the work has been permitted Irorn this end.

10. The charge is debitable Irom the NHM Flexi-pool 2019- 2020 under the head of

Accounts "Infrastructure Strengthening of SCs to HWCs" [Tally AIC Head -

B.04.01.07.01]

11. Senior Accounts Officer, National Health Mission will be the drawing and

disbursing officer in this respect after submission of requisition of funcl by DHF\\'S, Birbhurn.

13. Anv change in the narne of the notified Sub Centres should be immcdiatelv informed; '- ;

to SPI\1U for issuing necessary amendments.
