Activities or programmes that focus on issues related to inequality of health from the aspects of sex, age, ethnicity, disability, socio-economic status and other issues related to health.

Activities or programmes that can improve environmental health by creating a healthy school, the workplace, village/mukim, market or in certain communities as well as activities or programmes to form partnerships among certain groups or organisations.

Promotion of health through sports and cultural activities
Activities or programmes previously financed or sponsored by tobacco or alcohol companies.
Evidence-based activities or programmes in line with the objectives of the Board.

Organisations or associated members that receive any form of financial aid from any tobacco or alcohol companies or their agents.
Applications from individuals, profit-making groups, private consultants or organisations/bodies.
The main objective of the activities is to collect funds or awards/gifts giving.

Procurement of goods or main equipment such as vehicles, buildings, medical equipment or computers.
To fund any abroad expenses, participation in conferences or trade exhibition.

Activities that support any political organisation at any level.
Any activities deemed to become core activities of government agencies or which are deemed as the responsibility of federal, state, district governments or local authorities.

Any activities related to treatment, rehabilitation and therapy projects or activities.
Activities or programmes that are not within the Board’s priority areas and objectives.

• Promoting healthy lifestyles through health related organisation, sports and cultural.
• Research in health promotion.
TYPES OF PROJECTS

The project shall commence within one month of receiving the first instalment of the said grant.
The project must be completed within the stipulated period.
Application to extend the duration of the project must be submitted to the board at least three (3) months before the project completion date. The latest progress report and expenditure statement must be included in the application.

The project leader shall be responsible for the entire project. He is to submit a written progress report to the board according to the schedule fixed by the Board.
The Board may audit approved projects if and when necessary.
The applicant shall carry out project evaluation and submit the report within 2 (two) months after the completion of the project.

Subsequent instalments of the grant will be disbursed when the Board is satisfied with the progress of the project. The last instalment will only be disbursed after the Board receives the project report which includes evaluation report and certified financial statement.

•Utilization the grant for approved purposes only.
• To adopt a smoke-free policy in the implementation of the project.
• To give due recognition to the board in any forms either verbally or through printed materials in all events or activities financed by the Board
The applicant shall fulfil the following requirements:

• To obtain prior approval from the board of any changes or amendment to the approved project.
• To return any unused allocation in the case of breach or cancellation of contract.
• To invite officers/representatives from the Board to attend activities related to the project.
The applicant shall fulfil the following requirements:

• In the event of false information given in the application form.
• The project or activity is not carried out in accordance to the agreed proposal.
The Health Promotion Board has the right to terminate the grant if:

• Application forms were incomplete, such as;• Objective(s) of the project not stated clearly.• Location and target groups were not
identified.• List of activities and detail of expenditure not
given.
• The amount of grant required was too high as compared to the activities, i.e. RM2 million for4 medical camps a year for 2 years.
• Unregistered association.
REASONS FOR TENTATIVE REJECTION

• One-off projects, such as one/two-day event.• Large portion of the budget is to purchase
equipments or food and refreshment.
• Grants are to fund activities, such as rehabilitation and treatment, which is part or core activities and responsibility of MoH.
• Scope of projects are not within the priority areas of the Board.
REASONS FOR TENTATIVE REJECTION

• Application for the grant shall be using the specified official form.
• Applications may be sent by fax or e-mail but the original copy must be submitted by post or by hand. Supporting documents, VCDs, CDs, pictures or any other forms of relevant documents, if any, must be attached together with the application form.
• The Board is not responsible for the loss of an application form or any supporting document sent through the post.
