• Document indicating child’s age

• Immunization Records• Kindergarten Health Assessment

• Proof of Residence (Two of the following): Current utility bill

Current property tax statement

Current gas bill

Current water bill Rental/Lease Agreement that shall include the name of the legal custodian of the child

Copy of deed to home that shall include the name of the legal custodian of the child

A Non-Parental Affidavit of Residence that has been notarized, signed, and approved by the Assistant Superintendent or Executive Director of School Administration.

(Must be on “PINK” State Kindergarten Health Assessment Form)
