The goods and services shown below are those we can provide to our customers. You may choose only the items you

desire. However, any funeral arrangements you select will include a charge for our basic service and overhead. If legal or

other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the

statement we provide describing the funeral goods and services you selected.

PERSONALIZED SERVICE OPTIONS To make funeral services arrangements easier for the families we serve, we have created special funeral service

packages of the services we most frequently render. You are free, however, to make funeral service arrangements on

an itemized basis if you so desire.

• Our special funeral service packages are offered on a cost saving basis that is available to those who purchase a

casket from this mortuary. (Additional charges apply for Saturday, Sunday, or Holiday Services.)

ITEMIZED SELECTIONS

PROFESSIONAL SERVICES, FACILITIES AND EQUIPMENT:

Our fee for the basic services of funeral director and staff includes, but is not limited to, staff to respond to initial

request for service; arrangement conference with family and responsible parties; arrangement for funeral; preparation

and filing of necessary authorizations and permits; recording vital statistics; preparation and placement of obituary

notice; shelter of deceased; and coordination with those providing other portions of the funeral, e.g., cemetery,

crematory, vault companies and others as required. Also included in this charge are overhead expenses relative to our

facilities such as professional licensing, legal and accounting fees, insurance, building and utility expenses, parking lot

and grounds, maintenance, taxes, equipment, furnishings, inventory costs, recordkeeping, and secretarial and

administrative expenses. This fee for our basic services and overhead will be added to the total cost of the funeral

arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials and

Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as immediate burial or direct cremation. The Utah State Vital Statistics Act requires that embalming, refrigeration, cremation or burial take place within the first 24 hrs.

ABOUT OUR TERMS OF PAYMENT

In order for us to provide the highest standard of personalized and compassionate service to the families we serve; we request payment in full one day prior to services.

We are a small business. We operate on a cash basis, relying upon the families we serve for prompt payment. We are not able to extend credit. Utah State law does not allow us to fulfill the responsibilities of a bank or other lending institution.

Please have an open discussion of these matters with your funeral director. We want to assist you in any way possible.

PAYMENT OPTIONS

• Payment may be made by cash, credit card or check.

• Pre-paid funeral plans with Myers Mortuary or any mortuary, subject to verification that the policy is in force and valid.

• Assignment of death benefits of a verified insurance policy.

We accept assignments of death benefits from an insurance policy with the complete cooperation of the beneficiary and the insurance company. Myers Mortuary will verify that the Insurance Company will accept the assignment of death benefits and that the Insurance Company will cooperate in providing needed information to the beneficiary and to the mortuary. The Insurance Company will pay Myers Mortuary the assigned amount of the death benefit to pay for the funeral services. The balance of the insurance death benefit will be paid to the beneficiary.

Documentation and Service Fee $150

** Myers Mortuary is unable to accept payment from estates or from a trust, due to the complexity and time required for payment.
