Benefitting to the time and topic of everything and industrial professional's concern, a bla seminar on "Fire-Safety Management in Industrial Establishments & High-Rise Building was organised by our Maharashtra Chapter. The Seminar was inaugurated by Shri V. B. Sant the Director General of National Safety Council, India. Sarvashri A. P. Gokhale, Dy. G.M. HPCL-Mumbai Refinery, S. J. Lele, Ch. E. (Fire & Safety) - RCF Ltd, Thal, D. S. Lote, Dy. G.M. (SHE)-Mahanagar Gas Ltd, Sunil Nesrikar, Div. Fire Officer-Chembur Fire Station, MCGM, were the eminent and expert faculties to present papers. Shri A. P. Gokhale who made presentation on 'Tank-Farm Management' stressed upon the necessity of a scheduled inspection visit to Tank-Farm with a suitable and specific check-list. The check-list must include all the related points to be inspected and be replied and filled very religeously. He displayed various model check-list on screen and made his presentation imperative. Shri S. J. Lele presented his paper on 'Fire Hazard in Chemical Storage'. He stressed not only fire-dangerous properties but also all other properties directly or indirectly affecting the safety of storage should also be taken into

account. He elaborated his presentation with the types of fires, types of fire suppression systems for various types of storage tanks and statutory requirements. Shri D. S. Lote presented his paper on safety in Gas supply activitiy of Mahanagar Gas Ltd. when the highly flammable domestic and industrial fuel is supplied through pipeline. He explained the strategy for ensuring safety of over 4.6 lacs domestic customers and over 2 lacs of CNG user for their vehicles. He elaborated their Asset Integrity through Safety of system and Design & Inbuilt Safety. He explained the safety procedures for safe operations and safe maintenance. "At the same time, he said, "the patrolling and monitoring activities are also given equal importance". Shri Sunil Nesrikar, Div. Fire Officer of Mumbai Fire Brigade presented topic "Safety in High-rise Buildings, Fire Prevention Act & Importance of Fire-drills" in an informal way. He could establish a very close contact with the audience through practical examples and anecdotes. His questions and answers session was very lively. The discussions, deleberations, information exchange of seminar and the question & answer at the time were interesting and meaningful.

CbI (Cautions before an Incident & Causes behind an Incident)Incident in Brief:

Flashover occurred while racking in SFU (switches fuse unit) feeder of MCC causing burn injury to two persons.

Sequence of Events:

• Cable termination job at SFU feeder of MCC was completed by two persons.

• After completion of the job, racking started. During this operation, shutter of vertical bus bars was creating obstruction.

• To help operation the person used non-insulated type screwdriver to open the bus bar shutter. During this action the gap between the bus bar and metallic enclosure got bridged causing flashover of the bus bar due to short circuit.

CBI (Cautions Before an Incident):

• Alignment issues during racking in operation needs discussion,

understanding and corrective actions need to be taken.• Lack of understanding of risk – Training & accurate

communication about the operation and risk involved.

CBI (Causes Behind an Incident):

• Improper selection of tools: Healthy tools should be used. In this case non-insulated type screwdriver was used for operating shutter. Use of insulated screwdriver could have avoided the incident.

• Failure to use proper safety gears, use of face shield & electrical safety gloves, could have avoided the incident.

The data available for a six year period from 2004 to 2009 reveal that the there were 3701 fatalities during this period with a progressive increase from 534 in 2004 to 651 in 2007 and then registering a decreasing trend of 622 in 2008 and 571 in 2009. Perhaps this decreasing trend could be attributed to stricter enforceent of Traffic Rules by the Police in Mumbai after increase in road accidents such as incidents involving one Mr. Pereira mowing down to death about 7 Construction Workers during late night in Brandra, few years back. Besides the fatalities that occurred during the six year period, there were 13,405 Serious Accidents on city streets were due to Driver’s faults such as rash driving, not observing the basic traffic rules, drunken driving ect. 99 accidents were due to pedestrians’ faults. Some of the sginificant revelations of the Analysis made theirin are summarised below:

• Many accidents occur during the night when the roads are empty and there are fewer policemen on the road.

• Motor driving schools, through graded by RTO according to the quality of training provided, do not provide adequate and proper training to the students.

• The trainnees strat training with hatchbacks for a short

duration and then start driving an SUV without even basic knowledge of the size and acceleration of these powerful vehicles.

• Duration of Training in driving schools range from 15 to 20 minutes per day, which is not adequate for the porpose of ensuring safety.

• Most of training schools except few like Western India Automobile Association do not have simulators for training, where in they can learn to maneuver traffic.

• Our city roads are narrow and some of them do not have adequate footpaths. Hence, the drivers and pedestrians must be aware of the conditions and speeds limits.

• Fatalities were also high among two wheeler drivers and pillion riders as they do not wear helmelts. Consequently they suffer servere head injuries from which it is very difficult to survive.

• Dumper drivers, heavy vehicle drivers drive dangerously and contribute to accidents. It is known that dumper drivers are paid per trip and so they might like to drive fast and earn more.
